OURVISION
VISION AND MISSION
Our vision is to provide patient-centered primary care
in a physician-guided, collaborative team model.
OUR MISSION IS TO:
 provide increased access to primary health care for patients so that residents in our City have the
opportunity to access a primary care physician.

attract additional primary care physicians to the City of Kawartha Lakes.

provide patient-centered care where the patient is a key member of the team, who uses information and
support to make informed decisions on how to manage his/her self-care needs.

provide comprehensive family health care services through an interdisciplinary team including: family
physicians, nurse practitioners, registered nurses, patient educators, mental health counselors, social
workers, and pharmacists, each working within their scopes of practice.

serve as a central catalyst for the development of new, comprehensive community-based chronic disease
management and self-care programs.

emphasize health promotion, illness prevention and early detection/diagnosis.

link patients to other parts of the health care system such as acute care, long-term care, public health,

A WORD FROM OUR PRESIDENT
Welcome everyone to our 4th annual General
Meeting of the City of Kawartha Lakes Family
Health Team. This is an opportunity not only to
bring the community and our partners up to date
on what we’ve accomplished so far, and where we
stand, but it’s also a chance to celebrate all the
milestones that we’ve had as a Family Health Team
so far.
Overall, not only have we increased access and
reduced the number of residents without a primary
care provider, but the quality of care has improved
immensely with the broad range of services we
now provide. This year, we had close to 34,000
rostered patients: more than ever before!
While the first few years of operation of our FHT
were full of setting up programs and procedures,
the last year or so has mostly been about maintaining the focus on providing excellent care and
programs and developing each to the fullest. We
also developed a few new programs this year.
Our FHT’s priorities – as approved by the Board of
Directors – were to: focus on current work and
results, including Advanced Access; enhance the
evaluation of our FHT programs now that they had
been up and running; and proceed with our new
FHT “health hub” in Lindsay.
Notably, our FHT also expanded to Bobcaygeon this
year, welcoming Dr. Steve Oldridge and Dr.
Michelle Snarr.
A large part of the success of our FHT has been the
strong community partnerships. We appreciate all
the cooperation and excellent relationships that
have been forged.

I’d like to recognize Mike Perry, our Executive
Director, for the endless hours that he has dedicated to the success of our team. He’s been a huge
part of our success.
I would also like to extend special gratitude to Dr.
Peter Petrosoniak and Dr. Eric Ready, two of the
founding members of the FHT, and, along with myself, the original executive of the team. Without
them we wouldn’t be here. They have demonstrated tireless dedication and a clear vision of the way
forward for health care in our community, and we
owe them immense thanks for all they’ve done.
I am extremely proud of our Family Health Team
and where we are to date and what we’ve
accomplished. We have a tremendous group of
inter-disciplinary health professionals, but also
management and administration, who work hard
every day with the overall goal of helping improve
the lives of the residents of the City of Kawartha
Lakes. I am constantly impressed at the quality of
people working on our team. I have been honoured
to be part of the FHT and look forward to seeing
what direction the FHT takes in the future.

