Fixed-term 12 months
This post offers an exciting professional development opportunity for an experienced pharmacist to further
develop their own clinical skills and contribute to the implementation of the Scottish Antimicrobial
Management of Resistance Action Plan ScotMARAP 2 within NHS Lothian. You will have both a primary and
secondary care remit to facilitate improvements in antimicrobial prescribing through implementation of
prescribing audits, provision of advice on appropriate antibiotic prescribing to clinicians and delivery of
education. Also, as an integral member of the Antimicrobial Management Team you will monitor strategies
and policies for evidence-based, cost-effective use of antimicrobial agents throughout NHS Lothian.
Good organisational and communication skills are essential and you will be able to work alone and as part of
a team. A postgraduate qualification in clinical pharmacy and clinical experience in a variety of specialties

2. JOB PURPOSE
1. To contribute to the overall strategic planning for NHS Lothian for the provision of infection control
services and to participate in the development of pharmaceutical services for patients requiring
antimicrobials across Scotland.
2. To co-ordinate, develop and deliver specialist clinical pharmacy services including antimicrobial
stewardship ward-rounds and to work with patients, health care and other professionals to provide
the required pharmaceutical care for a defined patient group in any care setting (includes both
hospital and community).
3. To provide expert advice in all pharmaceutical matters relating to antimicrobial use.
4. To contribute to the development and delivery of specific education and training opportunities relating
to antimicrobials for clinical staff in NHS Lothian, including doctors, nurses, pharmacists and other
antimicrobial prescribers.
5. To undertake clinical and financial risk management associated with the use of antimicrobial
medicines ensuring compliance with relevant legislation, standards and protocols. To implement
effective measures and initiatives to support cost-effective prescribing of antimicrobial therapy.
6. To provide evaluated prescribing reports, advice and education on appropriate antibiotic stewardship
to GP’s and other healthcare professionals within primary and secondary care
3. DIMENSIONS
1. Responsible for participating in development of systems to improve antimicrobial prescribing within
all care settings in NHS Lothian including general hospital (wards and clinics), community hospitals
and other community settings.
2. Supports the development of the Antimicrobial Management Team, in collaboration with the Lead
Doctor, consultant microbiologists and infectious diseases consultant.
3. Links with the other antimicrobial pharmacists nationally.
4. Influences, provides guidance and teaches clinical and pharmacy staff regarding antimicrobials.
a. Staffing
As a member of the NHS Lothian Antimicrobial Management Team the post-holder will coordinate 60wte clinical pharmacists to promote effective antimicrobial use and undertake audits of
antimicrobial usage within NHS Lothian.
b. Budget
Prescribing expenditure for antimicrobial therapy in NHS Lothian is approximately £10 million per
annum.
4. ORGANISATIONAL POSITION
5. ROLE OF DEPARTMENT
The function of the pharmacy service is to provide integrated patient focused care that meets the present
and anticipated needs of the NHS Lothian population in line with local and national strategies. The
service promotes and develops all branches of the pharmacy profession through a co-ordinated
approach to delivery of pharmaceutical care to patients and the public.
Pharmaceutical care reflects a systematic approach that makes sure that the patient gets the right
medicines, in the right dose, at the right time and for the right reasons. It is about a patient-centred
partnership approach with the team accepting responsibility for ensuring that the patient’s medicines are
as effective as possible and as safe as possible. This is done by identifying, resolving and preventing
medicine-related problems so the patient understands and gets the desired therapeutic goal for each
medical condition being treated.
The focus is on active participation in and contribution to multidisciplinary/multiprofessional teams in a
manner to fully integrate pharmaceutical skills and resources in overall context of health and social care
in Lothian.
To this end the objectives are:
1. To provide pharmaceutical care to individual patients wherever they present by meeting their
particular needs while maximising efficiency in the use of resources.
2. To provide medicines through systems of quality control that ensures safe, effective and economic
use.
6. KEY RESULT AREAS
6.1 Strategic Leadership (50%)
In conjunction with the Lead Antimicrobial Pharmacists to lead and co-ordinate the strategic development
of antimicrobial prescribing management across NHS Lothian:
1. To be the Antimicrobial Pharmacist for the Antimicrobial Stewardship ward-round on the acute sites
to ensure prescribing of antibiotics is optimised, use of intravenous antibiotics minimised and
development of antibiotic resistance reduced where possible.
