Primary Care Nurse Practitioner
Job details

Post title: Nurse Practitioner

Responsible to: General Practitioners, Practice manager and Nurse Team Leader

Qualifications: RGN. Relevant diploma level qualification(s). Minor Illness qualification or
relevant experience preferred.

Nurse Independent prescriber – preferred, or willing to undertake.

Place of work: Student Health at SHU and Porter Brook Medical Centre

Pay according to qualifications and experience.
Job purpose:
To provide comprehensive healthcare sensitive to the needs of the population, including
assessment, diagnosis and prescribing of care, referring as appropriate. To enable patient choice in
the provision of care, complementing the roles of the GP and multidisciplinary team and promoting
the development of advanced nursing practice.
The role
1. Clinical:

To provide first point of contact role in managing an undifferentiated caseload in both acute and
chronic illness.

To provide a comprehensive health assessment, including physical, mental, emotional, and
social health

To undertake consultations in surgery or over the telephone

To assess, plan, implement and evaluate patient care

To provide support and informal counselling for patients with emotional difficulties, pre and
post termination, life style problems, eating disorders, etc.

To participate in the health promotion activities of the practice

To make referrals to, and receive referrals from other health care professionals.

To demonstrate a level of knowledge clinical skill appropriate to the needs of the role

To order and interpret tests as appropriate to patient need

To discuss test results and their implications with patients

To prescribe medications as a nurse independent prescriber
2. Management and leadership:

To participate in the development of practice philosophy, strategy and policy

To be involved in the management of change within the practice and the implementation of
systems to improve patient care.

To participate in, and contribute to, the clinical governance agenda of the practice.

To assist with the management of chronic disease programmes

To assist the practice in achieving the GMS2 Quality and Outcome Framework

To evaluate and audit care

To participate in multidisciplinary protocol development

To ensure the maintenance of supplies and equipment
3. Training and education

To contribute to the training of GP Registrars, Medical and Nursing students, and other students
and members of staff as required

To contribute to the clinical meetings within the practice

To be available as a clinical and educational resource to other nurses outside of the practice as
appropriate.

To maintain own professional development and PDP

To participate in the practice appraisal process
4. Communication

To maintain accurate and contemporaneous records

To work with the wider nursing team in developing an integrated service for patients.

To make referrals within and without the practice team, and to voluntary and statutory agencies
as necessary.

To engage with the CCG and the wider health community on issues relevant to nursing.
5. Professional

To work within the NMC Code of Professional Conduct and Scope of Professional Practice

To undertake professional development as required by the role.
6. Confidentiality and Data Security

You are required to preserve the confidentiality of any information regarding patients, staff
(in connection with their employment) and the Practice business and this obligation shall
continue in perpetuity.

You have a responsibility to comply with the Data Protection Act 1998, NHS
Confidentiality guidelines (e.g. Caldicott, GMC.) and any code of practice on
Confidentiality and Data Protection, as accepted by the practice.


