Vineland-II Parent/Caregiver Rating Form
Information obtained from Vineland-II Survey Forms Manual p. 43-44
I. Preparing for Administration
A. Complete front cover of record booklet
B. Determine a starting point
1. Determine chronological age
2. Mark starting point for each subdomain
3. Cross out subdomains, the motor domain, or maladaptive behavior
domain if you are not administering it.

Administrating the Parent/Caregiver Rating Form
A. Establish Rapport with Parents
B. Review the form with parent so he/she understands how to complete it
Explain Instructions
1. “Please read the directions and mark a rating for every item in
each section after the start point circled or highlighted.”
2. “The items on this form cover a wide range. (Child’s name) is not
expected to have all the skills described by all the items”
3. “Mark the rating that best describes how often the individual
performs the behavior when it is needed, NOT whether the
individual can perform the behavior.”
4. Mark 2 if ind. usually performs the behavior w/out help or
reminders; mark 1 if ind. sometimes performs the behavior
w/out help or reminders or performs part of the behavior w/out
help; mark 0 if the ind. never or rarely performs the behavior or
never performs it w/out help or reminders
5. Mark 2 if the ind. doesn’t perform a behavior b/c he/she has
outgrown it.
6. “If you have a questions about an item, mark a 2, 1, or 0, but then
circle the question mark to right of the item scores. Afterwards, we
can talk about the question”. They can also write comments at the
end of each section by referring back to the item number with the
question mark.
7. Please let me know if you have any questions or need help while
you complete the form.
C. Ask parent to read directions on pgs. 3-4 in the record booklet.
D. Review the completed rating form
1. Correct starting points
2. Establish a basal (parent marks a score of 2 on four consecutive
items)
3. All items above basal completed
4. Establish a ceiling ( four consecutive items scored 0)
5. Follow up with parent on items marked with DON”T KNOW .
