To receive contact hours for this continuing education activity, the participant must:
 Sign in on provided Attendance Roster
 Attendance requirements: Must arrive at least 5 minutes prior to start of presentation & be present for the entire activity
to receive contact hours.
 You must submit a completed Evaluation Form and Certificate Request Form
 These will be sent to the Support Center and the Certificate of Successful Completion” will be mailed to you.
