Service Description
 The Community Mental Health Nursing Service for Older People provides high
quality nursing care for elderly people in their own homes.
 The Community Mental Health Nursing Service is centrally based at Bensham
Hospital but is in sectors in line with the Consultants in Old Age Psychiatry.

 This means that every GP surgery will have a named Community
Mental Health Nurse for this client group.
 Any person over the age of 65 who has a mental health problem can be
referred to the CMHN Service.
 Although dementia sufferers are one of our largest client group we
also see elderly people with other or multiple needs including
depression, anxiety, loss and those elderly people with severe and
enduring mental health problems (e.g. schizophrenia).
 As a service we take referrals from both GP’s in the sector and from
consultants linked to the practices. We support individuals whose needs
are under primary care.
 Following CMHN assessment, we will refer those requiring more indepth assessment to the consultant liaising with the GP. In these
instances we will request an updated medical history and medication
list.

Following an assessment by the Community Mental Health Nursing Service, we will
forward a written report of this to the referring GP.
This will include a summarised nursing care plan and recommendations if appropriate.
The Community Mental Health Nursing Service will continue to visit according to this
plan and communicate as required with the referrer.
Discharge from the Community Mental Health Service will also be in written form.

Referral can be made preferably in written form, using the standard patient Referral Form (PRL1) or
on our own proforma overleaf and sent to the address below. Alternatively referrals will be taken by
telephone (number below) with the provision of a follow–up written referral. Faxed referrals are also
acceptable (number below).
Patients with memory problems under the age of 65, should be referred to the Consultant for the
relevant sector.

Following assessment if Consultant involvement is required, a direct referral can be made by the
CMHN with the GPO’s agreement, and additional up-date of medication and medical history.
