Organization mission: Vista Community Clinic (VCC) provides primary health care and
health education to the community focusing on those facing economic, cultural or social
barriers. VCC operates two teen clinics that serve approximately 700 youth each month
with free, confidential reproductive health visits. The teen clinic is staffed primarily by teens
and serves as a healthcare workforce development opportunity for many local high school
students.
The purpose of the video is to introduce potential teen patients to the teen clinic and
explain what happens when they come in for a visit. It should 1) show the Teen Clinic
environment 2) explain the peer-provider model 3) explain the components of a visit
(check-in, health education, provider visit) and 4) address the core concerns of teens: cost,
confidentiality, transportation. The audience is potential teen patients and potential teen
volunteers and employees. The video will be used by outreach staff in classroom
presentations, place on social networking sites such YouTube, MySpace, Twitter and
Facebook. It will also be used for funding. It will be played on the internet and DVDs.
Vista Community Clinic will make sure that all participants have relevant consent forms
signed. The Teen Clinic is staffed with teen volunteers many of whom would be delighted to
participate in this project.

The organization requests a short video will be used by the YMCA marketing department to
tell the “Y” story to potential YMCA members and donors. The purpose of this video is to
raise awareness of the YMCA in the community and to assist in the annual support
campaign fundraising goals. The video will be watched on a large screen at their kick off
dinner, via their website, and handed out via DVD in packages for fundraising purposes.

Their Annual Support Campaign to raises funds to provide financial assistance to people
who would not otherwise be able to afford our programs. Their idea is to have a variety of
short videos that tell different people’s YMCA stories that can rotate on our website to help
share the mission of the YMCA to the community.

The Transitional Youth Academy is a program that serves at-risk youth in the tri-city
communities of Oceanside, Vista, and Carlsbad. TYA is a prevention and intervention
program offering intensive behavioral health and crisis intervention counseling, as well as
case management, educational supportive services, vocational development, and life skills
training. TYA also runs the "Working Pirates" program at Oceanside High School, which
provides academic and vocational services as well as gang prevention and intervention for
low-income, high-risk ninth and tenth graders. The main goals of TYA's youth programs are
to promote positive self-esteem and mental health, support youth in building life skills, and
help them develop the tools needed excel, prosper, and become successful in life.
The purpose of the video is to raise awareness of the program in the community, and to
serve as a fundraiser for the program. They apply for numerous grants each year and would
like to include the video in our grants application, and also to have the video on hand when
prospective donors tour the program. The organization would like a video that would
capture all the elements of the program, about 7 minutes. The video will be used by the
Development Team at Interfaith and TYA staff.
The audience for the video is prospective funders and policy makers. This includes private
and public foundations, local and federal government, private individuals, and law-makers
(most likely local City Council members, School District Boards).
The video will be watched in the following venues: on a dvd player, in large screenings on
projection, on the web, sent out in packages to fundraisers, etc.

success. The programs are designed to prepare students to become successful, selfsufficient and independent adults. The Foster Youth and Homeless Education Services
Programs focuses on improving the educational outcomes of wards and dependents, ages 4
to 21, residing in licensed children's institutions, foster family agency and county foster
homes as well as those students and families who are experiencing homelessness.
The purpose of the video is to raise awareness on the issue of youth homelessness and
would be utilized for training and awareness proposes for districts, county, and community
providers. The video would be snap shots of different successful individuals in the
community who have experienced homelessness during their youth and are now self
sufficient and successful individuals. It will capture in their eyes what made them
successful. How did education or the support they got get them to where they are today? It
will also showcase some of our homeless liaisons and community providers on their stories
of success in helping students experiencing homelessness.
The audience will be professionals such as educators & administrators and community
service providers for youth experiencing homelessness. The video will be watched on a dvd
player during trainings and awareness workshops in classrooms or other training venues &
facilities. They would like to also post the video on their county office website.
The contact requests that the video be 10 – 15 minutes, but will accept a 7 minute guideline
for the class purposes.

a community based program which provides high quality tutoring to youth currently in or
emancipated from foster care in San Diego County. These Youth are referred to the program
by social workers, case managers, court appointed special advocate (CASA), attorneys,
judges and caregivers. The program goals are two-fold: (1) to assist foster youth in
overcoming educational obstacles. (2)To inform future teachers about the unique needs of
foster youth, and provide them with information on the Child Welfare System.
Tutors in Tutor Connection are students enrolled in a teacher education course within the
College of Education at California State University, San Marcos. Curriculum includes
instruction on tutoring from an educational framework and is complemented with
information about trauma, abuse, neglect, foster care, professional boundaries, behavioral
management and the laws related to the education of students in foster care.
The video is proposed to be 5 – 7 minutes in length. The audience for the video is funders,
and community stakeholders including university and college partners. The video will
watched in classrooms on a dvd player, in large screenings on projection, at home by
families, on the web, and sent out in packages to fundraisers.
Need to determine if they have depiction releases and how youth can be pictured.
