Our Practice operates an appointment system for all surgeries. To make an appointment please telephone or call during opening hours. If your problem is urgent you will be seen on the same day, if your problem is not urgent you will be seen within 2 days.

This is for emergencies only. There is always a Doctor on call.

How to Register at the Practice

You can register at the Practice if you live in our area. You will need to fill in a questionnaire and bring in your medical card and a photo ID e.g. driving licence or passport. The Doctor will require you to come into the Practice for a new patient check.

DNA Appointment

Due to the high rate of patients not attending appointments this will be stated in your medical records and the Doctors will act accordingly.

How to Make a Complaint

We have a complaint’s procedure. Please ask reception for details.

Violent or Abusive Behaviour

We have zero tolerance to patients with abusive or violent behaviour. If this occurs, you will be asked to leave the

Surgery and change Practices.

Disabled Access

The Practice has facilities for disabled patients.

Access to Patients Information

Clinical information is shared, where appropriate, amongst clinical staff to the benefits of patients.

Repeat Prescriptions

Please give at least 48 HOURS notice for a repeat prescription. Requests can be made by either:

Telephone 0114 253 0307 between 11:00am - 1:00pm

AND 3:00pm - 5:30pm.

Letter to the Surgery including a stamped address.

Envelope.

In person.

Please help us by stating the medication that you need. Please see a doctor at least every 6 months if you are on regular medication.
