Introduction
Job Title:Consultant Psychiatrist in Perinatal Psychiatry

NHS Lothian is seeking to appoint two Consultants in Perinatal Psychiatry (combined
1.4 wte) to work together in leading the Lothian Perinatal Mental Health Service. The
inpatient part of the service was recently awarded an “excellent” rating by the
RCPsych Quality Network for Perinatal Mental Health. There are 2 components: the
6 bedded East of Scotland Mother and Baby Unit providing dedicated in-patient care
for mothers and infants from Lothian, Borders, Fife, Tayside and Highland regions
(approximately 20,000 live births per annum); and the Lothian Perinatal Community
Mental Health Team - an experienced multidisciplinary team providing specialist
community care across Edinburgh and the Lothians, (approximately 10,000 live births
per annum).
The MBU and community base are co-located at St John’s Hospital, Livingston (10
miles to the West of Edinburgh. The Perinatal service forms part of the West Lothian
Mental Health Services, other components include comprehensive community mental
health services across all age groups, in-patient wards for general adult and old age
psychiatry, an IPCU, a regional eating disorders unit and a liaison psychiatry service.
This is an excellent opportunity to join a first class team in an active and progressive
teaching hospital environment based in Edinburgh and the Lothian’s.

Main Duties and Responsibilities
Clinical
To take a clinical lead in the provision of perinatal psychiatric services to the
designated catchment area, taking medical responsibility for patients under
Lothian Community Perinatal Mental Health Service and inpatients on the M&B
Regional Inpatient Unit.
To undertake assessments, risk assessments and treatment planning in
conjunction with the multi-disciplinary team within both community and inpatient
services
To promote a multi-disciplinary/multi-agency approach and provide consultancy
support to General Psychiatric Services, maternity services, primary care teams,
social work services and voluntary sector agencies across the Consortium
catchment area.
To work proactively with families and carers of individuals under the care of the
service and inpatient unit.
To take a lead in the consideration of infant mental health and promote the needs
of the baby as a key priority when health care is delivered to mums across NHS
Lothian
To act as an Approved Medical Practitioner and as Responsible Medical Officer
within the remit of the Mental Health and Incapacity legislation.
To participate in the on-call rota providing general psychiatry cover to West
Lothian during the day and outwith working hours, and to work collaboratively with
general psychiatrists to provide cover for annual and study leave
NB These postholders are not expected to provide medical cover for the babies on
the inpatient unit – this will be provided through a separate arrangement with
appropriate services
Liaison With Other Agencies
To maintain working links and provide expert advice to other stakeholder agencies
and groups across the consortium area
To provide a psychiatric liaison service to St Johns maternity unit
Where appropriate, preparation of reports and attendance at child protection case
conferences. However, for the regional health boards, the emphasis will be on
local services taking on this responsibility in the community, with liaison
advice/support from these postholders
Management
To contribute to the continuing strategic development and evolution of mental health
services overall and perinatal mental health in particular, in line with national and
local guidance, including Framework for Mental Health Services in Scotland 1 and
Community Health Partnerships as outlined in the White Paper Partnership for Care2.
To actively participate in the NHS Lothian Clinical Governance programme, including
active involvement in clinical audit, critical incident reviews and complaints
management.
To lead the service development of the Lothian Perinatal Community Mental Health
Team
To provide specialist perinatal mental health supervision to all members of the
Lothian Perinatal Community Mental Health Team
Education, Training & Research
To participate in the training and development of staff, including medical students on
attachments, nursing staff and other medical staff.
To provide supervision to the FY2 within the team. From time to time there will be a
SpR attached to the service for whom appropriate supervision will also be required.
To participate in audit, critical incident reviews and the Clinical Governance
Infrastructure
To participate in the local post-graduate programme. There is also an opportunity to
become involved in the MRC Psych teaching Course.
To create and deliver an educational programme for stakeholders and referrers
within Lothian, including GPs, midwives, health visitors, social workers,
psychologists, obstetricians, psychiatrists and CPNs.
To create and deliver an educational programme for regional stakeholders and
referrers, including psychiatrists and CPNs.
To participate in public health projects locally and nationally relating to perinatal
mental health, including developments of guidelines, participation in the Scottish
Perinatal Mental Health Forum, and Child and Adult Protection meetings.
Location
It is anticipated the principal base of work will be St John’s Hospital but the role will
require the postholders to work at other sites within NHS Lothian.
On-call
The post holders will be expected to participate in the psychiatric consultant on-call
rota for the West Lothian psychiatric service out of hours, (currently a 1:9 rota),
covering old age psychiatry, mother and baby unit, eating disorder unit and general
psychiatry.

