I,
, hereafter referred to as the Adopter/Buyer, hereby agree that the above described animal/K-9 is
being adopted by me as a pet for myself and/or my immediate family, a commitment for the life of the dog. I agree that I will not sell, give
away, or otherwise dispose of said animal/K-9 to any person(s), dealer, retailer, auction, institute, or any other entity for any reason. If at any
time in the animal/K-9’s lifetime the Adopter/Buyer is unable or unwilling to keep said animal/K-9, I the Adopter/Buyer agree ownership of
said animal/K-9 will automatically REVERT back to the Current Owner/Breeder and I agree to give back/release the said animal/K-9 to the
Current Owner/Breeder, at no charge/refund and as per the agreed upon, signed contracts pertaining to this adoption. I understand that I MUST
bring back the animal/K-9 to the address above at no cost to them. This animal/K-9 can never be taken to an animal shelter of any kind or be
resold/given away. This is in the best interest of the animal/K-9 and assures that the Breeder can be sure the animal/K-9 does not end up with
an irresponsible or abusive owner. All vet records/AKC paperwork must be signed over to the Breeder when returned. This paragraph does not
pertain to genetic or hereditary defects diagnosed by a licensed veterinarian or bonded specialist. No refunds will be given. If failure to return
the animal/K-9 and Breeder sees advertisements or is informed of transfer anyway or is blocked at any time from making contact in any way,
such as text, email, phone, postal mail, or social media such as Facebook or Messenger, a suit will be filed for breach of contract in the amount
of $3000.
I hereby agree…that this pet shall wear a collar or harness with identification tags at all times; to care for the above-described pet in a
humane/responsible manner; to provide it with clean/adequate shelter, food, water, veterinary care; that said pet shall reside inside my home
and shall not be allowed to roam freely. I understand that if found in neglect or breach of contract, the pet/K-9 WILL BE repossessed by the
Current Owner/Breeder and/or Animal Control. I acknowledge I am still liable for payment as per the agreed upon, signed contracts pertaining
to this adoption and a suit will be filed for breach of contract in any manner in the amount of $3000.

I hereby understand and agree that the Current Owner/Breeder makes no representations or warranties, expressed or implied, about the abovedescribed animal/K-9’s temperament/disposition and is hereby absolved from any liability for future damages or injuries caused by the abovedescribed animal/K-9. I also understand and agree that the Current Owner/Breeder further gives no guarantees, expressed or implied, of the
suitability of the animal to the Adopter/Buyer and/or family.

I certify that all statements made by me on this adoption agreement are true and correct. I agree that the Current Owner/breeder has the right to
confiscate/repossess the above-described animal/K-9 in the event that any statements made by me are found to be false and/or my check for the
payment of the animal/K-9 at any time is returned for insufficient funds. By signing this agreement, I understand and accept all aspects

HEALTH GUARANTEE/VACCINATION/WORMING RECORD

1) Your new puppy will be vaccinated by myself with all age appropriate vaccinations starting at 6 weeks of age with Canine
Spectra 5-Way Vaccine {Recommended for vaccination against canine distemper, hepatitis, adenovirus cough, parainfluenza, and parvovirus}
and Canine Spectra 7-Way Vaccine {Recommended for vaccination against canine distemper, canine adenovirus type 1 and type 2,
parainfluenza, parvovirus, Leptospira canicola, and Leptospira icterohaemorrhagiae.} It is necessary that you follow up with your local
veterinarian to receive more vaccinations, age/breed appropriate.

2) Your new puppy will be dewormed with Pyrantel Pamoate, a prescription given by our veterinarian, starting at two weeks of age every two
weeks or age appropriate, or as advised by our veterinarian.

