Make sure your Microsoft Word program is set to display “Hidden Text”. This document contains helpful
information, examples, and instructions that are only visible (and will never print) when the “Hidden
Text” feature is enabled. “Hidden Text” will be displayed highlighted yellow, italics, and underlined. Go
to the “Tools” menu, “Options”, on the “View” tab make sure “Hidden Text” has a check mark, and click

Federal regulations require IRBs to review survey measures to be used with subjects in human
research studies. Investigators are required to inform the IRB of exactly what measures will be used
and to provide the IRB a copy of these measures so that the IRB can evaluate the risk to subjects.
Investigators are required to provide the IRB with a copy of these measures even if the measure is
copyrighted and/or if the investigator has to purchase the measure(s).
