Integration of
Health and

Services
Public Bodies (Joint Working) (Scotland) Act 2014
The Act above has made changes to the law that will create statutory
Health and Social Care Partnerships in each local authority in
cooperation with health boards to replace Community Health
Partnerships (CHP’s).
In Lanarkshire two Health and Social Care Partnerships have been
developed, one each in North and South Lanarkshire and from April
2015 these replace existing CHP’s. Discussions between NHS
Lanarkshire and the two Local Authorities are on-going in considering
the range of services that will be managed through the Health and
Social Care Partnerships.
In both North and South a Shadow Integration Board is now in place.
Integration Schemes have been submitted to Scottish Government for
approval, on which a Joint Integration Board will be established. This
service may in the future be included within this range.
Management

The supporting operational and clinical management structures are
focused on enhancing patient safety, quality improvement and local
delivery at hospital level, with visibility of clinical leadership and support
for individual professional accountability. Our Medical Managers will
provide professional leadership to medical staff, ensuring that they are

POST INFORMATION
The Department The department is part of the Surgical, Anaesthetic and Orthopaedic
Directorate and provides anaesthetic services within the 8 theatre
operating suite, which is located on the 2nd floor.
Elective theatre sessions will be advised in a rota published in advance.
The period of advance notice given will be determined by local
agreement
If any of their ‘normal’ direct clinical care sessions are not needed for
any reason, Consultants will be required to be available during that time
to fulfill another departmental commitment. Alternatively, by agreement
with the Clinical Director (CD) or nominated deputy, the Consultant will
make him or herself available for a commitment on a different day/time
from that indicated in the indicative weekly work programme.
Consultants will give the CD or nominated deputy at least 6 weeks
advance notice of requests for re-scheduling of any fixed DCC
commitment or other non-availability. The CD may agree to such
requests at less than 6 weeks notice if DCC activities are not
compromised
There is a modern 10 bedded Adult ITU/HDU adjacent to the theatre
suite which is the responsibility of the Anaesthetic department. Broadly,
these are currently funded to permit 6 level 3 and 4 surgical level 2
beds. There is an additional 4 bedded medical and 4 bedded surgical
level 1 facility within the hospital. A review of these facilities has begun
and an expansion in bed number is likely in the future. All major forms of
organ support are provided.
There are currently 7 consultants with a daytime commitment to ICM.
Our long term goal is to develop a stand-alone consultant rota for ICM.
However, currently the ITU/HDU is staffed at night by a resident trainee
with consultant cover from the whole department. NHS Lanarkshire has
committed to the development of Advanced Nurse Practitioners in
Critical Care to enhance the critical care team.
We provide a rolling in-house programme of postgraduate ICM teaching
and there are weekly meetings in the Department which are coupled
with an educational/development forum. We have close links with the
other 2 ICUs within NHS Lanarkshire, and there are monthly regional
forums via teleconference.

We are fully engaged with the Scottish Patient Safety Programme and
have appointed a dedicated Infection Surveillance Nurse to that end.
The majority of our patients are admitted on the day of surgery and we
are expanding the type of cases we can do as day cases. We are also
actively embracing the ERAS concept and this is developing across
several specialties. Virtually all our elective patients are seen preoperatively by an excellent pre-assessment service.
The department is responsible for the Acute Pain Service which includes
a Pain Specialist Nurse.
NHSL have centralised Vascular surgery to Hairmyres hospital. As a
result there is excellent communication and relationships between
anaesthetists, interventional radiologists and cardiologists on site.
There is a dedicated vascular theatre and access to a recently
modernised interventional radiology imaging room.
Ophthalmology is also centralised to Hairmyres hospital where there are
9 local and 3 general anaesthetic lists per week.
The Anaesthetic department is situated on the 2 nd floor. All the
consultants have an office (shared). There is also an office for the SAS /
trainee staff. There are excellent IT facilities.
Departmental CME meetings are held monthly when all elective surgery
is cancelled. We also run regular M&M meetings during the CME.
Three times a year, the 3 Lanarkshire hospitals hold joint CME
afternoons which are followed by the Lanarkshire Anaesthetists Group
Meeting.
There is a dedicated daily CEPOD list and there are 7 trauma lists
weekly. Weekend duties are split between CEPOD and trauma. There is
also an evening CEPOD list from 5-9pm and surgery commencing after
this time is generally limited to ‘life and limb’ only.
Going forward, there is a possibility that 3 session working days may be
introduced to both Intensive Care and certain theatres. These
discussions are at a very preliminary stage.

