Purpose
People credited with this unit standard are able to describe the
impact of chronic illness on a client’s health and functional
status, and manage a client’s chronic illness as a health
assistant.

1
Legislation relevant to this unit standard includes:
Health and Disability Commissioner (Code of Health and Disability Services
Consumers’ Rights) Regulations 1996;
Health Practitioners Competence Assurance Act 2003.

In the context of this unit standard, support should aim to maintain, improve, or
restore a client’s independence and/or interdependence; utilise the client’s existing
strengths; and, where possible, utilise the resources of the local community.

This unit standard cannot be assessed against in a simulated environment. For
assessment, candidates must demonstrate competence in the workplace through
paid or unpaid employment, or in placements in a service provider workplace
negotiated by an education provider.

Candidates’ practice must reflect appropriate values, processes, and protocols in
relation to working with Māori and Pacific peoples and/or people from other cultures,
in a range of settings and environments.

Definitions
Candidate refers to the person seeking credit for this unit standard.
Chronic illness may include but is not limited to – asthma, depression, diabetes,
hypertension, congestive heart failure, respiratory disease, epilepsy, Parkinson’s
disease, multiple sclerosis.
Client in the context of this unit standard refers to a person accessing services in an
aged care, health, or disability setting. Clients may also be known as consumers,
patients, or tūroro in particular contexts and settings.
Health assistant refers to any member of the health and disability workforce who is
not a registered health professional, and who directly assists a health professional in
the provision of health services to clients.

Health professional refers to a person who is registered with an authority (which is
appointed by or under the Health Practitioners Competence Assurance Act 2003) as
a practitioner of a particular health profession to deliver health services in
accordance with a defined scope of practice.
Organisation’s policies and procedures are the policies and procedures of the
employing organisation of the candidate and include ethical codes, standards, and
other organisational requirements.
Managing chronic illness refers to carrying out activities within the health assistant’s
role and responsibilities under the directives of delegating health professional(s) that
will alleviate the effects and impact of chronic illness, and promote a client’s physical
and emotional health and wellbeing.
Service plan is a generic term that covers the individual or group plans (which may
also be referred to by other names) that are developed by service providers for
people receiving support (and may include their family/whānau as appropriate).

Outcome 1
Describe the impact of chronic illness on a client’s health and functional status.
Evidence requirements
1.1
Chronic illness is described in terms of the features that distinguish it from acute
illness and recurrent illness.
1.2
Chronic illness is described in terms of its impact on a client’s health and
functional status.
Range
1.3
evidence is required for three clients with different types of chronic
illness;
for each chronic illness, impact includes a minimum of one of –
physical, emotional, psychosocial, cognitive, environmental,
psychological.
Service plans for clients with chronic illnesses are described in terms of own
role as a health assistant in managing the effects and impact of chronic illness.
Range
evidence is required for service plans for three clients with
different types of chronic illness.
Outcome 2
Manage a client’s chronic illness as a health assistant.
Range
evidence is required for one client.
Evidence requirements
2.1
Management of the chronic illness by the candidate is in accordance with the
instructions in the client’s service plan and/or the directives of a delegating
health professional(s), and the organisation’s policies and procedures.

Feedback on the health and functional status of the client with chronic illness is
documented and reported to the delegating health professional(s) by the
candidate in accordance with any requirements of the service plan and the
organisation’s policies and procedures.
This unit standard is expiring. Assessment against the standard must take place by
the last date for assessment set out below.
