Directions: Please answer the following questions about your mental approach to baseball. Please be
honest and specific with your answers. Your answers will help us improve your mental game. Also,
provide a brief description of your baseball background and any statistics you have kept in the last year.
1. What are your long-term goals in baseball (1-2
years)? Be specific. What interim practice or
performance goals do you need to achieve in
order to reach your long-range goals?
2. What are your biggest mental challenges that
hold you back that you are aware of? What are
your specific objectives for mental game
coaching? Please explain.
3. Rate your level of motivation to achieve your
goals (from 1-10)? Do you lack motivation? If
you think you lack motivation, what beliefs your
hold (such as: “I don’t have ability, so why
try?”) or other people hurt your motivation?
4. How do past performances, how others treat
you, or your own beliefs about your sport
performance prevent you from reaching your
goals?
5. What motivates you to succeed in baseball: (1)
the love of baseball, fun of competition, and to
achieve personal goals. Or (2) To get trophies,
praise, acknowledgement, and to gain the
respect of people in your life? Please explain.

6. Describe your attitude and your behavior when
you are in “the zone” or performing your best?
7. Describe your attitude and behavior when not
performing well? Compare this to when you are
at your best. What are the biggest differences?
8. How would you rate your level of confidence
today that you can perform well (from 1-10)? If
you do not rate yourself as a “10,” what doubts
or beliefs hold you back from having high
confidence?
9. How sturdy or fragile is your confidence? Do
you lose confidence easily after a performance,
match, or routine? What events or situations
cause you to lose you self-confidence in your
baseball?
10. How often do you “doubt” before, during, and
after each performance (0%--100% of the
time)? In what situations do you doubt yourself
the most? What thoughts do you have when
you doubt yourself?
11. (a) How often do you have lapses in
concentration during play? (b) What are your
major sources of distractions in baseball, both
internal distractions (such as negative thinking)
and external (such as others shouting during
your performance)?
12. At what times during practice and competition
are you most likely to become distracted? What
triggers your distraction?
13. When not performing up to your ability, do you
stay composed and “grind it out” to finish as
well as possible or do you lose emotional
control and interest?

14. Do you think you perform better in practice
than in competition? If so, how does your
mindset change when you go from a practice
situation to competition?
15. Do other activities in your life (school, job,
spouse, relationships, etc.) distract your from
performing your best and not allow you to focus
solely on baseball? What activities in your life
are distractions?
16. What expectations do you have before a game
that may limit your performance (i.e., Do you
expect to have a perfect performance, win
every time, or score a certain number)?
17. How often when performing better than you
expected, do you perform defensively and have
problems finishing the competition (does your
“comfort zone” prevent you from breaking
performance barriers)?
18. Does fear of failure, anxiety, or tension prevent
you from performing your best? What are your
biggest fears or what pressures affect you the
most when performing?
19. Are you able to perform relaxed and have your
performance feel effortless in competition? Or
do you over-control your body, swing, or
movements when in competition? In what
situations do you over-control?
20. How dedicated are you to training and
improving your performance by working closely
with your coach, improving your mental game,
improving fitness and health, and practicing
effectively? What is missing in your training
program?
21. What else do you want to say about your
