It is the mission of Portland Beauty School to provide an opportunity to achieve a
superior level of education, training, and ethical standards to be applied in the
fields of Hair Design, Esthetics, Nail Care, Barbering, & Massage Therapy. Our

A Message from the School Director
Welcome to Portland Beauty School!
It’s time to realize your dream as a salon professional! Get ready to launch your career
with an in depth training in hair, nail, skin care, and/or massage therapy. 18 Years ago,
Portland Beauty School was founded, and has since been committed to creating many
successful entrepreneurs in the industry. You’ll start by learning the fundamentals of
your new career and expressing your creativity and talent in our unique clinic experience.
Our mission is to provide students with an opportunity to achieve an unsurpassed level
of education and training, while developing the professional skills and ethical standards
that are required for succeeding in the arts and sciences of Cosmetology and Massage
Therapy. Upon completion, you’ll be ready to succeed as a licensed Cosmetologist or
Massage Therapist.
Competency based learning allows you the power to customize the pace of your
Cosmetology education, matching you as an individual. This exclusive approach to
learning may allow you to confidently begin your career at a rate faster than traditional
clock hour schools. By recognizing your individual approach to learning, you can control
your advancement and progression through your career training. The school also offers
Massage Therapy and Cadet Teacher Training which are clock- hour programs. Portland
Beauty School is a pioneer in competency based education in the Portland Metropolitan
Area. Portland Beauty School’s graduating students will be granted a certificate of
completion and be qualified to take the Oregon State written examination. Students
pursuing a cosmetology career will receive an Oregon Practitioner’s Certification
immediately upon passing the exam at the Oregon’s Health Licensing Agency and
students pursuing massage therapy career will receive a license through the Oregon
Board of Massage Therapies upon passing their examinations with OBMT.

Campus Description
Portland Beauty School’s campus is located on the corner of Northeast 82nd Avenue & Siskiyou
Street. Our school is easily accessed by either the I-205 or the I- 84 Freeways. The Administrative
Office is conveniently located on the school campus.
Our brand-new campus is housed in a single story building. Our day spa-like facility includes four
academic classrooms used for theory classes. These classrooms are used for all levels of students
including freshman, junior, and senior students. The classrooms will accommodate approximately
16-24 students (actual classroom size varies on enrollment). Unfortunately, student housing is
not available.
The large and modern clinic floor is where the practical applications of Hair Design/Barbering,
Massage Therapy, Esthetics and Nail Care are performed. Separating the clinic floor and training
classrooms is our lobby/reception area.
We offer a kitchen/lunch room for our students. This room consists of a refrigerator and 3
microwave ovens for their use. There is also a full service restaurant located within our Campus
Plaza. Three restrooms are available for student, faculty, and client use. Student lockers are
available and are issued on a first come basis.
There is also a media library in which students may rent out material for at home study. This library
is available in the administration office. There is a hold fee of $10.00 to rent any of the items. Once
the item rented has been returned the fee will be reimbursed. If the item is not returned with in a
30-day period, the fee will not be reimbursed.

All Instructors are required to meet educational qualifications in order to obtain an
Instructor’s Registration issued by the Higher Education Coordinating Commission.
Instructors are required to have 30 hours of Continuing Education every two years to
renew their license. Our Instructors are talented with many years of experience in the
industry. We evaluate each Instructor’s performance to ensure educational excellence.

Nail Technology- Competency Based
Get
ready to explore the world of manicures,
pedicures, and artificial nail application
techniques. You can choose to specialize in
natural nail care or to become a nail sculpture
artist. You will learn advanced spa techniques
used in today’s high end salons. This program
prepares you for all artificial nail applications
including acrylic, gel, silk, fiberglass, and nail
forms. The Nail Care Program is designed to
help you learn nail diseases and disorders, hand
and foot massage, anatomy and physiology,
chemistry, and various aspects of nail care and
attention. The Nail Care course is specifically
designed to help you succeed as a licensed Nail
Technician. As thousands of our graduates can

European spa facials, makeup artistry, and body

Your Esthetician training will bring you the

Massage Therapy- 600 Clock-Hour Program
Portland Beauty School offers a 600 hour
program approved by the Oregon Board
of Massage Therapists. This professional
program is designed to provide a solid
framework in the basics of therapeutic
massage. Graduating students will be
prepared to take the Oregon state
licensing exam and the National
Certification Exam.
The course is divided into two terms.
Each term is designed to take students
from a basic understanding of practice
and theory to a more advanced level of
learning that includes in depth knowledge
of professional practice standards, ethics,
and the business of Massage Therapy.
There is a one week break between
terms. Make-up hours and tutoring is
available to students. Speak to Director
or Instructor to make arrangements for
make-up hours.