____ LIMITED AKC REGISTRATION: Any puppy(s) purchased on limited registration are eligible for American Kennel Club (AKC)
registration; I will provide you with the AKC registration forms needed to register your pup. What this means is that s/he will be registered as
purebred and be eligible to compete in AKC events (with exception of conformation), including but not limited to obedience trials, hunt tests,
agility, etc. S/he WILL NOT be eligible for breeding with limited registration. Autumn Woods reserves the right to repossess said dog and
litter(s) if breeding(s) occur and a suit will be filed for breach of contract in the amount of $2500 for said dog as well as for each puppy sold.
____ FULL AKC REGISTRATION (Show/Breeding): Any puppy(s) purchased with full registration, must not be bred until 2 years of age.
Vaccinations and deworming must be kept current. Autumn Woods reserves the right to repossess said dog if breeding is done prior to two
years of age along with the entire litter(s) at adopter’s expense. See separate agreement for any puppy(s) sold on co-own agreement.
____ Our animals are priced according to pedigree, quality, breeding history, and/or possible future potential. We make no guarantees implied
or otherwise that any animal sold will gain any type of title.
____ SHIPPING/TRANSPORTATION CHARGES: We ship nationwide via ground transportation or via air out of Bangor International
Airport in Bangor, Maine. All shipping costs are the responsibility of the buyer along with costs for special health requirements and supplies
needed before/for travel and due one (1) week prior to travel. We ship as a courtesy. Shipping charges vary based on the weight of the animal.
We take every precaution to insure the safety of all of our animals, but because we have no control over the animal once it leaves our home,
Autumn Woods accepts no responsibility for death or illness of animals which may be brought on by/or as a direct result of shipping or
exposure to other animals and environments.
HEALTH GUARANTEE:
____ Failure to sign or return a signed copy of the contract will make the health guarantee VOID.
____ The seller agrees that the dog is in good health at time of purchase. The dog’s health is guaranteed by Autumn Woods Kennel for 72
hours in case of any communicable disease. Buyer agrees to have the puppy examined by a licensed veterinarian within 10 days of
purchase/delivery or the health guarantee is forfeited. Seller further guarantees this puppy to be free from any inherited/genetic defects/diseases
directly related to the breeding of the dam and sire which impairs good health or that will shorten the life of the dog or cause it to die for 1 year.
If such a hereditary/genetic disease/defect directly related to the breeding of the dam and sire should occur, it must be verified in writing
{Cause must be proven as genetic or hereditary 100%} by a qualified veterinarian and bonded veterinary specialist when appropriate within 2
business days of the diagnosis, at the buyer’s expense and veterinary/seller’s discretion, you will have the following options: The buyer may
bring the puppy back for another one of equal value, full refund, or ½ the vet bills, (not to exceed ½ the purchase price of the animal), not
including shipping expenses, within one year of the birth of the puppy or as dictated by the State of Maine. Choice is to be at the discretion of
the seller. This does not include minor illnesses or diseases that do not affect long life of the puppy. Vet must state in writing that the problem
existed in the animal at the time of delivery and sent to the breeder within 48 hours.
____ The seller has the right to get a second opinion from a veterinarian of seller’s choice at seller’s expense. “Health problem” means any
disease, illness, or any genetic/hereditary condition that would impair the health or cause death of the animal. ***Breed specific health
problems and those that we do not cover are excluded.*** We do not guarantee size, temperament, or breeding/show ability. Our puppies are
sold as pet quality/limited AKC registration only unless other arrangements have been agreed upon.
____ If the dog dies from a hereditary/genetic illness within one year of its whelp date, an autopsy/necropsy must be performed by a licensed
veterinarian at the buyer’s expense and the veterinarian’s written report given to seller. This may be examined by a veterinarian of seller’s
choice at seller’s expense. If it is determined that the dog died due to a hereditary illness by both veterinarians, the buyer may choose to be
reimbursed the original purchase price of the dog, not including shipping expenses. If buyer chooses not to have a autopsy/necropsy done and
the cause is never found, then breeder is not responsible in any way.
____ If the dog is euthanized, disposed of, or returned before a definitive diagnosis is made or the option given to obtain a second opinion from
seller’s veterinarian, all rights to this guarantee are null and void.
____ If your puppy ends up with a health problem and you choose to keep him/her you will be required to spay/neuter your animal because no
animal with a known health problem should be reproducing and you will be required to send us proof of spay/neutering.