Revalidation Performance Management
Comply with revalidation and appraisal process. Revalidation commenced on 3 rd
December 2012 and is the process by which licensed doctors are required to
demonstrate on a regular basis that they are up to date and fit to practice. Licensed
doctors have to revalidate, usually every 5 years, by having regular appraisals with
their employer. You are required to ensure:

Medical Staff Management

Departmental and Specialty Information
The Mental Health Services in West Lothian are managed by the local Community
Health and Care Partnership. However they form part of the Lothian wide services
and are integral to the Lothian Mental Health and Wellbeing Strategy. All psychiatric
consultants working at St John’s Hospital are expected to engage in regional
planning for their particular specialty and relevant continuing professional
development activities.
In-patient Mental Illness services, including an Intensive Psychiatric Care Unit, and
Care of the Elderly with Mental Disorder beds are in St John’s Hospital, Livingston.
There are two regional inpatient services covering the South East of Scotland, a
Perinatal Psychiatry Mother and Baby Unit and an Eating Disorders Ward.
Day patient services are provided at a Day Hospital for the Elderly with Mental
Disorder, at St John’s Hospital, and 5 Community-based Rehabilitation Day Hospitals
[including depot and Clozapine clinics]. A peripatetic service provides day care for
the Elderly with Mental Disorder at a range of community resources. Clinical
Psychology, Occupational Therapy and Psychiatric Nurses run a range of
psychotherapeutic groups in St John’s Hospital.
There is a wide range of out-patient clinics, both hospital and primary care based.
With Clinical Psychology Services and Community Psychiatric Nursing also based
within Health Centres, this offers the opportunity for close networking and liaison with
General Practitioners and other community staff. A community outreach team
comprised of Consultant Psychiatrists, Community Psychiatric Nurses and Social
Work staff provides a service to patients with a severe and enduring illness.
There is a dedicated team for Mental Health Social Work and another for Mental
Health Officers and Adults with Incapacity.
The Clinical Psychology Department is also part of the CHCP. The department
offers dedicated sessions to Care of the Elderly with Mental Disorder, Child and
Family Psychiatry, in addition to 8 primary care based clinics.
Occupational Therapy and Physiotherapy Services are provided from within St
John’s Hospital by the respective departments. The Occupational Therapy service is
especially active in Mental Health.
Staffing
West Lothian Health and Care Partnership currently employ 12 Consultant
Psychiatrists (excluding these posts), including 2.0 wte in the Care of the Elderly with
Mental Disorder and 0.5 wte in Eating Disorders.
Supporting Medical Staff
There are 2 Specialist Trainee posts based in West Lothian that are part of the South
East of Scotland Psychiatric Rotation. These posts offer training in Care of the
Elderly with Mental Disorder and Child and Adolescent Psychiatry. There is 1 core
trainee post. 3 General Practice Vocational Training posts and 3 Foundation Year 2
posts, providing experience in General Psychiatry and Care of the Elderly. There is a
Foundation Year 1 post in Liaison and Perinatal Psychiatry.
Specialty Trainee Registrar attachments are currently available in General
Psychiatry, Liaison Psychiatry, Rehabilitation, Care of the Elderly with Mental
Disorder and Child and Adolescent Psychiatry.
Organisation of Psychiatric Services
Adult Psychiatry including Rehabilitation
The model of general psychiatric care and the organisation of West Lothian services
was last reviewed in 2009. Clients with the most complex ongoing needs are cared
for by 1.4 wte Consultant in Rehabilitation Psychiatry, using services like the
Community Outreach Team and Day Services.
4.0 Consultants in General Adult Psychiatry are responsible for the acute care of
other patient groups and for a sessional commitment to locality based service
supporting primary care
Care of the Elderly with Mental Disorder
Two Consultants are currently identified as having a special responsibility for the
Care of the Elderly with Mental Disorder. They share responsibility for the 24 bedded
Assessment Unit within St John’s Hospital, the day-care services and a total 60
community based old age psychiatry beds for patients with dementia and challenging
behaviour. An additional post has recently been developed.
Liaison Psychiatry
A service is provided for St John’s Hospital at Howden by a Consultant Liaison
Psychiatrist. This includes the assessment of patients presenting after deliberate
self-harm.
Forensic Psychiatry
One Consultant is responsible for the running of the Intensive Psychiatric Care Unit
at St John’s Hospital. A supra-regional in-patient Forensic Unit is on the Royal
Edinburgh Hospital campus.
Child and Adolescent Psychiatry
The Child and Adolescent Psychiatry service based at St John’s Hospital is part of
the Lothian CAMHS service. A regional inpatient unit is part of the Royal Edinburgh
Hospital campus. A day patient unit operates in Livingston and an outpatient service
is offered at St John’s Hospital.
Substance Abuse Services
There is a regional service for people with Drug and Alcohol Problems which includes
a Consultant Psychiatrist. This provides dedicated community services in West
Lothian and in-patient alcohol services are provided at the Royal Edinburgh Hospital.
Perinatal Psychiatry
The East of Scotland Mother and Baby Unit and Lothian Perinatal Community Mental
Health Team are based at St John’s Hospital. The out of hours on-call rota for West
Lothian provides cover for this.
Eating Disorder Services
A new inpatient Eating Disorder Unit was opened at St John’s Hospital in 2012. The
out of hours on-call rota for West Lothian provides psychiatric cover for this.
Learning Disability Services