Cadet Instructor Training- 1000 Clock-Hour Program
Cadet Instructor Training is the first step
to inspiring new generations of beauty
technicians. In this program you will learn
all the information needed to instruct
within the beauty industry. You will work
in real classroom settings and with
students on the clinic floor. You will
acquire the skills needed to be able to
lesson plan, manage a classroom, and
building a curriculum. While working on
the salon clinic floor you will learn how to
access
students,
communicate
effectively with students and team
members,
and
professional
presentational skills.
This is a clock-hour program and will
need to complete 1,000 hours of
instruction to complete this program.
This program is broken down into two
areas. Classroom(C) is instruction
completed in the classroom and Floor(F)
is instruction completed on the clinic floor.

Career Development
As a part Hair, Skin, Barbering and Nail programs taken at Portland Beauty School, you will
be trained in Career Development. This will prepare you for entering the workplace in your
new career, concentrating on your professionalism, customer service, and Business practice
skills.
Students are expected to perform Salon Cleanliness procedures throughout the school which
will be assigned to students on a weekly basis.
Supplies are provided by our School during instruction.
Safety & Sanitation
As a part of Hair, Skin, Barbering and Nail programs chosen at Portland Beauty School, there is
a requirement of Safety and Sanitation. This is required by Oregon Law and is mandatory training
towards graduation from Portland Beauty School. This will involve training in Decontamination
and Infection control, Salon Cleanliness, Disinfection, Client Safety, Practitioner Safety, and
overall Oregon Law requirements. Each student enrolled will be expected to perform Sanitation
Duties expressed in Oregon Law and Administrative Rules. You will also learn OSHA hazardous
chemical information. The daily sanitation procedures will need to be performed prior to, during,
and after the services that students perform. This course will include Technical Training and
Hands-on Experience. Each student will be tested during and prior to graduation that they
comprehend the importance of work place safety and sanitation.
Supplies are provided by our School during instruction.

school offers student placement and career services. The school maintains placement services

The following graph contains a listing of our career directed programs, the average time required
to graduate and achieve professional skill level, as well as the cost associated with each career
path choice. Portland Beauty School is committed to your success as a salon professional, and
dedicated to assisting you with choosing the career path that is right for you. Our Administrative
Department is dedicated to assisting you to make the right financial, academic, and vocational
choices that best suit your career goals. Tuition costs are based on the applicable minimum
hourly training requirements.

*Registration Fee is waived when adding a program with any other course
** Students that combine hair and nail programs will only receive one book. There is a reduction
in book costs.
Students enrolled in a competency-based program can remain enrolled after the school
determines they are competent. Students enrolled in competency-based programs may remain
enrolled in those programs without incurring any additional costs until they have “been trained for
an amount equal to the” minimum hourly training requirements established in OAR 581-0450200(1).
Policy on Extra Instructional Charges
Portland Beauty School will charge $7 per Clocked Hour of extra instruction beyond the minimum
hourly training requirements (contract end date). Portland Beauty School will only charge a fee
prior to the completion date appearing on the enrollment agreement if we have secured from the
student an acknowledgment of credit to their account. All monies received for extra-instruction
prior to completion of the student contract are refunded if the student terminates. Once a student
met the maximum hourly time established by Portland Beauty School, the student is no longer
eligible to receive additional instruction and will be dismissed, if the student has not been declared
competent (competency-based programs) or satisfied the applicable graduation criteria(clockhour programs). See Satisfactory Progress Policy for Maximum Time Limits.

