Clinical Pharmacist

JOB DESCRIPTION:
1.0
JOB SUMMARY:
To assist the Senior Clinical Pharmacist and Director of Pharmacy in managing all
aspects of dispensing practices within the Pharmacies.
2.0
DUTIES AND RESPONSIBILITIES:
2.1
Serves as clinical response for other pharmacy personnel, medical, nursing
staff and other healthcare professionals.
2.2
Actively participates in patient care rounds with other members of the
healthcare team.
2.3
Reviews and interprets drug chart for the dosages, drug interactions and
appropriate suggestions to the healthcare teams.
2.4
Provides drug information to healthcare professionals.
2.5
Gathers and processes medication orders and counter checking the prepared
discharge medications for accuracy and assures timely delivery of the
medications to the wards.
2.6
Provides in service education lectures or publishing the articles in hospital
newsletter and attends the meetings of Pharmacy & Therapeutics Committee,
TQM, Infection Control, etc.
2.7
Documents the therapeutic recommendations (like TDM for narrow safety
margin index drugs) in the patient’s medical record and pharmacy intervention
form.
2.8
Reports adverse drug reactions.
2.9
Follows all hospital related policies and procedures.
2.10
Performs other related duties as assigned by the Director of Pharmacy.

HOURS OF WORK:
48 hours per week.

REQUIREMENTS:
Qualifications or Relevant Experience:

4.1
A degree in Pharmacy, B.Sc, B. Pharm. or equivalent.
4.2
A clinical post-graduate qualification in pharmacy will be a definite advantage.
4.3
One year experience in Hospital Pharmacy as Clinical Pharmacist.