____ Problems that are not covered or have limited coverage under the health guarantee include but are not limited to: We do not guarantee for
Wobblers Syndrome, hip/elbow dysplasia, pano, OCD, luxating patella, or HOD - these have many variables that can be the cause and is not
solely a genetic/hereditary issue; do not try to speed up the growth of your puppy or over-exercise your puppy. We do not guarantee for
entropion/ectropion which is the rolling in or out of the eye lids or ocular conditions leading to it, PRA, eversion of the cartilage of the third
eyelid, or congenital glaucoma or cataracts. We do not guarantee for diabetes, hypothyroidism, or Addison’s disease/Cushing’s syndrome
because the cause is not solely a genetic/hereditary issue. We do not guarantee against cancer, Von Willebrand’s, megaesophagus, uneven
bites, undescended testicles, demodex mange, inguinal or umbilical hernias, skin diseases, worms, fleas, ticks, or ear mites, or Coccidian and
Kennel Cough (common in puppies that are shipped; often caused by bacteria). Breeder will not be responsible for any conditions related to
Hypertrophic Osteodystrophy or improper feeding of your dog. Dogs must be fed a good quality adult dog food and must be breed-specific,
preferably the food recommended by the seller or by the BARF diet. Never feed your puppy, Puppy Food, of any kind, always switching
foods very slowly or this will cause growth defects or medical conditions to occur for which Autumn Woods holds no responsibility nor offers
guarantees for and are not covered under this health guarantee. We have a limited hip and heart guarantee. If your puppy is diagnosed with a
hereditary/genetic degree of hip dysplasia that is so severe he/she needs surgery to correct it or needs to be put down because of it, we will
cover it with one of the remedies under the health guarantee, vet fees are not refundable. If you choose to operate for dysplasia it must be done
before one year of age. If the disorder is hip and/or elbow dysplasia, it must be certified by Orthopedic Foundation for Animals {OFA}. Only
life threatening, inheritable/genetic heart conditions will be covered under the health guarantee, if a vet finds a genetic/inheritable heart
condition, it must state in writing that the condition either caused death, will shorten its life, or require constant treatment. We do not guarantee
for bloat, this is not a 100% inherited condition and there is no history of this in your dog’s lines. This health guarantee does not include any
illness due to the ingestion of foreign objects, chemicals, or physical injury; accident, abuse, or unintentional, and does not cover well puppy
visits, regular vaccines, deworming, or any other veterinary care or medication. Please understand your refund will be in a timely manner,
but payments may be necessary due to our own animals’ care and medical bills.
BUYER AGREES TO:
____ Abide by and adhere to all contracts pertaining to this adoption. All risk of loss or injury is the responsibility of the buyer after it leaves
seller’s kennel.***Not to abuse or neglect the pup, if so breeder has the right to repossess the dog at anytime.*** In the event that a puppy is
being replaced, the buyer is responsible for shipping of both the original dog and the replacement. If for any reason this contract needs to be
settled in a court of law, buyer recognizes Maine and Penobscot County as the court district, and the buyer will be responsible for all legal
expenses incurred by Autumn Woods in any case filed by the buyer or seller that is ruled in the favor of Autumn Woods Kennel. Buyer is
aware that Maine State Law recognizes verbal agreements and the contents of this written guarantee are recognized as a verbal agreement as
well as /written contract between the seller and the buyer. At such time the terms and conditions of this/these contract(s) are broken, buyer is
found in breach of contract(s), breeder has the right to repossess the pup/dog from buyer, all veterinarian/AKC papers are to be turned over to
the breeder, and a suit will be filed for breach of contract in the amount of $3000. I agree, if said animal/K-9 is to be repossessed by Current
Owner/Breeder, willingly returned/surrendered, that said animal shall continue to reside with me until arrangements are made and/or readoption occurs, per Current Owner/Breeder’s choice/discretion. This agreement/health guarantee is not transferable without written consent
by Autumn Woods; it is only for the buyer listed on the agreement. This is a binding contract enforceable in the court of law with
the State of Maine. I, Nadine Maxham, the Breeder D.B.A. Autumn Woods and buyer(s) certify that we have read this
contract, understand the contents, and agree to be bound by the terms of this agreement and buyer acknowledges they
received a copy of the State of Maine Title 7, Chapter 745: Sale of Dog and Cats Document (Attached to Contract)

The sale of dogs and cats is subject to consumer protection regulations. Maine law also provides
safeguards to protect pet dealers and animal purchasers. Attached is a copy of the Maine Revised
Statutes, Title 7, chapter 747. Continued in this law is a statement of your consumer rights and
remedies. Also, is your pet’s health history and specific warranty information.