Total deposit is due before the start of any program. If the student wants to move seamlessly through each
of their programs, the total deposit due is the combined deposit. If a student would rather pay the deposits
as they go through their programs, they have the option to pay the deposits prior to the start of the each of
their additional programs.
Example
If a student is enrolled in Hair, Nails, and Esthetics and the first program started is Nails the deposit due to
start classes is $500.00. If that student then wants to enter esthetics, they will need to pay another deposit
installment of $700. When that student wants to start their final program, Hair, they will need to pay another
deposit installment of $800. The balance of the full program(s) must be paid prior to the release of
transcripts. This deposit is not included in the monthly minimum payments the student is currently making.
Minimum monthly payments are 0% interest. There is a 10% price reduction from the total cost to any
student that pays their balance in full prior to enrollment.
Transcripts will not be released to any persons, state or institution until balance is paid in full.
Students have the options of making payments with Visa, MasterCard, Discover, American Express,
Cash, or Check.
Fee Waiver Policy
Portland Beauty School does in some cases grant fee waivers. This is at the sole discretion of Portland
Beauty School. Economic hardship, immediate family death, and medical illness are examples of cases in
which a fee waiver may be granted. There are no forms associated with this policy however, additional
information may be needed (such as a tax form or medical note) in order for a fee waiver to be granted.
Release of Transcripts Prior to Contract Completion
If a student would like transcripts to be released as they complete each program, the portion of tuition for
that program must be paid. There will also be an additional $100 administration fee per transcript released
that must be paid prior to the release of transcripts. If the student waits until all programs are complete, the
administration fee is waived.
Student Technical Kits and Books
Students have the option of purchasing Technical Kits and Books from a source other than Portland Beauty
School. Technical Kits and Book lists are provided at the request of any admitted student. Technical Kits
and Books are required to begin instruction. Any student that provides materials from an outside source will

Admissions Policy
Portland Beauty School welcomes students who wish to pursue a Post-Secondary-level
program of study. It is our goal to make the admissions process an easy one to follow.
Service to students is our priority, and academic achievement is expected, nurtured, and
rewarded.
Portland Beauty School may admit any person to our cosmetology programs, as a
student, who meet the following criteria and provide the following documents:
 Must be the minimum age of 18 years old.
 Provide a valid Driver’s License or Picture Proof of Identification(any governmentissued id)
 Social Security Card or Tax Identification Number (used for school transcripts
which are sent to the Oregon Health Licensing Agency for licensure. This
information is protected and not shared with any other sources).
 High school transcript, college transcript, or a passing GED score or Certificate.
Students without a High School Diploma or G.E.D will be required to take the
Entrance Exam (passing score is 75%). The Entrance Exam is an oral and written
list of questions that will be given prior to admission to Portland Beauty School.
The Entrance Exam measures word comprehension, basic math, and
communications.
After the above items have been received and if needed, verified for authenticity, an
applicant will be notified verbally within 30 days of admission to the school. The school
will inform an applicant via phone, email, or in person. If for any reason Portland Beauty
School denied admission to any applicant they will be informed in writing of the denial
and the reason for denial within 30 days. Any applicant that was denied admission will
be able to re-apply pending a resolution of reason(s) of denial.
Admission Fees
Portland Beauty School does not have any admission fees.
It is NOT the policy of Portland Beauty School to recruit students away from other
schools.
Enrollment Agreement
Once a student, is admitted into Portland Beauty School, a contract entitled “Enrollment
Agreement” is to be signed by the student (*parent or legal guardian when necessary)
and the Director of Portland Beauty School. This agreement will delineate the course of
study and the requirements for course completion. After being understood and signed by
all parties, the original agreement will be retained in the student’s personal records file
and a copy will be provided to each student.

Student Personal Files and Records Policy
Portland Beauty School will prepare and maintain an accurate and confidential file for
each student to be kept in the Administrative office. Portland Beauty School will maintain
student records, and provide access to such records in accordance with Oregon
Administrative Rule 581-045-0018. Any requests made by a 3rd party for records is
received, a written and signed authorization from the respective student must accompany
the request, in order for records to be released to the 3 rd party. The student may review
their file by appointment or written request, a written letter of intent, under the supervision
of the Director.
Statement of Non-Discrimination
Portland Beauty School does not discriminate on the basis of race, sex, religion, age,
handicap, color, sexual orientation, marital status or ethnic origin in its education
programs, admissions, instruction, graduation policies, or any other activities in which it
operates and is prohibited from discrimination in such manner by law. Any persons
unlawfully discriminated against, as described in ORS 345.240, may file a complaint
under ORS 659A.820 with the Commissioner of the Bureau of Labor and Industries. Any
instructional staff or other school personnel have been found to have engaged in
discriminatory behavior towards any affiliated with Portland Beauty School, will be subject
to the schools personnel policies, and could lead to immediate termination.
Student Grievance Policy
Students who feel aggrieved by the action of a Portland Beauty School, student or
employee, should first attempt to resolve any issues with the Educational Coordinator. If
the grievance is with the Education Coordinator, the student can go directly to the school
Director. In the event this procedure should fail, students should submit the nature of the
complaint in writing, including names or positions of staff involved, to the Director within
30 days of the occurrence. If this process still should fail and students still feel aggrieved,
students can then contact: Oregon Higher Education Coordinating Commission, 775
Court St, NE Salem, OR 97301 or telephone(503) 947-5751 or the School Director.
Placement and Counseling Service
Portland Beauty School will provide referrals for general counseling and other social
services to appropriate agencies within our community, such as the Housing Authority of
Portland, Multnomah County Mental Health Department, the Schools Uniting
Neighborhood (SUN) programs for services such as housing, basic needs (food, clothing,
utilities), mental health and domestic violence. These services are not provided by
Portland Beauty School; however, Portland Beauty School maintains an active
relationship with above mentioned agencies as well as the Northeast Portland
Neighborhood Accountability Board, and the Rose City Neighborhood Association.
These agencies work in conjunction with Multnomah County Family and Community
Services Department, and can provide direct services referrals for students and their
families. Students who need these services should speak to either their course instructor
or the School’s Administrative Office staff for a direct referral to these agencies. This
information is kept confidential to the fullest extent of law.

