of Expenditure Request Form. Tickets can be purchased by TA accounts or personal
checks/money orders. Please make checks payable to Excellence in School Nursing.

Medical Services) on May 9th, from 11:00 am to 2:00 pm. The contact person, or designee,
should pick-up tickets and distribute.
