primary ICD-9 diagnosis codes V72.31, V70.0, or V70.9 as the PRIMARY diagnosis with ANY procedure, will CAUSE THE ENTIRE CLAIM TO BE DENIED.

other insurers. It includes the most commonly found cytology ICD-9-CM codes, but is not complete. It was compiled from the 2013 ICD-9-CM code book, which