Attendance Policy
To help prepare you for the workplace, Portland Beauty School operates similarly to a
professional salon and spa. Late arrivals, absences and other interruptions in your training have
a significant effect on your ability to gain skill and competence, in the same way they would if you
were employed in a salon or other professional environment. According to the Higher Education
Coordinating Commission, we must keep track of your training hours for licensure or certification.
Attendance is determined by the contracted schedule. Full-Time is defined at a minimum of 7.5
hours per day or 135 clocked hours per month. Part-time is defined as hours totaling less than
7.5 hours per day or 135 clocked hours per month.
In order to maintain satisfactory progress, each student must attend scheduled classes during
each calendar month. Students must be clocked in via a computerized attendance tracking
program prior to their start time. All students are expected to attend school regularly and maintain
a minimum of 75% of the contracted schedule. Students are required to be prepared for school
every day. This includes having all tools required for training including but not limited to technical
kits and instructional books. If a student comes to school without the required tools, they may be
sent home and receive no hours or credit for the day.
If a student is ill or an emergency arises and will be absent or late, the student must notify the
school directly by calling the front desk, prior to their contracted start time. If a student does not
contact the school prior to an absence or late arrival, they may not receive credit for the day. Any
planned absences must be brought to the instructor’s attention before the date of the absences,
and documented in writing to the instructor. Any absences of 3 days or more will need to be
accompanied by a note from a certified medical physician, excusing the absence.
If the attendance requirement is not met, the student will receive a verbal warning. At this time
the student will be counseled as to how they can make up the theory and practical classes missed.
If the attendance requirement is not met after the second month, or if there is no attempt to make
up theory and or practical classes the student will no longer be considered as maintaining
satisfactory progress.
If the student requires a leave of absence for medical reason, or for circumstances beyond his/her
control, the school will permit up to 90 days leave. The student must request the leave in writing
to the Director. If the student fails to return to the school from a leave of absence, he/she will then
be terminated.
Late Arrivals
A late arrival is defined as more than 15 minute late for regularly scheduled class. If a student is
excessively late (being late more than 1 time per week), an advisement session with the Education
Coordinator will determine what actions should be taken by the student and the school to reduce
tardiness. Tardiness will lead to suspension if the tardiness happens more than 7 times in a
month. Late arrivals and early departures will count against the student.

Disciplinary Actions
A student may only acquire 3 absences or 7 late arrivals per month excused or unexcused.
The student will be verbally advised by the Education Coordinator after 2 absences and/or
if a student is late more than once a week. If the student is still absent or late after the
advisement, the following will take place:
1st Offense: Verbal Warning- A verbal reprimand and will review over further disciplinary
actions that will take place if another offence should take place. (If the student acquires 3
absences or 7 late arrivals per month excused or unexcused)
2nd Offense: Written Warning/Probation - A written warning, which states that further
violation(s) within a 30 day period will result in suspension. Specific stipulations may be
prescribed in this probationary period. (If the student acquires more than 3 absences or 7
late arrivals per month excused or unexcused)
3rd Offense: Suspension - Immediate suspension of the student from Portland Beauty
School. This suspension will be a maximum of 30 days (If a student acquires any absences
or late arrivals while on probation).
4th Offense: Termination – Once a student returns from suspension they will be placed on
a probationary period of 30 days. If the student acquires any absences or late arrivals while
on probation, they will be terminated. This is the immediate withdrawal of the student from
Portland Beauty School.
All disciplinary actions will be documented in the students file.
Previous Enrollment
Any student who has previously attended Portland Beauty School, and wishes to reenter/re-enroll/restart their program of study, must execute a new Enrollment
Agreement. If accepted for enrollment, the student will be subject to the most current
rules that apply to the Enrollment Agreement if revisions apply. Portland Beauty School
reserves the right to deny re-entry to any student previously terminated from this
School. In the instance of such decision, the student shall be notified by mail.

