Student, please read and initial:
1. ______Dean’s office will keep a copy of this form on file and send a copy to the
Registrar. The Registrar will code the minor into the registration system.
This change will become effective at the end of the current semester.
2. ______The student is expected to see the departmental advisor of the minor for
selection of courses.
3. ______At the beginning of the final semester before graduation, student must fill
out a Certification of Minor Form, take it to the minor department for
verification and signature.