Family Health Team
HIGHLIGHTS 2013-2014
Our FHT Lab Services
Having restored and expanded the lab services in
Bobcaygeon, Fenelon Falls and Omemee for our
patients last year, this year showed excellent
results for these communities. All combined,
5,922 patients visited the labs from 45 different
communities! The average age of patients having
blood work was 64, evenly split male and female.
These were patients that could not have their
blood work done locally before our FHT provided
the service. Thanks goes to our FHT phlebotomists
Rossi Turner and Lois Wright, and to Jean Walsh,
Operations Manager, who oversees the delivery of
our lab services. Access to such a vital service is
key to providing health care where we live.
Part of our Community
Working in partnership as part of the community
enhances patient care and makes our communities
healthier. This fiscal year, our FHT concluded a
number of Memoranda of Understanding with
partner organizations in our area including the local
Alzheimer Society; Boys & Girls Clubs of Kawartha
Lakes; Brock Community Health Centre; Canadian
Mental Health Association Haliburton, Kawartha
Lakes Pine Ridge; the Central East LHIN; CHIMO
Youth & Family Services; Community Living
Kawartha Lakes; Ontario Early Years (Haliburton
Victoria Brock); and the Ross Memorial Hospital.
The primary purposes of these MOUs was to set
out in writing our mutual commitments to working
together, sharing information and preventing
duplication of services in order to maximize
patient care.
Strengthening Our Quality
and Research
Cameron Berry joined our team
in 2013/14 as our Quality
Improvement Decision Support
Specialist. Cameron has a
professional background in IT
and past experience in quality support at the Peterborough Regional Health Centre. His priorities are
helping to standardize patient data for improved
measurement and health population research;
enhancing access to patient data between providers
(e.g. RHM); and helping implement our FHT's
quality improvement plan.
Building Our
Mental Health Team
Lisa-Ann Janveaux joined our
FHT last fiscal year offering increased mental health services
to our patients and helping link
our patients to social services
and community resources. A
registered Social Worker, Lisa
Ann come to us with over 25
years of experience working with children, youth
and adults in a variety of medical settings. She has
run a private practice and worked in psychiatric
hospitals and FHT settings. Lisa-Ann has extensive
experience working in rehabilitation, palliative
care, adult mental health and with the dually diagnosed. She is a willing advocate for her clients and
her past experiences are an asset in assisting them
to navigate the system. Lisa-Ann lives in Cameron.
Another Sterling Audit
Our FHT’s auditors, Collins Barrow LLP, audited our
FHT’s finances again to an excellent result.

More Happy Feet!
Increased funding from the Ministry
enabled our FHT to offer increased,
regular foot care to our FHT patients.
Foot care specialist, Wendy Lenters,
RN, official joined our FHT in a part-time capacity,
bringing her background in medical and geriatric
nursing. Wendy holds a certificate in advanced foot
care nursing and is a member of the Canadian Association of Foot Care Nurses. Wendy is passionate
about chronic disease management and advocates
that foot care is necessary to prevent illness and
maintain overall good health. Our FHT provides foot
care primarily for our most "at-risk" patients including seniors, diabetic patients and patients who may
not be able to afford private chiropody.
FHT Christmas Dinner 2013
Because nothing says "festive season" like a good
murder mystery, a bold group of 15 FHT team
members, doctors, and guests gathered at the
Lakeview Arts Barn late last November for dinner
together and a murder mystery party. A fun time was
had by all. The play was interactive and very well
done with a good, seasonal theme. Thankfully, none
of our team was accused of murder!
Your CPR-Trained FHT
A total of 18 FHT staff were certified/
re-certified in CPR this year inhouse. Special thanks to our FHT
Clinical Practice Coordinator, Linda
Ready, for delivering the training.
New FHT Physicians!
On April 1, 2013, our FHT welcomed
two new physicians to the team.
Dr. Steve Oldridge is a University
of Ottawa graduate and has been
practicing family medicine in Bobcaygeon since 1991.
His rural practice encompasses office-based, urgent
care, long term care and palliative home visits.

Dr. Michelle Snarr practiced comprehensive family practice in Northern
Ontario for 19 years before moving to
the area with her family in 2007. She
is a graduate of Queen's University
and completed her residency in Newfoundland. She has practiced and taught in a wide
range of clinical and administrative settings.
In joining our FHT, both Dr. Oldridge and Dr. Snarr
noted the opportunity for increased interdisciplinary
health services for their patients. The arrival of Dr.
Oldridge and Dr. Snarr established a presence for our
FHT in Bobcaygeon and enhanced access to care for
patients living in the Fenelon Falls-BobcaygeonOmemee corridor.
NP for Bobcaygeon Patients
With our new FHT physicians in Bobcaygeon, we were able to provide a
new nurse practitioner to enhance
patient care. Sarah Reynolds began
seeing patients – the majority of whom are seniors at our FHT location in Bobcaygeon three mornings a
week in the fall. Sarah has worked with Kawartha
North Family Health Team and is currently with the
Community Care Access Centre seeing senior
patients in their homes.
The FHT Main Office Location
The FHT main office moved to a new location at 50
Mary Street in order to provide more room for our
interdisciplinary health providers at our FHT's
location at 55 Angeline Street in Lindsay. The new
location also came with access to a large Board room
used for both FHT meetings and our family health
programs. The main officer is located just around the
corner from Dairy Queen and staff settled well into
the new location. Visitors are always welcome.

New FHT Physician for Fenelon Falls
This year our FHT was thrilled to welcome Dr.
Kesang Pema to Fenelon Falls. Dr. Pema received
her medical degree from Lady Hardinge Medical
College (India) and completed her family medicine
residency training at the University of Western Ontario. Dr. Pema, and her husband Chaun, first visited the City of Kawartha Lakes in August 2012 for
KLHCI’s Summer Family Medicine Residents’ Weekend and were very taken with the beauty of the
countryside and waterways, along with the congeniality of the medical community. Originally from
Bhutan, Dr. Pema and her husband Chaun now live
in Cameron. Dr. Pema is the first ever female doctor in Fenelon Falls and quickly becoming a community fixture.
Public Consultations—Health Accord
As the federal government renegotiates health
care funding with the provinces, our FHT held a
series of public consultations to ensure views of
local patients and health care providers were taken
into account. Consultations were held in Beaverton, Bobcaygeon, Fenelon Falls, Haliburton, Lindsay
and Omemee in partnership with Brock Community
Health Centre, Haliburton Highlands Family Health
Team, Kawartha North Family Health Team, and
the Omemee Medical Centre. The opinions of local
residents were compiled into a written report that
was presented in person to Barry Devolin, MP,
Laurie Scott, MPP and the Premier of Ontario
Kathleen Wynne.
Server at 55 Angeline
A new server at the 55 Angeline
location brought increased efficiency and data security to our
FHT. A partnered project with
the After Hours Clinic, the new server has brought
"paperless" referrals to our FHT and a way to track
and manage data more efficiently and with more
security. Two and a half years of patient paper records were scanned into the server and our FHT
IHPs have also designated space to save and share
resources electronically.
FHT Intranet
Our FHT Intranet was
launched this past fiscal year
and met with rave reviews. The Intranet is an
internal website accessible
only to FHT physicians and
team members and is a central source of workplace resources, FHT news and staff
updates. The Intranet has made it easier to find
FHT information – eliminating the need to comb
through emails – and helps us all feel a bit more
connected to a central FHT presence as a team instead of separate professionals working at ten (10)
sites across the City.
Workplace Health & Safety
The FHT's Joint Health and Safety Committee continued to meet this year, with the elected worker
representative Lesley Broadworth and our FHT
Executive Director Mike Perry both obtaining their
certification in workplace safety under the Ontario
Occupational Health and Safety Act. Members of
the JHSC also conducted onsite, workplace inspections at locations where FHT staff work to ensure
their safety on-the-job. Providing safe, healthy
workplaces is a top priority of the City of Kawartha
Lakes Family Health Team.

FHT's bylaws were also approved by the membership in FY2013/14, which affected the number of
Directors, Directors' standards of care in governance, and an enhanced roll for the FHT Finance
Committee, in preparation for the entry-into-force
of Bill 65. Copies of these policies and bylaw amendments are available on the Intranet and/or upon
request to the FHT Executive Director.
The Hottest Staff Meeting Ever!
Our FHT's semi-quarterly "all staff" meeting took on
a new dynamic this year. Once again hosted at the
Executive Director's house in mid-July, the temperature hit 32*C during the meeting. While the agenda
was completed without anyone fainting, our FHT
certainly now holds the record for literally the
hottest staff meeting ever held. Red faces did enjoy
a pot luck BBQ – and abundant dessert table –
following the meeting. Go Team!
FHT Staff Makes Mark at AFHTO 2014
This year, the entire FHT staff attended the annual
conference of the Association of Family Health
Teams of Ontario in Toronto. Our FHT was very
active at the conference, hosting panel discussions
on building health networks and increasing rural lab
services. FHT staff were also panelists in a smoking
cessation net-work presentation. Our FHT was also
nominated for two Bright Lights awards, including
for integrating health care beyond the FHT. While
not receiving an award this year, a fun time was had
by all at the gala awards banquet.
Enhanced FHT Governance
In the 2013/14, the FHT Board of Directors revised
and/or adopted new policies to make governing the
FHT more effective. These policies included a risk
management framework, performance indicators,
and a Board policy manual. Amendments to the
Rostering days!
After years of drought without access to a family
doctor, the City of Kawartha Lakes Family Health
Team launched a "Rostering Days" campaign to
spread the word that the rumour was true: new
patients were finally being accepted again in our
community! Working with some non-traditional
partners – including BOB FM, Bell World and local
grocery stores – the City of Kawartha Lakes Family
Health Team worked directly with Health Care
Connect, meeting people where they are with the
ability to link residents with a care provider onsite. During the week-long "Rostering Days", 546
residents engaged with the FHT and Health Care
Connect resulting in 139 new patients rostered
within the two months following the campaign.
Update—FHT “Health Hub” in Lindsay
Consistent with the FHT's original Business Plan,
efforts continued in 2013/14 to find a location to
build a medical facility where FHT physicians, interdisciplinary health providers and other professionals
and health service would be located to enhance collaboration and systems navigation for patients. The
Facilities Committee considered different locations
and development options and decided to investigate a possible partnership with the City of Kawartha
Lakes, the Ross Memorial Hospital, and Community
Care to buy and develop the existing OPP facility
located at 21 Angeline Street North in Lindsay.

FAMILY HEALTH PROGRAMS
Participation in most of our family health programs requires a referral from your family doctor or nurse
practitioner. Ask your doctor or nurse practitioner if our family health programs may be right for you.
Anticoagulant Clinic: The Anticoagulant Clinic
provides patient education and point of care INR
testing for patients taking warfarin. The clinic is
held two mornings each week. Eligible patients
include those newly initiated on warfarin, requiring
bridging, with labile INRs or difficult phlebotomy.
Patients have their INR tested and receive dose
adjustments during their appointment. Screening
for drug interactions and adverse effects of warfarin is performed at each visit.
Craving Change Workshop: Craving Change is a
group program that focuses on why we eat the way
we do. Patients learn self-awareness skills and
strategies that promote cognitive and behavioural
change with regard to problematic eating. It also
provides the opportunity for
interactive discussion and
sharing of experiences with
others in the group. This fourweek workshop is designed
for individuals who struggle to
maintain healthy eating habits,
suffer from emotional eating, are frustrated with
the usual weight loss/diet programs and/or want to
feel more in control of their eating.
Foot Care – Healthy Feet And You: For “most at
risk” patients (e.g. diabetes, severe vascular deficiency) and/or those who cannot care for their
feet. Circulation, skin and nail conditions will be
monitored. Patients will also be educated on selfcare, hygiene and appropriate footwear.
Healthy Lung–Asthma/COPD Education: Individual referral visits help patients
learn to manage their Asthma or
COPD. Patients are provided information, and education on their
condition, medication delivery
devices, symptoms and triggers.
Spirometry can also be conducted. Individual action plans are developed to encourage self-management and improve quality of
life. Group education sessions will also be offered.
Healthy you: The Healthy You program is a group
weight management approach focusing on lifestyle
changes to help patients reach and maintain a
healthier body weight.
Patients learn how nutrition, physical
activity, self-esteem, the environment
and behaviours affect their weight. The
program consists of one two-hour class
a week, for eight weeks. The first part
of each class focuses on activity and
exercise led by a registered nurse. The
second part, led by the dietitian, teaches an aspect
of nutrition. An optional 15-20 minute walk with
the group is offered each session. Pedometers are
used to track individual progress. Additional individual follow up sessions are offered with the
dietitian after the completion of the program.
Memory Clinic: Standardized tests, assessment
and treatment plans for patients experiencing
cognitive impairment, memory loss and/or early
onset dementia, as well as their care-givers. Treatments, medications and community resources may
be advised, in consultation with the primary care
provider, including a possible recommendation
that the patient retire from driving. Additional support is provided on-site by the Alzheimer Society.

FAMILY HEALTH PROGRAMS
Mindfulness Meditation Group: An
eight-week program to teach patients
to connect with their inner strength,
wisdom and healing nature, has been
proven effective in the treatment of
stress-related ailments, anxiety, hypertension and
chronic pain. Follow up sessions are held once a
month to allow patients to reconnect and practice
techniques learned in the program.
Pain Self-Management Group: five
two-hour education sessions address
common difficulties of chronic pain
and offer solutions for patients.
Sessions provide a venue for sharing
experiences, which enhances
patients’ capacity to manage their
health. Sessions employ only non-pharmaceutical,
self-management approaches to care using psychoeducation and occupational/lifestyle techniques to
complement existing treatment.
Smoking Cessation:
Implementing the Ministry of
Health’s STOP Program, this
program helps patients
understand the underlying reasons for the patient’s
smoking, develop a Quit Plan and select a ‘quit
date’. On-going clinical support and motivational
coaching are provided. Free nicotine replacement
products are available.
Stressbusters!
Anxiety Education
Group: Three two-hour,
discussion-based classes
teach patients about the
causes and effects of anxiety and offer practical
strategies for dealing with anxiety (not group therapy). Especially suited for patients experiencing unhealthy levels of stress in daily life or the workplace.

Get W.I.T.H It Walking Group: Offers patients of all
ages a chance to walk in a warm, safe environment
during winter months. It is run at L.C.V.I. one hour
each week from November to the end of April and it
is suitable for all ages and fitness levels. Pedometers
are used to track their progress and step logs are
used so patients can visualize their progress each
week. This program is free and offered in collaboration with CCCKL–CHC to all members of the
community.
Let's Get Walking! Offers
patients of all ages, and
different fitness levels,
medically-supervised, outdoor and mall walking in
the community. It is offered for one-hour sessions
twice a week in Lindsay during the spring, summer
and fall months. Pedometers are used to track individual progress and to motivate the participants to
increase their step count throughout the program.
Wound Care: Assessment and treatment of complex
wounds by our wound care specialist. Treatment
options are offered and education on nutrition, ambulation and self-care are given. Follow up appointments or referrals are arranged as necessary. Home
visits can also be arranged if they are needed.

The accompanying summary financial statements, which comprise the summary statement of
financial position as at March 31, 2014 and the summary statement of operations for the year ended March 31, 2014 are derived from the audited financial statements of City of Kawartha Lakes
Family Health Team for the same periods. We expressed an unmodified audit opinion on those
financial statements in our report dated June 24, 2014. The summary financial statements do not
contain all the disclosures required by Canadian generally accepted accounting standards for notfor profit organizations, applied in the preparation of the audited financial statements.
Management's Responsibility for the Summary Financial Statements
Management is responsible for the preparation of a summary of the audited financial statements in
accordance with Canadian accounting standards for not-for-profit organizations.
Auditor's Responsibility
Our responsibility is to express an opinion on the summary financial statements based on our
procedures, which were conducted in accordance with Canadian Auditing Standards.
Opinion
In our opinion, the summary financial statements derived from the audited financial statements of
City of Kawartha Lakes Family Health Team for the year ended March 31, 2014 are a fair summary
of those financial statements, based upon Canadian accounting standards for not-for-profit organizations. However, the summary financial statements are incomplete to the equivalent extent as the
audited financial statements of City of Kawartha Lakes Family Health Team for the year ended
March 31, 2014.