Satisfactory Progress Policy
Satisfactory progress and attendance academic work is a requirement for all students enrolled
at Portland Beauty School. Any students enrolled with Portland Beauty School must maintain a
minimum 75% for academics and attendance.

If a student fails to reach a ‘competent’ level before the maximum time is reached the student
will be dismissed from Portland Beauty School. Maximum hours is based on 200% of hourly
minimums. Students enrolled in competency-based programs receive more time due to
graduation being based on a student’s ability to reach a competent level.

If a student fails to meet required training needs before the maximum time is reached the student
will be dismissed from Portland Beauty School.
Clock hour maximums are based on 125% of hourly minimums. Clock-hour programs have a
lower maximum time due to students are required to complete clock-hours.

All students must maintain at least 75% cumulative attendance average in
order to be considered making satisfactory progress and complete the program within the
maximum time frame.
ACADEMIC PROGRESS
The following factors will be measured in determining academic progress:

DETERMINATION OF PROGRESS
Students are able to request a progress report at any time during enrollment. A Satisfactory
Progress Report will be given once per quarter. Students meeting minimum requirements at
evaluation will be considered to be making satisfactory progress until the next scheduled
evaluation. In order for a student to be considered making satisfactory progress, he or she must
meet both attendance and academic minimum requirements. If a student has a challenge with
the information provided on a progress report, the student can, in writing, make a request for the
information to be corrected. The request will be granted or denied within 10 school days. The
student will be informed of any changes or denial of changes verbally within 10 school days.
Satisfactory: Students with a minimum of 75% GPA in each course and 75% attendance.
NOTE: Attendance is evaluated on a cumulative basis. At each evaluation point, the attendance
for the month will be added to attendance from the preceding month to determine whether the
student will complete the course within the maximum time frame established in this policy.
Progress Reports are monitored monthly and Satisfactory Progress Reports are calculated every
quarter. Evaluation times are program based.
PROBATION
If a student is not maintaining satisfactory progress they will be placed on a warning until the
following Satisfactory Progress Report. If a student fails to maintain satisfactory progress for two
consecutive report periods, the student will be placed on probation. If a student fails to maintain
satisfactory progress more three consecutive periods, the student may be terminated. After each
unsatisfactory report, the student will have a meeting with the school Educational Coordinator to
determine the cause and resolution for the student’s success in their enrollment.
LEAVE OF ABSENCE
Students returning from a leave of absence or other official interruption of training must return to
school in the same satisfactory progress status they had prior to their departure. Request forms
are obtained from the school Director.
APPEAL PROCESS
Students who are terminated after failing to achieve minimum requirements may appeal this
determination. The student must submit a written appeal to the School Admissions Director along
with any supporting documentation, reasons why the decision to terminate should be reversed,
and request for re-evaluation of progress. The School Director, within (5) business days of
termination, must receive this appeal. Should a student fail to appeal this decision, the decision
to terminate will stand.
An appeal hearing will take place within (5) business days of the receipt of the written appeal.
This hearing will be attended by the student, the student’s instructor, and the School Director. A
decision on the student’s appeal will be made within three (3) business days by the school Director
and will be communicated to students in writing. The school’s decision will be final. Grievances
may be filed.
Should a student prevail on his or her appeal and be determined as making satisfactory progress,
the student will be automatically re-entered in the course, and financial aid funds will be reinstated
to eligible students.

