SUMMARY: Provides case management and therapeutic services for older adults in nursing facilities, home and community based environments. Provides clinical assessments to meet OBRA regulations of referred clients from nursing facilities and hospitals. Responds to requests for such assessments and provides a written clinical report to the State specifying diagnosis and suggested treatments. Works as part of a multidisciplinary team to effectively meet the clinical needs of the clients. Demonstrates the ability to perform and document mental health assessments, interventions, and services. Maintains working knowledge of community resources and effectively provides that information to clients and/or their caregivers.

MINIMUM REQUIREMENTS:

Licensed clinician (LCSW, Ph.D. or Psy.D.) in good standing in the state of Colorado able to bill Medicare

Experience working therapeutically with specified population

Excellent verbal and written communication skills

Ability to be computer literate and type 65+ w.p.m.

Expertise working as a liaison to community agencies

Prior experience in a supervisory role, or ability has been demonstrated at the level of supervisor.

Experience working in the mental health field

Experience working with geriatric clients in a mental health setting

Experience providing services to the severely and persistently mentally ill clients

Experience conducting mental health evaluations

HOURS: Monday – Friday 8 am – 5 pm

MINIMUM SALARY: 38,800

Works as part of a multidisciplinary team to effectively meet the clinical needs of the clients. Responsible for accurate clinical assessment and diagnosis of clients. Responsible for effective treatment planning for all services provided by the team. Demonstrates the ability to perform, analyze and document mental health and substance abuse assessments, interventions and services. Maintains working knowledge of community resources and effectively provides that information to clients and/or their caregivers. Effectively works with referral sources and other community providers to cooperatively meet the needs of clients.

Specify particular job assignments and duties, if necessary. Will provide services to both committed and parolees, including multi-family, individual and family for Spring Creek, Zeb Pike and the community. Will also provide parenting groups in our school-based programs, utilizing the Incredible

Years Curriculum.

MINIMUM REQUIREMENTS:

 Master’s Degree in a Behavioral Science field that is licensable in the state of Colorado

 Or Bachelor’s Degree in Behavioral Science field PLUS 5 years of clinical/case management experience at PPMHC

Excellent verbal and written communication skills

Experience working therapeutically with specified population

Experience in various treatment modalities appropriate for specified population utilizing strength-based therapeutic approaches

Experience working with juvenile offenders

Care Coordination Team. The Care Coordinator III coordinates the flow of referrals submitted by DHS and CPA Caseworkers to the appropriate treatment providers under the clinical supervision of the

Program Manager. Responsible for complete, thorough and timely computer entry and distribution of referrals. Strives to streamline service delivery to customers; researches and resolve questions, concerns and complaints both internally and externally. Assist with coordinating services over multiple funding streams across multiple community agencies. Other responsibilities include assisting with keeping track of Substance Abuse expedited appointments and answering questions from providers and caseworkers.

MINIMUM REQUIREMENTS:

Masters in behavioral science field.

3 years clinical/case management experience.

Working knowledge of DSM IV.

Working knowledge of family systems.

Excellent verbal and written skills.

Ability to be computer literate and type 25 w.p.m.

Excellent Customer Service SkillsKnowledge of DHS system.

Knowledge of managed care principles.