2. To develop, implement and monitor the work plan for antimicrobial prescribing management, in
partnership with other relevant committees and stakeholders.
3. To devise and implement antimicrobial policies, guidelines and delivery strategies for Hospitals and
General Practice which take account of local and national objectives, evidence-based practice and
local resistance profiles to ensure the safe, clinically effective and cost-efficient use of these
medicines. This includes implementation of the recommendations in the Antimicrobial Prescribing
Policy and Practice (APP&P 2005) and the Scottish Management of Antimicrobial Resistance Action
Plan (ScotMARAP 2008).
4. To ensure antimicrobial pharmacy services develop to meet patient needs, national policy and the
overall strategic aims for antimicrobial utilisation.
5. To ensure safe and cost effective clinical practice with antimicrobials and, through clinical
governance and risk management systems, ensure national standards are applied and maintained
locally.
6. To co-ordinate the reporting arrangements on the impact of the AMT on the costs and quality of
antimicrobial prescribing.
7. To work with senior medical and pharmacy staff to oversee the managed entry of new antimicrobials
within local systems for formulary management.
8. In conjunction with the Area Drug and Therapeutics Committee to co-ordinate audit of antimicrobial
prescribing to assess adherence to local guidelines and the NHS Lothian Formulary
recommendations.
9. To provide an expert level of clinical leadership regarding antimicrobials to other clinical practitioners.
10. To take a lead role in audit and evaluation of clinical practice in antimicrobial use in line with national
standards and frameworks.
11. To take a lead role in the development of a research agenda associated with optimal antimicrobial
use.
12. To provide individualised prescribing advice and reports to GP’s (including GMS
Prescribing action packs related to antimicrobial prescribing) and support to primary care
Prescribing advisers particularly with respect to the inclusion of infection related targets in
GP contracts.
6.2 Resource Management (10%)
1. To monitor and report on expenditure on antimicrobials, identifying areas for cost improvement,
where clinically appropriate, and contributing to the wider budget setting process.
2. To provide expert advice on horizon scanning to the Director of Pharmacy, Clinical Management
Teams and Area Drug and Therapeutics Committee regarding current and anticipated demands for
antimicrobials to deliver a safe and effective level of service to patients.
6.3 Clinical practice (20%)
1. To provide specialist pharmaceutical care to defined groups of patients with infection within NHS
Lothian. This includes:
a) Review of patient medical notes and medicine history
b) Interpret test results and other patient clinical factors to ensure safe and effective treatment for
individual patients
c) Determine potential drug interactions or contraindications
d) Review and verify individual patient doses where many of the medicines are used within a narrow
therapeutic range
e) Interpret disease symptoms and review toxicities of medicines
f) Review and interpret response to treatment
g) Recommend appropriate dose or therapy changes
h) Provide advice to all health care professionals and patients on all aspects of drug therapy and
medicines usage
i) Participate in consultant ward rounds and multidisciplinary meetings
2. To supervise junior clinical pharmacists and technical staff rotating through the clinical area as
allocated by the principal pharmacists. This includes prioritising and allocating work based on patient
need to ensure the continuity of the clinical pharmacy service.
3. To identify and develop opportunities for pharmacist supplementary and independent prescribing of
antimicrobial agents.
4. As part of the role as Antimicrobial Pharmacist on the Antimicrobial Stewardship ward-round:
a) To provide advice and influence senior medical staff to alter prescribing of antimicrobial agents
such that adherence to the UHD Antimicrobial Prescribing Guidelines and Alert Antibiotic Policy is
promoted.
b) To monitor and promote the principles of prudent antimicrobial stewardship to prescribers
including intravenous to oral switching of antibiotics asa soon as possible, daily review of
antibiotic therapy and cessation of unnecessary use of antibiotics.
c) To provide evaluated reports at the required frequency detailing the impact of the Antimicrobial
Stewardship ward-round on key prescribing measures, prescribing indicators and antibiotic
expenditure. These reports will be used by Senior Management to evaluate the effectiveness of
the ward-rounds in achieving the aims of this Invest to Save initiative.
6.4 Education and training (10%)
1. In conjunction with the AMT, identify training needs, develop, co-ordinate and deliver education
on antimicrobials for all clinical staff in NHS Lothian including doctors, pharmacists and nurses,
on an individual or group basis to improve antimicrobial utilisation.
2. To plan, develop, implement and evaluate the effectiveness of training in optimal utilisation of
antimicrobials for all grades of pharmacists including undergraduates, pre-registration
pharmacists and those undertaking post-graduate courses.
