STRUCTURE OF THE ASSESSEMENT REPORT
The annual assessment report is a summary of the findings of that year’s assessment of one or more of the
Program Learning Outcomes. It describes the process of the assessment; presents the findings, and discusses
the significance of these findings to the curriculum content, the faculty, the support staff, and the students in
the program; and outlines the changes perceived as necessary
1. BACKGROUND INFORMATION
1.1 Brief introduction and overview of the program
Some brief background information to the program mission learning outcomes etc.
1.2 Response to Recommendations of Earlier Program Assessment Activities
Briefly outline the findings of previous program assessments and the program improvements that
have resulted from them
2. CURRENT LEARNING OUTCOMES UNDER ASSESSMENT
2.1 Briefly describe the cognitive/knowledge, behavioral/skills and/or affective/values expressed the
learning outcomes you are assessing.
2.2 Learning Criteria: Explain the characteristics in students’ work that would show that the outcome
are being achieved.
2.3 Standards For Your Program’s Success: State what you consider satisfactory success in student
achievement.
3. THE PROCESS
a. Evidence: Describe summative evidence you analyzed: the source, the collection procedures, the
population & the size of the sample.
b. Assessment Tool/Method: Describe the evidence collection procedure, the sampling, the analysis
and the tools/methods used for this
c. Assessment Process: Describe assessment process: methods employed to ascertain validity etc.
d. Participants: List participants who assessed evidence.
4. FINDINGS
a) Results of Student Learning: Present the results of the analysis of the evidence of student teaching
(tables, graphs, charts.
b) Achieving Standards: Explain whether your program is achieving its standards for success in the
learning assessed.
c) Discussion of Results for Program Improvement: Provide a discussion of what you’ve discovered
about student learning and the significance of these findings for your program.
d) Participants in Discussing/Reviewing Results: List the members of the assessment team (indicate
full/adjunct faculty & attach sample minutes).
e) Communication of Results: Briefly summarise the discussion during the meeting to share the
findings with other stakeholders (attach minutes as evidence
5. PLAN TO IMPROVE YOUR PROGRAM
Briefly describe the improvements that you suggest for the curriculum as a result of this assessment and
provide a schedule of activities towards this improvement

6. REFLECTION ON ASSESSMENT PROCESS
Explain what you consider the strengths of this year’s assessment and what you would like to improve in
your existing assessment process as you plan next year’s assessment.
7. PLAN FOR NEXT YEAR’S ASSESSMENT
List the PLO(s) (the cognitive/knowledge, behavioral/skills and/or affective/values) you plan to assess in
the year academic year.
