AMENDMENT to the AGREEMENT
between
THE UNITED NATIONS
Office for the Coordination of Humanitarian Affairs (OCHA)
And

Whereas the United Nations Office for the Coordination of Humanitarian Affairs (hereinafter
referred to as “OCHA”) and [Name of recipient Agency] (hereinafter referred to as "the Agency”),
on the basis of mutual trust and in a spirit of friendly cooperation, have agreed to Amend the
Original Agreement governing funding for [“Emergency Up-scaling of CMAM Support in
Collaboration with Partners in Sindh” (ERF-DMA-O487-074)] as follows:
1. Section A. Project summary – Time frame

The terms of the present agreement shall expire on [Day.Month.Year].
2. Section B. Substantive Report
Paragraph 1: It has been mutually agreed to revise the MOU as follows:

Within three (3) months after the Grant has been fully expended or the Project has
been completed, whichever is first to occur, [Name of recipient Agency] shall provide a
final substantive report detailing achievements, constraints, and impact with regard to the
utilization of the Grant for the Project. This final substantive report shall be provided by
[Day.Month.Year]. The final substantive report should reflect the utilization of the Grant
in the context of the overall contributions to the project.
3. Section B. Financial Report
Paragraphs 3, 4 and 5: It has been mutually agreed to revise the MOU as follows:

Once the Grant has been received into the [Name of recipient Agency] account,
[Name of recipient Agency] shall provide OCHA with Interim Financial Statements listing
amounts received and expended from the Grant utilizing the standard “Financial Statement
on Income and Expenditures for Funds Allocated from the Trust Fund for Disaster Relief”.
The Interim Financial Statement reflecting expenditures from the inception of the project to
30 November 201X shall be due by 31 December 201X. This Interim Financial Statement
shall be signed by the [Title of duly authorized designated official of recipient Agency].

In addition to the interim reporting requirements set out above, [Name of recipient
Agency] shall provide OCHA with Annual Financial Statements listing amounts received
and expended from the Grant utilizing the standard “Financial Statement on Income and
Expenditures for Funds Allocated from the Trust Fund for Disaster Relief”. The Annual
Financial Statement reflecting expenditures from the inception of the project to

31 December 201X shall be due by 30 June 201Y. This Annual Financial Statement shall
be certified by the [Title of duly authorized designated official of recipient Agency].
5.
Upon completion of the project, [Name of recipient Agency] shall provide OCHA
with a “Final Financial Statement on Income and Expenditures for Funds Allocated from the
Trust Fund for Disaster Relief.” This final financial report reflecting expenditures between
the inception of the project and the completion of the project shall be due six months after
the closing date of [Name of recipient Agency]’s financial period in which the project is
completed, in any case not later than [Day.Month.Year], and shall be certified by the [Title
of duly authorized designated official of recipient Agency].
ALL OTHER TERMS AND CONDITIONS OF THE ORIGINAL AGREEMENT
REMAIN UNCHANGED.
IN WITNESS WHEREOF the undersigned, being duly authorized thereto, have on behalf
of the Parties hereto signed the present Agreement at the place and on the day below written.
