We respectfully forward to your office our requirements to be considered as an eligible
supplier/contractor and be deemed accredited by SUKELCO, with list to wit:
1. Company Profile;
2. List of on-going, completed or awarded contract not yet started within the relevant period
specifying the following:
a. Name of Contract
b. Date of Contact
c. Amount of Contract and value of outstanding contracts
d. Date of Delivery
e. End-User’s acceptance if completed
3. DTI Business Name Registration / SEC/CDA Registration Certificate, whichever is appropriate;
4. Valid and current Mayor’s permit/municipal license (principal place of business);
5. PCAB License if engaging in construction business;
6. Current Audited Financial Statements stamped “received” by the BIR;
7. Copy of TIN issued by BIR or copy of BIR Registration showing TIN;
8. BIR Value Added Tax Registration;
9. Statement that the bidder is not blacklisted or banned from bidding by the government or any of
its agencies, offices, corporation or LGUs, and other private corporations or electric
cooperatives; including non-inclusion in the Consolidated Blacklisting Report issued by the
Government Procurement Policy (GPPB), as provided in Section 69.4 IRR-RA 9184;
10. Compliance with Executive Order No. 398:
a. Proof of VAT payments for the past six months;
b. Tax Clearance for the last two quarters; and
c. A Certification under oath from the bidders’ responsible officers that the bidder is free and
clear of all liabilities with the government.
11. Accreditation Fee of Php 500.00 (non-refundable). Pay at our Cashier and photocopy O.R.;
12. Authorization from the Manufacturer to Supply Goods to the Bidder, if applicable; and
13. Notarized Authorization if transacting through a representative;

BDO Tacurong City. Please email scanned deposit slip (include company’s name for O.R.
preparation) to acepedroso@yahoo.com.
Please submit the above-stated requirements in a folder (indicate the company name in front ,
labeling or numbering each requirement in the margins) to:
