The Fund for Public Health in New York, Inc. (FPHNY) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH),

FPHNY implements programs to address pressing public health needs, fosters private sector support to enhance health and health care, and helps educate the public regarding the protection of individual, family and community health.

PROGRAM OVERVIEW

New York City Teens Connection (NYCTC), a program of the DOHMH, will build on the successful efforts of previous DOHMH initiatives to implement evidence-based sexual health programs, and increase teens’ access to contraception and health care. NYCTC will work in three target communities that have persistently high birth rates (above the national average of 26.6 per

1000): the South Bronx (42.8 per 1000), Central Brooklyn (34.2 per 1000) and Port Richmond,

Staten Island (33.3 per 1000).

NYCTC will work through partnerships and existing systems to bring evidence-based programs and clinic linkages annually to 15,000 young people ages 14-19. High schools and foster-care agencies will also be linked to local “teen-friendly” clinics that provide quality sexual and reproductive health services to young people; instructors will incorporate a visit to the linked clinic within each evidence-based program. NYCTC will also establish local Community

Advisory Groups and Youth Leadership Councils in each of the target communities who will inform the program work, encourage systems-based changes, and help disseminate information throughout the target communities. An evaluation plan that includes regular monitoring of key process and outcome measures for each component will provide the basis for continual improvements.

This is a 5- year grant opportunity through June 30, 2020; the position is contingent upon the availability of federal funding, and successful annual reapplication for continued funding. There is strong potential for this position to transition to DOHMH in Year 2 (if renewal is approved) and therefore candidates must meet DOHMH eligibility requirement including NYC residency.

POSITION OVERVIEW

New York City Teens Connection (NYCTC) offers an exciting, unique opportunity for a public health professional to work on an innovative project to engage with teachers and other school leaders, teens, parents, community leaders, clinical services providers, and other stakeholders that work with teens. The overall project aim is to foster an environment where all teens develop the skills and have the tools needed to make informed choices about their sexual and reproductive health, and know where to go for health services.

NYCTC’s Evidence-Based Program Coordinators – 3 located in the Bronx, 2 in Brooklyn, and 1 in Staten Island – are responsible for working directly with program partners that implement evidence-based sexual health programs in high schools, colleges, and agencies that provide foster care services. Evidence-Based Program Coordinators will be responsible for recruiting and retaining program partners to implement one of three evidence-based sexual health programs –

Reducing the Risk and Teen Health Project in high schools, and Seventeen Days in college and foster care settings – among young people in the focus communities, and will provide training and technical assistance to ensure that partners implement the selected program with fidelity.

Evidence-Based Program Coordinators will be supervised by NYCTC’s Deputy Directors of

Evidence-Based Programs to enact the project plan for partner recruitment, training and technical assistance.

Evidence-Based Program Coordinators work within Borough Teams that also consists of a

Community Engagement Coordinator; in Bronx and Brooklyn, the team will also include a

Clinic Engagement Coordinator). Borough Teams work collaborate to ensure that all program activities in each program component are well-integrated on a local level, and are responsive to each community’s unique needs and available resources.

DUTIES

The New York City Teens Connection Bronx Evidence-Based Programs Coordinator will:

Work with both Deputy Directors of Evidence Based Programs to recruit and confirm implementation partners for Reducing The Risk (RTR) and Teen Health Project (THP) among school partners, and Seventeen Days among college, clinic and foster care partners;

Implement Reducing The Risk , Teen Health Project and Seventeen Days at partner sites throughout designated areas

Provide training, technical assistance and supplemental materials to partners;

Work with the Deputy Director of Clinic Linkages to facilitate the implementation of linkage relationships between partnering schools and local clinics, as needed;

Work closely with the implementation partners and NYCTC staff to collect data and track all students receiving the selected evidence-based programs in all settings

Will represent NYCTC to partnering organizations

Work with and collaborate with other Borough Teams

Other duties as prescribed by the Project Director, as needed

QUALIFICATIONS and EXPERIENCE

 Bachelor’s degree in Public Health, or relevant field of study; master’s degree preferred

2-5 years of satisfactory work experience in a health services setting, schools, colleges, foster care agencies, and/or other organizations that work with young people

Knowledge about adolescent sexual and reproductive health

Experience with providing training and technical assistance; experience with evidence-based sexual education programs preferred

A minimum of 1-3 years in recruiting and maintaining relationships with community-based organizations, including schools, colleges, and foster care agencies

Knowledge of program evaluation and data collection methods

Excellent verbal, written and presentation skills;

Strong organizational skills and ability to meet deadlines

Ability to work well collaboratively with a diverse group of individuals

A minimum of 12 months of managerial experience (independent decision-making concerning program management, planning, allocation of resources, and/or scheduling/assigning work to others) is preferred

NY State Drivers License preferred
