Instructions for Fieldwork Form
1. If you have not already saved the form to your PC, click on file &
save as and save the form to your PC.
2. Complete the 3 pages of the form by using your TAB key to move
through the form.
3. When the form has been completed, close and save your changes
and then use the file attachment in your E-mail to attach the
fieldwork form and mail it to Dr. McCook at
kmccook@tampabay.rr.com
4. If you are using your USF mail account, use the browse button to
locate your fieldwork form and then use the attach button to affix the
form to your mail message.
5. Please remember to follow the instructions at the bottom of page 4
concerning the “Required Cover Sheet for Submission of Final
Report.”

Project or Specialization
The purpose of the optional project or specialization is to provide an opportunity
for the student to focus on an aspect of service. The goal is to allow students to
demonstrate their capacity for self-direction and creativity.
The project should be defined, with goals and objectives. (If one chooses the
optional project, the description should be included below, otherwise enter
NA)
Examples of Projects:
1. Creation of a Web page for a Library Consortium
2. Writing an electronic resource collection development policy for a
university library
3. Development of bilingual public relations materials for rural farm workers
to increase their use of the public library
4. Digitization of a seaman’s journal from the Caribbean archives collection
with hyperlinks to other related sites.