POST INFORMATION
The Post
We wish to recruit an enthusiastic doctor, committed to quality
improvement, who has a general interest and who may also consider
daytime sessions in Intensive Care. We will do our best to cater for any
subspecialist interest.
Presently, the on call rota consists of a first-on consultant who covers
the resident ITU/HDU trainee after 5pm and overnight and theatre work
after 9pm. The 2nd-on senior, who may be consultant or SAS, works in
the evening or weekend CEPOD list and is available to the 1st on, as
back-up if required. Currently first on call duties are split between 14
consultants. Second-on is split between 15 consultants and 1 SAS
doctor. The first on consultant is not scheduled for clinical duties the
following day. The second does have clinical duties as the frequency of
being called back is extremely low.
The post is offered as 10 PAs. 2.5 PA’s allocated to predictable and
unpredictable on call on the 1st and 2nd rota. 1 PA is allocated for perioperative care to enable theatres to start promptly at 08:45 and to allow
for occasional over-runs. 1 PA is allocated for SPA which does not need
to be undertaken within the hospital. It would also be expected that an
SPA review take place within the 1st year. This leaves 5.5 PA’s for Direct
Clinical Care sessions. Elective theatres are currently allocated 2 PAs
per day.
Theatre sessions are likely to be a mix between general surgery,
vascular, ECT, orthopaedics, trauma, CEPOD, gynaecology,
ophthalmology, ENT, urology and pre-assessment. However any
specialist interest, including ITU could possibly be accommodated.
The exact configuration of sessions will be determined following
appointment when a job plan will be issued as part of the contract.
The precise details of the job plan will be dependent on the successful
candidate’s subspecialty training and interests.
The job is offered as a full-time post on a 10 PA basis but applications
will be considered from those wishing to work less than full-time. If fulltime up to 2 EPA’s may be available to undertake additional outpatient
and/or procedure sessions or management activity.

All new substantive Consultants are initially offered a minimum of 1 PA
for SPA duties but this will be reviewed within 6 months (or earlier if
required) of appointment and revised upwards if additional
responsibilities have been undertaken. Additional SPA time (up to 2.5
PA’s in total) can be incorporated into the job plan depending on the
time required to support the successful candidate’s professional
activities and the needs of the service.
NHS Lanarkshire is supportive of applications from individuals with well

commitment to quality improvement, patient safety (including human
factors training), Medical Education and Research & Development.
Applicants wishing to commence on more than 1 PA for SPA duties

DUTIES AND RESPONSIBILITIES
Main Duties
The main duties and responsibilities of your post include:



Work
Programme
The provision of anaesthesia as outlined above. The days on which
these clinical commitments take place will be fixed as far as possible
though it is expected that you will be flexible with your duties in
keeping with other members of the department. There will be some
movement of days depending on the on-call arrangements but
notice of changes will be given in accordance with NHS Lanarkshire
policies.
Contribution to the consultant on-call rota covering ITU / HDU
Contribution to the evening and weekend CEPOD (emergency
surgery and trauma)
As required under Section 3 of the Terms and Conditions of Service, the
duties and responsibilities are supported by a job plan and work
programme detailed as follows:

work. 1 PA is a basic anaesthetic practice allowance which recognises
pre-, post-, and peri-operative work or its ICU equivalent (early start/late
finish) as appropriate

subspecialty training and interests. A minimum of 1 SPA will be included

On taking up post a Job Plan will be agreed between the person
appointed and the Clinical Director/Associate Medical Director. This job
plan is subject to review at least once a year by the post holder and the
Clinical Director/Clinical Lead as noted in the terms and conditions. The
procedures set out in the ‘Terms & Conditions of Service’ must be
followed if it is not possible to agree a job plan, either initially or at an
annual review.
The timetable is indicative and subject to negotiation with the Clinical
Director / Associate Medical Director. The core 10 PA working week will
be based on 9 PA’s of Direct Clinical Care (DCC) duties and 1 PA of
SPA time for a full-time post-holder.

