The assessment host is the National Council for Performance Excellence on behalf of the {Program Name}.

with the URL, ID Name and Password to access the assessment. The "Subject" line will read: "{Org Name} - Organizational

Assessment Login Information". The target is for you to complete the assessment by ____. The purpose of this assessment is to find out what your organization is doing well, and how the processes can be improved. Please be completely candid when completing the assessment.

“Forgot Password” - an email will be sent to you with your login information.

The purpose of this assessment is to evaluate "how your organization conducts its business", using the

Baldrige Criteria as its business basis. The Baldrige Criteria examines how mature an organization is in:

 determining what customers need and organizing to meet these needs reliably

 developing and executing strategy

 using appropriate measurements and analysis to understand the performance of the business

 developing and harnessing the skills and knowledge of its people

 organizing the flow of work (especially across and between functions) to make it easier for everyone to meet customer requirements

The assessment has a User’s Guide that can be printed out to assist you in completing the assessment. It can be found at http://www.ncpesurvey.com/demo/guide.html

. Completion of the assessment takes approximately one hour, depending on the feedback you provide.

Your data will be saved every time you click “Save Your Work” or “Next” which will advance you to the next page. Please keep in mind that your data will not be submitted unless the demographics and all of the seven categories say ‘Completed’.

Once the ‘Congratulations’ page is displayed, you’re done. You may reopen your assessment and edit any of your responses. The assessment can be accessed any number of times throughout the established timeline.

You should not feel intimidated about sharing your comments openly, for two reasons:

1.

The purpose of this assessment is not to evaluate the performance of departments or individuals.

The focus is on whether systems and methods for doing business are effective, and this is what the assessment report will focus on.

2. The assessment process is anonymous. The feedback report will not reveal names of individuals who may have provided comment on any of the questions. Assessment data will be aggregated together with the other participants and sorted demographically.

Soon after completing the assessment, the key findings will be available for all staff.
