Purpose: The Agreement Wind-Down Planning Checklist is intended to be a common source document used by both the Ministry and the service provider to document strategies developed by the service provider related to the wind-down of Targeted Initiative for Older Workers (TIOW) agreements. Application: Service providers must use this tool to aid in the development of comprehensive wind-down strategies that avoid or mitigate disruptions to client service. Ministry staff must use this tool to support service providers as they develop wind-down strategies; monitor the progress of developed wind-down strategies; and coordinate strategies and identify issues related to wind-down across an entire community. The checklist is a “living” document, in that initial wind-down strategies will likely change through implementation. It is expected that Ministry staff and the service provider will work out a schedule for the sharing of information related to this document and the timely updating of this document. Item Strategy/Additional Information/General Comments This section outlines the primary components of a winddown strategy for TIOW agreements. Ministry staff and service providers must use this section to provide information on the various components of the overall wind-down strategy for the TIOW agreement. Updates made to the initial Wind-Down Planning Checklist should be prefaced by the date and name of the individual adding the update Agreement Service Wind-Down 1. Determine cut-off dates for delivered TIOW services Consideration in determining the cut-off date for services: The date(s) should take into account the duration required for completion of the offered services The date(s) should (where possible) take into account any responsibilities the service provider has for client follow-up after completion of the services The date(s) should take into account the service needs of the community, and the availability of ES or other community services for client referral after the cut-off date The date(s) should allow the service provider a sufficient amount of time to give reasonable notice to -1- 2013-14 TIOW Agreement Wind-Down Planning Checklist Please return completed form to MTCU by September 30, 2013. community stakeholders (public, community organizations, other levels of government, relevant EO service providers) regarding the cessation of services The date(s) should be determined in consultation with MTCU, and community service providers that may receive client referrals from the exiting service provider The date(s) should allow enough time for ministry staff to give sufficient notice to internal stakeholders (for example: EO Hotline) 2. Client referral strategy Considerations for developing a client referral strategy: Identify service providers and/or community organizations that deliver similar services Notify and consult with EO providers and/or other community organizations regarding service closure and potential client referral (where appropriate) Confirm that service providers and/or community organizations identified for client referral have sufficient capacity to meet anticipated needs Ensure that your organization continues to have sufficient staff and resources to implement the strategy 3. TCU monitoring during the wind-down period Ministry staff will provide details regarding any monitoring activities that will be conducted during the wind-down period 5. Reporting strategy during the wind-down period Service providers continue to have a responsibility to follow the reporting requirements outlined in Schedule D of their TIOW 2013-14 Agreement. 6. Client follow-up strategy during the wind-down period Service providers continue to have a responsibility to conduct client follow-up during the wind-down period At the conclusion of the TIOW agreement: For clients that have exited TIOW services no further client follow-up will be required by service providers beyond the end date of the legal agreement.