These services are provided across Lothian, and West Lothian is served by a
community team with input from a Consultant Psychiatrist.
Population and Services
Catchment Area, Bed Numbers and Staffing
West Lothian has a total population of 180,000. The General Adult Psychiatry
Service covers all patients between the ages of 19 and 64 years, although there will
be continuing responsibility for some patients with functional illness over the age of
65. Patients under the age of 65 with possible dementia are generally initially
assessed by general psychiatry, who will usually initiate diagnostic investigations
before transferring to the old age service for follow up.
There is one in-patient general psychiatry ward in St John’s Hospital with 24 beds.
The responsibility for patients admitted to this ward falls on 4 general adult
psychiatrists. The consultants are responsible for patients admitted from their own
catchment area and they are allocated responsibility for patients admitted from out
with West Lothian on a rota basis.
There are separate in-patient wards for Intensive Psychiatric Care and Rehabilitation
that have their own consultants.
Acute Care and Support Team (ACAST)
This is a nursing team that are based in St John’s Hospital and who work with junior
medical staff in the urgent assessment of patients presenting to the Accident and
Emergency Department. ACAST also provide time-limited intensive home treatment
for patients with acute or relapsing psychiatric illness. General Adult Consultants
remain clinically responsible for patients from their catchment area, who are under
the care of ACAST
Outpatient Clinics
General Adult Psychiatry outpatient clinics are held at St John’s Hospital and at a
number of community bases including some GP surgeries. All non-urgent GP
referrals are discussed at primary care based triage meetings held fortnightly.
Research Audit and Teaching
Undergraduate medical students from Edinburgh University are attached to the
Old Age Psychiatry Department on a regular basis, for a period of 2 weeks.
The Mental Health Directorate has an active multi-disciplinary clinical audit group. All
medical staff within the hospital are expected to participate in Medical Audit, both by
their involvement in ongoing projects and by initiating review of their own work
practices.
The Consultant will be expected within his or her NHS responsibilities to undertake
research and to participate in undergraduate and postgraduate teaching as required.
As stated in the advertisement, the Consultant may be offered an appropriate
honorary appointment by the University of Edinburgh.
In certain circumstances, where the Consultant concerned has responsibilities for
organising teaching in a particular discipline or has a research interest which extends
beyond the duties of his/her NHS post, the Trust and the University may agree that a
Consultant be offered a part-time Senior Lecturer appointment with specific sessional
allocation. Such appointments will be subject to reconsideration by the Trust, the
University and the Consultant at intervals of not less than three years, and, in the
event of the contract being modified by the reduction or elimination of academic
sessions, the Consultant would be entitled to continue in the same number of total
sessions, all of which would then be devoted to Health Service work.
If the successful candidate wished to be considered for a part time Senior Lecturer
post, it would be necessary for the appointee to make special application to the
Division giving full details of the work to be undertaken.
There is an active psychiatric postgraduate programme at St John’s Hospital
every Tuesday from lunchtime during academic terms. Consultants and other
members of the teams present cases, journal clubs and there are also external
speakers. There are also regular CPD sessions.
5.
Job Plan and Conditions of Service
The rationale behind pooling sessions for the in-patient and community services
is that this will ensure flexibility in how the successful candidates choose to share
clinical and other duties. The job plans provided below are illustrative and
presume that one candidate wishes to work full time and accept responsibility
for the in-patient unit and the other wishes to work half time, primarily with the
community team. The final job plans will be agreed with the successful
candidates and approved by the Associate Medical Director. In the event of two
successful applicants both wanting to work full time options such as
incorporating some general adult or liaison psychiatry duties would be explored.
The Scottish Medical Managers group currently insists that all consultant
post in Scotland be advertised with a single SPA (reflecting the minimum required for
mandatory activity such as appraisal, personal audit and professional development).
However, once the candidate has been appointed more SPA time may be agreed for
activities such as undergraduate and postgraduate medical training which takes
place outside direct clinical care, as well as research and/or management. These
activities are specifically and clearly identified and agreed with the candidate.
An appraisal system for reviewing personal development and job planning will
be undertaken annually.

The Job Plans will be a prospective agreement that set out the consultant’s duties, responsibilities and objectives for the

NEGLIGENCE
In terms of NHS Circular 1989 (PCS) 32 dealing with Medical
Negligence the Health Board does not require you to subscribe to a
Medical Defence Organisation. Health Board indemnity will cover
only Health Board responsibilities. It may, however, be in your interest
to subscribe to a defence organisation in order to ensure you are
covered for any work, which does not fall within the scope of the
indemnity scheme.

Employment is subject to three months’ notice on either side, subject
to appeal against dismissal.
BASE You may be required to work at any of NHS Lothian’s sites as part of
your role.

acceptable behaviours when using social media. It sets out the