WITHDRAWALS
Any student who withdraws from his or her contracted course or fails to complete his or her
training will have a notice placed in his or her student file as to progress at the point of withdrawal.
A student making satisfactory progress at the point of withdrawal may apply for re-enrollment in
the school and will be considered to be making satisfactory progress at the point of re-entry. A
student failing to meet minimum satisfactory requirements at the point of withdrawal will be placed
on probation for the next quarterly evaluation when accepted for re-enrollment. Any student that
does not contact the school with a formal letter of withdraw within 1 year of the last date attended,
will be manually dropped from the program. If a student is manually withdrawn, any monies owed
will be given after a meeting with the director to discuss the reason for withdraw.
COURSE INCOMPLETES/REPETITIONS/NON-CREDIT REMEDIAL PROGRAM
Incompletes, repetitions, or non-credit remedial courses have no effect upon a student’s
satisfactory progress status in this school. Portland Beauty School offers refreshment courses in
all subjects offered. These are non-credit courses used to refresh or enhance a licensed
professional’s previous education.
CANCELLATION AND REFUND POLICY – OAR 581-045-0036
(1)A student may cancel enrollment by giving written notice to the school. Unless the school
has discontinued the program of instruction, the student is financially obligated to the school
according to the following:
(a)If cancellation occurs within five business days of the date of enrollment, and before the
commencement of classes, all monies specific to the enrollment agreement shall be refunded.
(b)If cancellation occurs after five business days of the date of enrollment, and before the
commencement of classes, the school may retain only the published registration fee. Such fee
shall not exceed 15 percent of the tuition cost, or $150, whichever is less;
(c)If withdrawal or termination occurs after the commencement of classes and before
completion of 50 percent of the contracted instruction program, the student shall be charged
according to the published class schedule. The student shall be entitled to a pro rata refund of
the tuition when the amount paid exceeds the charges owed to the school. In addition to the
prorated tuition, the school may retain the registration fee, book and supply fees. Students
(d)If withdrawal or termination occurs after completion of 50 percent or more of the program,
the student shall be obligated for the tuition charged for the entire program and shall not be
entitled to any refund;
(e)The enrollment agreement shall be signed and dated by both the student and the authorized
school official. For cancellation of the enrollment agreement referenced in Subsections 1 (a) and
(b), the date of enrollment will be the date that the enrollment agreement is signed by both the
student and the school official, whichever is later.
(2)Published Class Schedule (for the purpose of calculating tuition charges) means the period
of time between the commencement of classes and the student’s last date of attendance as
offered by the school and scheduled by the student and other legitimate charges owed by the
student;

(3)The term ―Pro rata Refund‖ means a refund of tuition that has been paid for a portion of the
program beyond the last recorded date of attendance.
(4)When a program is measured in clock hours, the portion of the program for which the student
will be charged is determined by dividing the total clock hours into the number of clock hours
accrued according to the published class schedule as of the last date of attendance. For
Competency- Based schools, the period of time in the student’s program is based on the
minimum hourly training requirement established in OAR 581-045-0200.
(5)When a program is measured in credit hours, the portion of the program for which the student
will be charged is determined by dividing the total number of weeks into the number of weeks
accrued according to the published class schedule as of the last date of attendance.
(6)For other measurements of time such as days or weeks, the portion of the enrollment period
for which the student will be charged is determined by dividing the total number of days or weeks
into the number of days or weeks, accrued according to the published class schedule as of the
last date of attendance.
(7)The term "tuition cost" means the charges for instruction including any lab fees. ―Tuition
cost does not include application fees, registration fees, or other identified program fees and
costs. The school shall adopt and publish policies regarding the return of resalable books and
supplies and/or the prorating of user fees, other than lab fees.
(8)The school shall not charge a withdrawal fee of more than $25.
(9)The school may adopt and apply refund calculations more favorable to the student than those
described under this policy.
(10)When a cancellation, withdrawal, termination, or completion occurs, a calculation of all
allowable charges under this rule shall be made. If such calculations evidence that the school
received total payments greater than its allowable charges:
(a)Within 40 days after notification of such cancellation, withdrawal, termination, or completion,
a written statement showing allowable charges and total payments received shall be delivered
to the student by the school, together with a refund equal in amount to monies paid to the school
in excess of those allowable charges;
(b)In the event payments to a student account are derived from federal and/or state tuition
assistance program(s), including student loan programs, regulations governing refund
notification and awarding within respective program(s) shall prevail in lieu of Section (10) (a) of
this rule, but only with respect to the covered portions thereof; and
(c)In the event payments to a student account are derived from a sponsoring public agency,
private agency, or any source other than the student, the statement of charges and payments
received together with an appropriate refund described under section (10) (a) of this rule may be
delivered instead to such party(ies) in interest, but only with respect to the covered portions
thereof.
(11)In case of disabling illness or accident, death in the immediate family, or other
circumstances beyond the control of the student that causes the student to leave school, the
school shall arrange a prorated tuition settlement that is reasonable and fair to both parties.
(12)A school shall be considered in default of the enrollment agreement when a course or
program is discontinued or canceled or the school closes prior to completion of contracted

