REGISTRATION FORM - SPRING 2014
A student who is transferring from an in-state nonpublic school or a home schooling program or a * Louisiana resident who is transferring from an
out-of-state school to enroll in the Louisiana public school system at grades 5 or 9 shall be required to take the 4th or 8th grade LEAP English
Language Arts AND mathematics tests. Required achievement levels: LEAP - 4th and 8th Grades- A minimal of BASIC on ELA or Math and
APPROACHING BASIC on the other. (SUBJECT TO CHANGE BY LDOE)
Column A - All nonpublic school students and any Louisiana resident who is transferring from an out-of-state school who wish to take
the LEAP Tests (Grades 4 or 8) must complete the appropriate registration information below in Column A.
OR
Column B - Home schooling students or returning non-graduates who wish to take the LEAP Tests (Grades 4 or 8), GEE Tests (Grades
10 or 11), or iLEAP Tests (Grades 3, 5, 6, or 7) must complete the appropriate registration information below in Column B.
