REQUEST FOR PROPOSAL - GENERAL DESCRIPTION
UTMB is requesting proposals for TDCJ IMAGING EQUIPMENT RELOCATION AND RENOVATION
to include work in the following trades: mechanical, electrical, plumbing, fire protection, fire alarm,
radiation protection and R/F shielding, cutting, patching, painting and incidental general construction
necessary to complete the work in accordance with the Construction RFP Documents. The project is
located at the Texas Department of Criminal Justice (TDCJ) Building (M016), located on the main
campus of UTMB, 809 Harborside Drive Galveston, Texas 77555.
In general, the work includes 4,394 square feet of renovation to an existing Imaging Department and
Pharmacy located on the third floor of the TDCJ building. Imaging modalities are existing and will be
installed by a third party vendor under separate contract. The scope is divided into three phases.
Phase I includes the renovation of the existing Pharmacy area to include an Audiology / Vestibular
Exam room, two telemedicine conference rooms and relocating an existing office to create a hallway for
access to the Phase I area through PT/OT. As an Add Alternate to Phase I, the balance of the existing
Pharmacy will be renovated to include five (5) exam stations and one (1) holding room. Phase II
includes renovation and relocation of existing R/F Radiology, Mammography and Ultrasound along with
staff support space. Phase III includes relocation of an existing MRI from Rebecca Sealy Hospital to
TDCJ, renovation of a portion of the imaging center for the relocated MRI and associated equipment.
Estimated cost of project is $805,000.00 (Eight Hundred and Five Thousand Dollars).
The work will be awarded under a one lump sum contract to the respondent offering the best value to
the Owner, according to the process and selection criteria indicated in Exhibit G, Appendix A,
Information to Respondents, Section II. The successful respondent will be required to enter into a
contract with the University of Texas Medical Branch at Galveston. Following contract award, the
Contractor will attend a preconstruction conference.
The Owner reserves the right to reject any and all proposals.

PRE-PROPOSAL CONFERENCE
A Mandatory Pre-Proposal Conference will be held on Monday, December 6, 2010 at 10:00 A.M. in
Conference Room # 1.864 located at the Primary Care Pavilion (PCP) Building, Bldg # M087, 400
Harborside Drive, Galveston, TX 77555. The project site is currently occupied, therefore a minimized
walk through of the project site will be held immediately following the conference. The walk through
shall be conducted in small groups so as to minimize disruption. Due to security requirements of the
project area, there will not be an additional walk through scheduled.
NOTE: All Contractor’s must submit, in advance, a list of their personnel that will be attending the walk
through to Tracy Jackson no later than 3:00 pm on Wednesday, December 1, 2010 for background
checks. List must contain the individuals Name, Date of Birth and Texas Drivers License Number. A
copy of the attendees valid TDL may be submitted in lieu of a list. Attendees without prior approval will
not be allowed on the walk through.
Respondents may park in the PCP Parking Lot, however they must display in their windshield that they
are attending a pre-submittal meeting and walkthrough for the above specified project. Respondents
are advised that all other parking surrounding the UTMB campus buildings are paid parking.
Respondent must allow time to obtain parking and arrive at the conference on time. The meeting shall
begin promptly at 10:00 a.m. Respondents may view the UTMB Campus Map by visiting the following

SUBMITTAL INSTRUCTIONS
Proposals will be received at The University of Texas Medical Branch, Galveston, Texas, at the Office
of Facilities Operations and Management until: Wednesday, January 5, 2011 at 3:00 P.M.


HISTORICALLY UNDERUTILIZED BUSINESSES (HUB)
UTMB encourages and supports the participation of Historically Underutilized Businesses (HUBs) in
responding to this RFP.
In accordance with Texas Government Code 2161, each state agency, including UTMB, is required to
make a good faith effort to assist HUBs in competing for, and receiving, contracts awarded by the
agency.
UTMB has reviewed this RFP in accordance with the Texas Administrative Code (Title 34, Part 1,
Chapter 20, Subchapter B, Rule 20.14) and has determined that subcontracting opportunities are
probable under this solicitation. Accordingly, a HUB Subcontracting Plan (HSP) is required as a part of
your proposal. The HSP shall be developed and administered in accordance with the UTMB Policy on
Utilization of Historically Underutilized Businesses (HUBs). The HSP Package, including the required
forms and instructions, can be accessed via the UTMB’s HUB Program website at: UTMB web site for

Programs” and in Exhibit I of the RFP documents.
Respondent must complete and submit the HUB Subcontracting Plan, as a part of your
Proposal. Failure to do so will result in your Proposal being disqualified. The HSP shall be
submitted within twenty-four (24) hours (one business day) of the due date of the proposals as
specified above and shall be submitted in accordance with the submittal instructions specified
in Exhibit G, Appendix A, Information to Respondents, Section XVII.

PROPOSAL SECURITY
A Cashier's Check or Certified Check payable without recourse to The University of Texas Medical
Branch at Galveston or an acceptable Surety Proposal Bond, in an amount not less than five percent
(5%) of the largest possible total proposal, including consideration of alternates, must accompany each
proposal as a guarantee that, if awarded the contract, the Respondent will promptly enter into a
contract and execute bonds in forms provided as outlined in the RFP Documents.
PERFORMANCE AND PAYMENT BONDS
A Performance Bond and Payment Bond in an amount of one hundred percent (100%) of the contract
price will be required upon award of Contract.
WAGE RATES
Attention is called to the fact that not less than the minimum wage rates prescribed in Exhibit D of the
RFP documents must be paid on this project.
QUESTIONS
The last day to submit questions shall be Monday, December 20, 2010 3:00 p.m. Please note that
UTMB shall be observing a Holiday on December 24, 2010 and December 27–31, 2010.