services. When a school is in default, student tuition may be refunded by the school on a pro
rata basis. The pro rata refund shall be allowed only if the Superintendent determines that the
school has made provision for students enrolled at the time of default to complete a comparable
program at another institution. The provision for program completion shall be at no additional
cost to the student in excess of the original contract with the defaulting school. If the school does
not make such provision, a refund of all tuition and fees shall be made by the school to the
students.
School Closure Policy
If a school closes permanently and ceases to offer instruction after students have enrolled, or if a
program is cancelled after students have enrolled and instruction has begun, students shall
receive a pro-rata refund of tuition. The pro rata refund shall be allowed only if the Director of
Higher Education Coordinating Commission determines that the school has made provisions for
students enrolled at the time of default to complete a comparable program at another institution.
The provision for program completion shall be at no additional cost to the student in excess of the
original contract with the defaulting school. If the school does not make such a provision, a refund
of all tuition and fees shall be made by the school to the students. If the school fails to make
refund the student has the option of contacting the Higher Education Coordinating Commission,
and accessing the Tuition Protection Fund.
The arrangements or teach-out is to be performed, by an institution in the same geographic area
as the original school which provided the program or course.
The Tuition Protection Fund
Students attending schools licensed by the State of Oregon, other than students covered by
another state's tuition protection, may apply to the Director of Higher Education Coordinating
Commission, when a school ceases to provide educational services, for a refund of tuition from
the fund established pursuant to this rule to the extent that such fund exists or has reached the
level necessary to pay outstanding approved claims.
The Director of Higher Education Coordinating Commission shall direct the State Treasurer to
pay the refund on behalf of the student to the student and/or the student's financial sponsor(s). If
the student is a minor, payment shall be made to the student's financial sponsor. Any inquiries a
student may have regarding this contract, may be made in writing to Portland Beauty School at

Transferring Students
Any clock hours or competencies achieved from another school will be transferred and credited
directly towards the students graduation requirements, once Portland Beauty School receives
official transcripts from the students previously attended school; transcript of attendance must be
provided within 30 days of beginning classes. Upon initial enrollment, placement level into
Portland Beauty School’s program will be determined after an evaluation of their skill level. Once
the evaluation has been completed, tuition will be based on the skill level of the student. Transfer
students may provide their own books and kits or may choose to purchase from our school. Any
and all credit hour or competencies will be credited according to validation of credit received as
well as State standards and requirements of transferring credit or competencies.
Evaluation Fee: $ 500
Tuition (depends on placement) - Not to exceed the maximum amount charged as a beginning
student.
Evaluation fee will be applied towards the cost of training.

Grading
All students attending Portland Beauty School must maintain a minimum of 75% on all Academic
and Practical scores. The assignments are divided into 3 levels, Freshman, Junior, and Senior.
The school will not promote students until their grades are up to percentage standards (minimum
75%). If a student misses any assignments, they must schedule time with their instructor to makeup any assignments. It is the responsibility of that instructor to grade any missing assignments. If
a student takes a leave of absence, they will return at the same level as before. They will not be
required to make-up any instruction that occurred during their leave of absence.

This grade scale will apply to all written (tests) and grading of practical
(performance) skills. A student may retake a failed examination.
Graduation Requirements
Competency-Based Programs
In order for a student to graduate, a student must be considered ‘competent’ in every
course of instruction they are enrolled in. The determination of competency is based on
the minimum requirements outlined in each program of study, passing score of 75% on
all written tests administered in each program, and a passing score of 75% in all practical
testing. Students enrolled in a competency-based program can remain enrolled after the
school determines they are competent. Students enrolled in competency-based programs
may remain enrolled in those programs without incurring any additional costs until they
have “been trained for an amount equal to the” minimum hourly training requirements
established in OAR 581-045-0200(1).
Clock-Hour Based Programs
In order for a student to graduate, a student must the minimum requirements outlined in
each program of study, passing score of 75% on all written tests administered in each
program, and a passing score of 75% in all practical testing. A student must also meet
minimum training hour requirements.
Full payment of all tuition, fees and costs must be met before any transcripts will be
released.

School Guidelines and Regulations
The following is a guideline of Disciplinary Action that will be followed. Depending
on the severity of the situation, warning or suspension may, but is not required to
precede involuntary termination. Any student receiving a third written warning may
be suspended for a maximum of 30 days.
Any student that is terminated from Portland Beauty School will have to re-apply
to continue enrollment. We reserve the right to refuse readmission based on
attendance, academic, and social conduct of the student in the previous enrollment
period.

