UPRIVER GARBAGE SERVICE ENROLLMENT FORM
Thank you for buying prepaid garbage bags. According to the schedule you set below, your garbage
will be picked up at your front door on Fridays by staff of the Yurok Tribe Environmental Program.
With each garbage bag purchased, you will receive a smaller bag for recyclables; you may put any
recyclable materials in this bag, including glass, plastic, aluminum, and steel food containers.
