Virginia, Inc., Attention: Scholarship Committee, P. O. Box 477, Holden, WV 25625.
In submitting this application, I certify that all information provided is complete and accurate to the best of my
knowledge. Falsification of information may result in termination of any scholarship granted.
I authorize Southern West Virginia Community and Technical College and the Southern West Virginia
Community College Foundation to verify all information contained in this application. Any institution, agency,
or individual may release this information to the College and Foundation for verification purposes. I
understand the College and Foundation may release this information for verification and/or publicity

It is the policy of Southern West Virginia Community and Technical College to provide equal opportunities to

The CEDAR of Southern West Virginia, Inc. 2014 Scholarship Program at Southern West Virginia
Community and Technical College will provide ten (10) $1,000 scholarships to students from the
CEDAR participating counties (Mingo, Logan, Boone, McDowell, Wyoming and Wayne).
High School Seniors Only. Any student in the graduating class of a high school or its equivalent, who is a
resident within Mingo, Logan, Boone, McDowell, Wyoming, or Wayne Counties, may file an application. All
applicants must be citizens of the United States of America on the date the application is filed. This
application must be filed with the CEDAR of Southern West Virginia, Inc., P. O Box 477, Holden, WV
25625. Only one application may be filed per student. The final date for filing completed applications is
April 18, 2014. All applicants must provide their Social Security Number.
The $1,000 Award will be used for an industry related degree at one of the Southern West Virginia
Community and Technical College campuses. (Industry related degree can range from administrative,
accounting, engineering, etc.)
GENERAL INSTRUCTIONS
1. Official high school transcript of student grades from the beginning of the 9 th grade to due date of
application may be photocopies that bear an original signature of the proper school authority.
2. SAT and ACT test scores must be included but may be photocopies.
3. Essay must be typed.
4. Applications must be properly bound on left side with hinges or staples in a standard binder or cover.
Elaborate bindings are discouraged. Clear plastic bulky covers are undesirable.