probationary period.
3rd Offense: Suspension - Immediate suspension of the student from Portland Beauty School.
This suspension will be a maximum of 30 days.
4th Offense: Termination – Once a student returns from suspension they will be placed on a
probationary period of 30 days. If the student violates any school rules, attendance, or academic
policies they will be terminated. This is the immediate withdrawal of the student from Portland
Beauty School.
All disciplinary actions will be documented in the students file.
School Rules
1. Any misconduct directed towards guests, faculty, or students of the school is considered
grounds for disciplinary action.
2. Any and all thefts, vandalism, physical aggression, inappropriate touching or conversing, will
not be tolerated. Students should report any of the above listed infractions to their instructor
or school administration immediately. Any students found in violation of any of the school
rules are subject to disciplinary actions or expulsion from the school.
3. NO USE OF CELL PHONES OR TEXT MESSAGING is allowed on the clinic floor or
classroom. Cell phones need to be on vibrate during School hours. Any calls need to be made
during breaks, and students should step out of the classrooms and off the clinic floor to make
phone calls.
4. Students should not give out their cell phone numbers as well as personal information to
customers, as this creates a safety issues for students and a liability issue for the school.

5. Students should refrain from public displays of affection or PDAs. Students who engage in
PDAs will be asked to discontinue the actions immediately. If students continue PDAs, they
will be asked to clock out for the rest of the day and will receive a zero for the day.
6. Students must contact the school directly and speak with the instructor in the event of an
absence.
7. No student is to interrupt another student while they have a client or model, either to visit or
attempt to instruct them. However, students are allowed to observe other students.
8. Any student proven guilty of soliciting clients for services outside the school will be subject to
disciplinary review.
9. Students are expected to dress professionally at all times. Clothing items which are prohibited
include, short skirts and shorts more than three inches above the knee, t-shirts without sleeves
or mid-section showing, tank-top style T-Shirts, pants sagging below the rear-end, low rise
pants, shirts or dresses showing any cleavage, T-Shirts with obscene pictures or wording.
10. Lab Coats, smocks, apron, or official school attire, must be worn at all times when in the
school building. Students are not allowed to modify official school attire, lab coats, or any
other official school clothing. Clothing and shoes should be clean and in good repair. Students
are also expected to maintain good hygiene standards while in school, i.e. showering, oral
hygiene, and personal grooming.
11. In compliance with Oregon Law there will be No Smoking in the front of the school building.
The southeast side of our school building is reserved for as a designated smoking area.
12. No eating, drinking, or gum chewing on the clinic floor.
13. All students are required to assist in daily sanitation duties before the end of each day.
Students are also responsible for keeping their work station clean, sanitary, and in good order.
Students who do not perform daily sanitations and have them reviewed and approved by their
instructor, will be subject to disciplinary actions as well as result in their inability to graduate
their course of study.
14. All students must sign in and out at the front desk.
15. Students should not have personal visitors, including children, spouses, significant others,
and friends, unless it has been cleared with an instructor.
16. Family and friends getting services need to be approved through the front desk prior to
services.
17. Students cannot accept personal phone calls. Any “emergency” phone calls for the students
will be accepted through the receptionist.
18. Students must receive permission from an instructor prior to receiving personal services.
There is a reduced charge for student services which must be paid in advance of the service
commencing.
19. Students are not allowed to receive personal services on Saturdays.
20. All point sheets and practical operations must be reviewed and signed by the instructor on the
day that it was performed.

21. Any time off request or leave of absence must be previously arranged through the office.
22. All students are obligated to complete live models to meet graduation requirements.
23. Only supplies (i.e. hair products, chemicals, and other items used in the service of guest)
purchased by the school are to be used at the school. Students and clients may not bring
their own products.
24. No student is allowed to refuse a client for any reason unless an instructor has discussed the
particular situation with them. If a student refuses to do a client’s service they will be sent
home with a zero for the day and subject to disciplinary action.
25. There is NO PARKING in front of the school. This area is reserved for clients.
26. Students are responsible for their time cards. Do not clock in another student for any reason.
Clocking in another student is grounds for disciplinary action.
27. All services must be consulted and checked by an instructor in order to receive credit for the
service.
28. Please remove all items from the refrigerator or cupboards daily. Any items left may be subject
to disposal at the conclusion of the day.
29. Any amendment to the Rules or changes will be circulated to each classroom and posted in
student gathering areas.
30. Students who are not maintaining Satisfactory Progress will be placed on a 30 day probation.
During this probation, the student needs to make up failed or missing assignments/tests, bring
up grade averages, and maintain Satisfactory Progress.
31. Any student receiving three suspensions will be considered for termination. Application for
readmission may be done by contacting Portland Beauty School. We reserve the right to
refuse readmission based on attendance, academic, and social conduct of the student in the
previous enrollment period.
32. Any time a student is clocked in they must remain on school campus. If a student is clocked
in and are not on campus, they will be clocked out and not receive any hours during the time
they were off campus.
