After completing her first full year living in her Living Well Disability Services

For the first time in his young life, Pao is able to move freely with a power wheelchair
and is learning how to communicate his preferences with assistive technology. Pao
is a big fan of his “road trips” that include volunteering at a MN RollerGirls event and

Joanne’s life changed dramatically when she moved from a state institution to the
support of Living Well Disability Services in 1990. Joanne now enjoys an active
retirement, assisted by staff members who honor her unique desires and interests.

About Us
Living Well Disability Services delivers exceptional services that transform the lives of people impacted by
intellectual, developmental and physical disabilities. Founded as Dakota’s Children in 1972 in West St. Paul by
parents, volunteers and community partners, we are a private 501(c)3 nonprofit organization that today provides
person-centered Customized and Residential Services to people living in the region.
Services are available to people regardless of where they live: in their own home, their family’s home or in one
of the 33 group homes operated by Living Well Disability Services. Innovation in service, accessibility, wellness,
and technology helps people pursue their goals and live as independently as they desire. Services are based
on the choices of each individual and developed to support their unique needs, hopes and interests.
Dakota Communities changed its name to Living Well Disability Services on August 1, 2013.

Julie Manworren recognized Maggie Miller (center)
with the 2014 Dreammaker Award for Community
Partnership. Maggie has served people with
disabilities as a Dakota County Social Worker
for more than 34 years. She has had a profound
impact on the lives of many people we support
including those of our founding families.
The following pages of our 2014 Annual Report include a selection of
accomplishments that illustrate how we impact the lives of individuals we
serve today. At the same time, our progress in 2014 positions us to serve
more people in the future, wherever they choose to live; their own home,
their family’s home or one of our 33 group homes.
Thank you for your continued support. We rely on every family member,
employee, volunteer, donor, advisor, and friend. As leaders in serving
people impacted by disabilities, we continue to learn and grow in a
changing environment. We invite you to participate with us as we continue
to innovate and lead change to serve more Minnesotans in the Living Well

Exceptional Services
Our mission statement sets the quality standard for our services to people
impacted by disabilities as exceptional. Since our beginnings in 1972, we have
listened and responded to self-advocates, family members, and the community
to make a noticeable and measurable difference in the lives of people we serve.
In 2014, our impact is illustrated by accomplishments that define the difference.
Quality of Care
We deliver and measure care based on
best practices that reflect the desired
outcomes of each individual. Our employees
have over 3600 years of combined tenure.
•
We defined and introduced Continuous Quality Learning (CQL)
outcomes into the annual planning
process to measure whether lives are transformed in meaningful ways.
•
90% of self-advocates indicate that
they are able to express themselves
and feel that their thoughts and ideas are being heard.
Complex Medical Conditions
We are recognized for our unique ability to
support people who have fragile medical
conditions complicated by multiple
diagnoses and aging.
• In 2014, doctors reported that 173 of
191 (91%) people they examined
experience good to excellent health.
•

We launched an in-home medical care service through BlueStone Medical
Group, offering people the choice to receive basic medical services at home, which we expect will improve overall health care and provide a quicker response for chronic medical issues.

Self-Advocate and receives
customized services to
support his choice for
independent living.
Culture of Wellness
Peggy told her story,
Peggy’s Dreams: Living
life with Down syndrome,
through a national radio
and online broadcast.
The story is an intimate
portrait of one woman
and her family, but it also
speaks to a broader story
about society’s approach
to Down syndrome.
Innovation in wellness programming
creates opportunities for people to be
more active, engaged and healthier
across their lifespans.
• 137 self-advocates reported improved physical health in 2014.
• 181 self-advocates report regular
engagement with people in the
community.
• 85 self-advocates report a reduced sense of isolation and loneliness.
•
As a founding member of the Altair Accountable Care Organization (ACO),
we shared our wellness program with
ACO members including Lutheran
Social Services, Hammer and Mount Olivet Rolling Acres.

vegetables and herbs are
a mainstay of recipes in
our healthy menu system
available to all people
we serve who live in our
group homes, their own
or their family’s home.
• We shared wellness best practices
with the Minnesota Department of
Human Services (DHS), ARRM and representatives from the State of South Dakota Department of Human Services and private providers through
consultation.

Oh, how Bob loved his sports! Any sports, but particularly the Twins. Above all
else, Bob loved his mom and dad very much.

A very sweet woman known for her love of chocolate and a good cup of coffee.
Julie loved having her nails painted. Red nails were a must! Julie had the best
smile and smiled all the time.
She had a passion for music and will always be remembered for her rendition of
“You Are My Sunshine.”
Ruth Schilling Reid Anderson
We join families and friends in
remembering those who passed away in 2014.
Ruth was a cheerful, optimistic woman who enjoyed family, old movies and
thumb wrestling.
Reid was a dedicated family man who enjoyed sports, especially football;
in his own words, “I bleed purple.”

2014 by the Numbers
We work towards our vision of a day when
all people impacted by disabilities live well
by extending our services to new families
and continuously evolving our operations
to meet the diverse needs of people where
they choose to live.
• 1,428,197 hours of direct support
were provided to people
• Our Customized Services team
increased services to people who live in their own or their family’s home by 16%.
Working together, we delivered
exceptional service to people
we supported throughout
2014 and strengthened our
knowledge and capabilities
to serve more Minnesotans
in the future.
• 98% of case managers would
recommend us to families
• 184 volunteers provided 4,100 hours
of service
• 30 Health and Nutrition classes were
held in the 10-county region
• 12,409 training hours were provided
to employees
• 113 new employees were referred by
current employees
•
50 employees were promoted
• $558,948 total value of gifts were
contributed, including in-kind, from individuals and organizations, and committed grants
deferred for 2015.

Team Victoria kicked off the inaugural Pursuit For Living
Well race in 2014 at Normandale Community College in
Bloomington. The August 13, 2015 Pursuit For Living
Well 8K and 1K will welcome runners, push rim
wheelers and athletes of all abilities.
