Routine Antenatal Support For Teenage Pregnancy
Pregnancy Confirmed - Advised regarding antenatal care, care pathway handed out to client and flow chart to be followed.
ICF completed and forwarded to relevant agencies following Inter-agency flow chart. If under 16 please refer to Children and
Young People Services and forward to safeguarding lead. Follow Unborn baby protocol from South West Child
Protection Procedures throughout pregnancy and refer to Children Services as appropriate. If under 16, or medical or
obstetric problem please book for consultant led care under Mr Narayanan. Public Health Midwife (01803 654644) to
forward to FHP.
20 Week Dating Scan - For review by Consultant in specialist clinic if under 16, medical or obstetric problem
28 Week Review (called by midwife) - All professionals engaged with the client’s meet with client and family. Date, time
and venue to be co-ordinated by midwife. To include midwifery, health visitor, connexions, family health partnership and
children services if involved at this time. Please use Pro-forma for minutes. Discuss any concerns and distribute as
appropriate. Consider a need for a CAF to ensure support is in place or referral to Children and Young people Services
following the Unborn baby protocol. Set date for 34 weeks meeting. N.B invite client to this meeting. If the case is
already Child Protection please do not duplicate meetings. Please send copy of minutes to parents, public health
midwife and all attendees. Maternity Care Assistant (MCA) to make contact with the Young Person.
34 Week Review Meeting - To include all professionals involved. If concerns were raised at 24 weeks social worker to be
invited. A client focused meeting to agree support plan for prior to, during and after the birth. Any concerns to be discussed
openly with the client. Follow the Pro-forma for minutes and distribute as point 3 states.
33-34 Week Pre Birth Planning Meeting - Midwife and client to discuss birth plan i.e. pain control, PN care, breast feeding
please ensure the MCA has visited
