Per Diem Patient Care Associate – 20 hours per week

The Fund for Public Health in New York, Inc. (FPHNY) is a 501(c)3 non-profit
organization that is dedicated to the advancement of the health and well-being of all New
Yorkers. To this end, in partnership with the New York City Department of Health and
Mental Hygiene (DOHMH), FPHNY implements programs to address pressing public
health needs, fosters private sector support to enhance health and health care, and helps
educate the public regarding the protection of individual, family and community health.
Project Overview:
The New York City Charter requires the New York City Department of Health and
Mental Hygiene (DOHMH) to promote and provide health services to those in the care
and custody of the Department of Correction. DOHMH’s goal is to provide high quality
health care that will improve the health of incarcerated individuals and provide continuity
of care for persons reentering the community from City jails. This is direct service
work in jail-based housing areas and clinics on Rikers Island.
Under supervision of a licensed professional medical staff in a health care setting,
performs a variety of health-related and clerical duties associated with patient care

Checks vital signs, weight and measures patient, obtains specimens and records
findings on patient’s chart.
Instructs patients on how to obtain specimens for laboratory testing and performs
basic specimen screening testing through dipstick, fingerstick, and hemocult.
Prepared collected specimens for pickup by labeling and preparing required form(s)
and pickup up lab results
Conducts controls of HIV test kits
Informs patient on proper preparation for various medical testing.
Conducts interview of patient to obtain routine personal and medical information and
appropriate medical history and social supports
Maintains, completes, enters, processes and files patient information in electronic and
/ or paper charts as appropriate.
Reports observations of patient to medical staff caring for patient to ensure proper
professional assessment
Answers routine inquiries of patients in-person and makes appropriate referrals
Participates in informing patients and their families of health care services and
achieving health and wellness goals.

Maintains inventory, supply and chain of custody records of the unit’s equipment,
including HIV testing kits and lancets, stock and supplies
Works effectively as a team member in the delivery of care and assists in the
orientation of new employees.
Directs and accompanies patients to other facility areas as needed
Coordinates efforts of various clinic and facility departments in scheduling
appointments through manual / automated systems and enters pertinent patient
information into data systems
Obtains bloods specimens for laboratory testing by performing venipuncture /
fingerstick testing
Operates glucometer, sphygmomanometer, and other vital sign monitors and medical
equipment for the purposes of patient health assessments.
Qualification Requirements
1. HS Diploma or equivalent
2. NYS Dept of Health certification in phlebotomy
3. Demonstrated proficiency in HIV testing by means of fingerstick
4. Must be available 20 hours per week during the following times:
Tuesday – Friday 12:00pm – 8:00pm or 1:00pm – 9:00pm and/or Saturdays
7:00am – 3:00pm
Preferred Skills / Qualifications
1. Bilingual in English and Spanish
2. Knowledge of HIV/AIDS, STDs, diabetes, asthma, cardio-vascular disease,
hypertension, and chemical dependency issues
3. CASAC certification
4. Familiar with issues faced by those involved in the criminal justice system
Salary commensurate with experience ($15.00 - $20.00 per hour)
