SCHOOL PURPOSE
Sly Park Fundamental Basketball School was formed to provide instruction so that 4th through 12th grade girls and boys can learn
the fundamentals of basketball and develop skills necessary to be successful varsity players. The school is designed to first evaluate
each player and then provide instruction based on ability and current skill level. A player evaluation will be given to each player at the
end of the school, which will note particular strengths and areas that need improvement.
In the past 29 years, with over 12,500 graduates, Sly Park Basketball School has become the most popular basketball camp in
Northern California. For young players learning the fundamentals necessary to become successful varsity basketball players, Sly Park
is “the” basketball school to attend.
The high number of returnees each year supports the reputation of Sly Park Basketball School as the place to improve your
basketball skills.

My son/daughter and I agree to the regulations of Sly Park Basketball School. ENCLOSED IS HALF THE TUITION AS A DEPOSIT. The deposit is not refundable after June
1, 2012 without a doctor’s note. Special arrangements for tuition payments can be made upon request. I authorize the School Directors or Trainer to obtain medical attention
for my son/daughter if he/she is injured or becomes ill while at the school. I have insurance covering my son/daughter in case of an accident or injury.
