Thank you for allowing us to participate in your healthcare . Our practice is dedicated to the comprehensive care of all
breathing abnormalities and sleep related disorders associated with snoring and obstructive sleep apnea.
Our medical staff consists of: Doctors; Jack H. Dadaian, Barry J. Weber, Efthymios I. Daniskas and Francesco Califano, all trained
and board certified in the specialty of lung disease. The group is on staff at Mountainside Hospital, Clara Maass Medical Center,
Columbus Hospital (closing June 2008) and courtesy at St. Barnabas Medical Center.
Insurance:
Participating Plans: Our office participates with Medicare and most managed care plans. You must bring your insurance identification
cards with you. We cannot render care to patients who do not have their plan cards or some other information from the insurance
carrier. Each insurance carrier has different guidelines that must be followed: i.e. referral forms from primary care doctors or
precertification rules and regulations that must be followed. Please help us help you to comply with your carriers rules by
understanding your own policy guidelines. Remember, it is ultimately the patients’ responsibility to ensure all providers follow the
appropriate rules and regulations.
Referrals: If your medical insurance requires a referral from your primary care physician, you must have it with you. You cannot be
seen under any circumstance without your referral. We can not accept faxed referrals from your primary care physician.
Co payments are due at the time of your visit.
What you will need: Please bring a list of all current medications you have been taking. If you have had a recent (last two months)
chest x-ray or chest CT scan, please bring the films and report with you.
Aetna, Cigna and Horizon NJ Health plans do not allow our office to perform chest x-rays. Please have your primary care doctor
arrange for a chest x-ray BEFORE your visit however, if you are coming only for SNORING and SLEEP APNEA you DO NOT have
to bring a chest x-ray film.
Appointments: Patients are seen by appointment only. We go by the appointment time and not the time the patient arrives. We will
do our best to honor your appointment time. We realize your time is valuable, but in our specialty emergencies or other unexpected
occurrences may cause unforeseen delays. If this does happen, we will try to inform you as soon as possible. Most days we have two
doctors seeing patients at the same time. While you are waiting we may call on a patient who arrived after you. If this happens, it
usually means that this patient is seeing another doctor. Please make sure you come to the window when you arrive.
Appointment Confirmations: As a courtesy, we have an automatic service that will call you two-three days prior to your appointment.
Please, if you discover you cannot make your appointment, notify us at least 24 hours prior so we can use this valuable time for another.
Telephone Calls: The telephone can be an important link between you and your doctor. Our support staff is highly trained and
capable to answer many of your questions. We encourage you to take advantage of their knowledge and expertise. If your concerns
require the doctors’ direct attention, please be assured that he will return your call as soon as possible.
Emergencies: If an emergency arises during regular business hours, immediately call the office and tell the receptionist or answering
service that the call is an emergency or go directly to the nearest emergency room.
Medication Renewals: Between your visits to the office, you may need your medications renewed. This can only be done when the
office is open and at least 2-3 days before your medications run out. We must review your chart before refilling prescriptions therefore;
this service cannot be provided during the weekend or evening hours.
Medication Preauthorization: We will only preauthorize medications if there is not an acceptable equivalent available.
Patient Recalls: If you require a recall for a repeat test; i.e. 3 month CT scan etc… our automated service will call to remind you it is
time to schedule your test. After you receive this call you must then call our office to initiate the process. Make sure you give us any
recent insurance changes (if any) at that time.
