The doctors and staff at Pavilion Medical Centre in Lambeth, South West London are proud to offer the highest standard of patient-centred healthcare.

We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

Extended Hours

Our extended hours 6.30pm - 8pm on Wednesdays and Thursdays and on Saturdays are avaialble by appointment only. You may also drop in to collect prescriptions or to register.

When We Are Closed

There is always someone at the surgery from 8am to 6.30pm, Monday to Friday, to take your call. Outside these hours, an answer phone will have details of how to obtain advice. When the surgery is closed please call the usual practice telephone number (020 7274 9252) and you will be directed to the out of hours service.

We are members of SELDOC, a co-operative of local GPs providing out of hours cover, (020

8693 9066). A receptionist will take your details and a doctor will call you back.

You may be offered telephone advice, or asked to attend the SELDOC base at Dulwich Hospital,

East Dulwich Grove, SE22 8PT. If it is needed, a doctor may visit.

NHS Direct

In the case of urgent need when the practice is closed you can call NHS Direct

(www.nhsdirect.nhs.uk/) (0845 4647) to speak to a triage nurse. Your needs will be assessed and advice offered or arrangements made for you to see a doctor.

Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.

Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.

Appointments

We aim to see people by appointment, in order to cut down on time spent waiting in the surgery.

If you feel you need a longer appointment please ask the receptionist. Although we try to keep to time, there may be emergencies or other unexpected problems that cause delays, so please be patient.

You may have an appointment with any of the doctors, regardless of who you are registered with. If the doctor you want to see is not available at a particular time, you may see any of the others. We also offer bookable telephone consultations.

When booking, please let us know if you or a child have a rash with fever, as we will try to minimise contact with other patients.

It is always possible to book up to several weeks ahead. But if you need to be seen the same day we will endeavour to accommodate you either with an appointment or a telephone consultation.

We can book an interpreter or use telephone interpreting services. The following languages are spoken by members of the practice team: Spanish, Russian, Sylheti, French, Turkish, Punjabi,

Hindi, Urdu.

Telephoning The Surgery

If you wish to speak to a doctor, we offer booked telephone consultations with all doctors, mornings and evenings.

Disabled Access

We have full access to all consulting and treatment rooms and toilets. There is a portable induction loop available for people with hearing problems.

Chaperones

If you would like a chaperone for any examination, please inform reception.

Home Visits

If possible please try to telephone reception before 10:00 if you require a home visit.

A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

House visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the surgery.

Repeat Prescriptions

For safety reasons, we do not take telephone requests for prescriptions, except by prior arrangement from the housebound. A repeat request slip is attached to each prescription.

Please keep this for your next order. Prescriptions will be ready in 2 working days, or can posted back if you enclose a stamped addressed envelope.

We can also accept on-line requests for repeat prescriptions. Please do not let yourself run out of important regular medication.

At intervals, we will ask people on regular medication to make an appointment for a check-up.

Medication Reviews

Patients on repeat medication will be asked to see either a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications.

Please allow two full working days between requesting and collecting your medication.

Prescriptions Charges and Exemptions

Extensive exemption and remission arrangements protect those likely to have difficulty in paying charges (NHS prescription and dental charges, optical and hospital travel costs).

The NHS prescription charge is a flat-rate amount which successive Governments have thought it reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment certificates (PPCs) offer real savings for people who need extensive medication.

NHS charges

These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions are free of charge.

 Prescription (per item): £7.40

 12-month prepayment certificate (PPC): £104.00

 3-month PPC: £29.10

If you will have to pay for four or more prescription items in three months, or more than 14 items in 12 months, you may find it cheaper to buy a PPC.

Clinics & Services

Nursing Services

We provide full nursing services including cervical smears, travel vaccinations, childhood immunisations, flu jabs, diabetes and asthma/COPD checks and blood tests.

Smoking Cessation

We provide NHS stop smoking services at Pavilion Medical Centre.

If you decide to give up, you will be seen on a one-to-one basis and will be offered appropriate treatment including nicotine replacement therapy, such as patches, gum, lozenges, microtabs, inhalators and nasal sprays.

We can also prescribe bupropion (Zyban), a drug that reduces the craving for nicotine. This isn't for everyone as it can cause epileptic fits in people prone to them and can affect liver and blood pressure, so needs close monitoring. Some people do very well with the medication, so it's worth considering.

There is also a recently introduced drug called varenicline (Champix), which blocks the effect of nicotine in the brain.

Diabetes

The practice provides diabetes checkups for all those affected by, or have a family history of diabetes. We are fortunate to be supported by the Lambeth Diabetes Intermediate Care Team

If you are keen for a check up, then please book an appointment for blood tests and then a second appointment with the nurse or a GP to get the results.

Contraceptive Advice

Confidential contraceptive advice is available, regardless of age, and is given by the practice nurse or GP’s. A full choice of methods is offered, including contraceptive implants and IUCDs

(coils).

Emergency postcoital contraception is available, up to 120 hours after unprotected sex by hormonal methods, or 5 days by fitting IUCD (coil). Click here for more information

An appointment will always be made for you as necessary. We can refer for termination of pregnancy, where appropriate, regardless of age. We can also give details of specialist contraceptive services.

Chlamydia Screening

As part of a national programme, we offer screening for chlamydia and gonorrhoea, aimed at young men and women. This is a simple and painless urine test for people who do not have any symptoms. We also offer routine sexual health screening.

Mental Health

We have counselling and psychology services available at the practice. There are also strong links with our local community health team.

We work closely with a number of services offering support and treatment for people with drug or alcohol problems, and now have a weekly specialist clinic on Friday mornings with the

Primary Care Psychology Services

Lambeth IAPT (Improving Access to Psychological Therapy) is a team of 36 mental health workers, including clinical and counselling psychologists, CBT therapists and psychological wellbeing practitioners, providing CBT and guided self help for people with common mental illnesses (predominately anxiety disorders and depression) across the borough of Lambeth. We have three additional target groups- those suffering the effects of worklessness , BME communities, and offenders and ex offenders.

Non NHS services

The NHS may not cover certain things; for instance, visitors from abroad may not always be eligible for free treatment (although emergencies are always covered). We can also provide certain services outside the NHS, such as LGV or Taxi medicals, insurance examinations, for which a fee is payable. Our charges for these are listed in reception. We do not countersign passports.

Sickness Certificates

Sickness Certificates

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .

Evidence that you are sick

If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).

It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.

You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.

Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.

Statement of Fitness for Work - ’Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Registration

New Patient Registration

We offer permanent registration to anyone living within our practice area. If you expect to be here for only a short time, we can provide temporary registration.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor.

Medical treatment is available from the date of registration. Please contact reception for further information.

Registering Online

Online 'pre-registration' with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