b)
This appointment is superannuable under the NHS Superannuation Scheme, which is
contracted out of the State Earnings Related Pension Scheme (SERPS). All new
eligible employees commencing employment from 1 March 2013 onwards will
automatically become a member of the pension scheme and your remuneration will be
subject to deduction of superannuation contributions, current personal contributions
range from 5% for the lowest earners to 14.5% for the highest. Levels of contributions
and scheme regulations are subject to change, any changes will be communicated to
you or can be found on the SPPA website. Contributions are taken from your pay
before tax, so you get tax relief on any amount you pay. This can reduce the actual
amount that you pay between approximately 3.5% and 5.1% depending on your
contribution rate, earnings level and personal rate of tax. In addition to personal
contributions NHS Lanarkshire will contribute 13.5%.
In accordance with recent legislative changes, new eligible employees commencing
from 1 March 2013 onwards will automatically be enrolled in the pension scheme. After
considering the benefits and personal circumstances, you can if you wish opt out of the
scheme. To opt-out, you must complete an opt-out form which is available on the
Scottish Public Pensions Agency website and return this to the Payroll Department,
Kirklands Cottage, Kirklands Hospital. Should you choose to opt-out in accordance
with these changes you will be enrolled into the scheme again every 3 years (June
2016, 2019 etc).
Under the NHS Superannuation Scheme Regulations pension records for short term
appointments, such as Bank Staff and the equivalent kind of contracts for other staff
groups, may remain open for short breaks in pensionable employment up to but not
more than 3 months from the date on which they last paid contributions. During this
period the member will continue membership in qualifying but not pensionable terms.
Where the period of break exceeds 3 months, membership of the scheme will be closed
and leaver information will be submitted to the Scottish Public Pensions Agency.

From 1 April 2015 the NHS Pension Scheme is changing in line with other public
pension schemes. All those joining the scheme for the first time from 1 April 2015 will
join the new scheme. Existing members who are not protected will transfer to the new
scheme, with benefits in previous scheme(s) preserved. The new scheme will be based
on a proportion (1/54th) of the member’s pensionable earnings each year. The pension
will be the sum of each year’s value built up over the length of the member’s career and
revalued each year in line with the rate of inflation. This is called a Career Average
Related Earnings scheme. Normal pension age will be linked to the member’s individual
state pension age.
Some existing members of the scheme will be protected, more information is available
on the SPPA website.

c)
The employment is subject to 3 months’ notice on either side subject always to the
appeal and other provisions of paragraphs 10.4 & 10.5 of the Terms and Conditions of
Service of Hospital Medical and Dental Staff and Doctors in Public Health and the
Community Health Service Consultant Grade.
d)
Reimbursement of certain telephone charges may be payable on application to NHS
Lanarkshire. The successful candidate must be contactable throughout any on-call
period.
e)
The successful candidate, if not already employed by the NHS Lanarkshire, will be
required to complete a medical questionnaire to obtain medical clearance from the
Occupational Health Physician.
f)
The successful candidate will be required to complete a disclosure Scotland PVG
(Protecting Vulnerable Groups Scheme) form. No approach will be made without written
permission of the successful applicant who will be asked to sign and complete a
Disclosure PVG application, giving authorization for the check to be undertaken.
g)
NHS Lanarkshire does not negotiate salary placements. On commencement the salary
will be in line with paragraph 5.1 of the terms and conditions of the new consultant
contract. Appointees start on the scale minimum except in the circumstances of
paragraphs 5.1.2 – 5.1.7 of the terms and conditions of service.
h)
From 1st April 2014 the starting salary for the post is £76,001 per annum (based on 10
Programmed Activities per week). The successful candidate’s total salary will be
dependent on his/her previous service history. Remuneration for any extra programmed

i) Dependent upon present place of residence, NHS Lanarkshire may require the successful
candidate to undertake to remove home to a distance acceptable to NHS Lanarkshire,
normally within 10 miles of the base Hospital, in which case removal expenses will be
payable in accordance with NHS Lanarkshire Policy. In some cases, however, a
residence more than 10 miles distant from the base hospital will be acceptable and in
this case removal expenses will be subject to the prior approval of the Associate Medical
Director for such a residence. The position will be made clear to the successful

For the purpose of immigration status, overseas doctors and dentists are
those who, regardless of where they obtain their primary medical/dental
qualification, are not nationals of the European Economic Area (EEA). A
list of EEA countries is attached.
On 7th March 2006 the Department of Health announced changes to the
immigration rules for postgraduate doctors and dentists. The changes
came into effect on 3rd April 2006 and mean that all doctors and dentists
who wish to work in the UK from outside the EEA will need to meet the
requirements of an employment category such as the work permit
provisions. New applications for permit-free training visas will no longer
be granted for doctors undertaking training posts
Non-EEA doctors and dentists will still be able to come and train in the
UK and are still eligible to apply, in open competition for training posts
provided all the other eligibility criteria is met. However, training posts
will now be considered employment for immigration purposes. Work
permits will only be issued where there is a specific post for the
overseas doctors that cannot be filled by someone from the EEA.
The changes in immigration requirements for trainee doctors and
dentists are a result of a Scottish Executive Health Department
announcement and immigration issues are the remit of the Home Office.
