Change of name or address/Lost, Stolen, or Mutilated
The following guidelines are provided to assist you when applying for a duplicate Firearms I.D. card.
1.
Take care in filling out the application, Form STS.033. All questions must be answered. Be sure to check the appropriate block indicating the reason for your application.
2.
If you are applying for a duplicate card for any reason other than lost or stolen, you must surrender your old card before a new one will be issued.
3.
Sign and date only
, the yellow “Request for Criminal History Record Information”,
Form SBI 212A. This form, along with the $18.00 fee will be sent to the State Police for processing. This fee MUST
BE in the form of a money order or cashiers check made payable to the Division of State Police, S.B.I. Personal checks and/or cash will not be accepted. (The NJ State Police are no longer accepting money orders from the Peapack-
Gladstone Bank.)
4.
Complete Part One of the Consent for Mental Health Search, Form SP-66. Do not sign this form until you are at headquarters. Someone will witness your signature.
5.
If you choose to have your new I.D. card mailed to you, please request to sign the new card and have your index finger printed on it, at the time you present this paperwork at headquarters.
6.
Your new card will be mailed to you, or you will be advised that the card is ready in approximately 2 to 3 weeks.