3. To prepare and deliver training programmes giving highly specialised information and advice
about medicines to patients and all grades of healthcare staff.
4. Responsible for own continuing professional development in line with the Royal Pharmaceutical
Society of Great Britain Guidelines and Code of Ethics.
6.5 Research, evaluation and audit (10%)
1. Participate in a research programme focussed at patient and population level to improve the safe,
clinical and cost effective use of antimicrobials.
2. Audit antimicrobial prescribing in relation to local guidelines and formulary management.
7a. EQUIPMENT AND MACHINERY
1. Regular use of JAC pharmacy stock control and dispensing reporter to produce reports on medicines
utilisation and expenditure.
2. Regular use of APEX laboratory system to access laboratory reports for individual patients.
3. Regular use of Microsoft Office computer software for email and internet/intranet enquiries, word
processing to produce documents, spreadsheets and reports of management information and
statistics, production of PowerPoint presentations for staff and development of databases for audit
purposes.

8. ASSIGNMENT AND REVIEW OF WORK
1. Works autonomously to meet objectives agreed with the Director of Pharmacy via the Lead
Antimicrobial Pharmacists.
2. Workload is mainly self-generated, influenced by patient and service needs and local and national
agenda especially national strategies for reducing antimicrobial resistance.
3. Accountable for own professional actions and outcomes – guided by legislation, national and local
policies and the local formulary.
4. Plans and prioritises own workload and that of senior and junior pharmacists rotating through the
clinical area.
5. Reviews and agrees work plan priorities through monthly meetings with the AMT.
9. DECISIONS AND JUDGEMENTS
1. The post-holder has a high degree of authority and accountability and will be responsible for the
interpretation of national/regional strategies and policies that inform the local priorities for
antimicrobial prescribing.
2. The post-holder is expected to anticipate both clinical and management problems/needs and resolve
these in a proactive independent manner.
3. Whilst not directly holding a budget, the post-holder will monitor expenditure on antimicrobials and
advise on trends for future budget planning.
4. Accountable for own professional actions guided by national, organisational and professional
policies.
5. The post-holder is expected to advise and influence nurses and clinicians regarding treatment of
individual patients on a daily basis, providing advice on therapy and dosage especially in highly
complex patients.
6. The post-holder is expected to advise and influence the Director of Pharmacy and Senior
Management regarding antimicrobial use.
10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB
1. To motivate a wide range of clinicians and healthcare professionals in an environment of conflicting
priorities in order that antimicrobial utilisation in NHS Lothian is improved.
2. To manage, analyse and act when faced with difficult and ambiguous problems where there is
sometimes limited information to guide decisions.
3. To influence others to comply with policies and accept antimicrobial resistance as a high strategic
priority.
4. The post-holder will need to balance the competing demands of strategic development, clinical
practice, Education and Training, audit and evaluation and national liaison. This will require the ability
to prioritise workload and work towards short timescales within a stressful environment.
11. COMMUNICATIONS AND RELATIONSHIPS
1. The post-holder will be an expert clinical practitioner and will be required to challenge information and
decisions made by senior staff, which may not be well received, and have to negotiate and influence
to obtain a satisfactory outcome.
2. The post-holder requires excellent interpersonal skills to motivate staff and communicate with a wide
range of people from all disciplines and professions within NHS Lothian including clinicians,
pharmacy staff, nursing staff and other healthcare professionals to ensure compliance with
antimicrobial policies and protocols.
3. The post-holder requires teaching, presentation and mentoring skills, at local and national level, to
fulfil the role of developing and supporting staff in an often stressful and changing environment. The
post-holder is required to communicate effectively with a variety of audiences in various settings to
present and discuss clinical information, patient cases or service provision. This includes one-to-one
tutoring, local group workshops and presentations at national meetings.
4. The post-holder must communicate effectively with patients, families and carers, some of whom may
have barriers to understanding e.g. sensory impairment, using verbal/non-verbal, written and
presentation skills as required. In addition, the post-holder will be required to instruct, reassure,
encourage and guide patients/carers and other healthcare staff in the prescribed treatment.

Physical demands:
1. There will be frequent travelling within Lothian, across Scotland and occasionally across the U.K. and
internationally for meetings.
2. Use of Visual Display Unit equipment involving computer keyboard skills for long periods on a daily
basis.
3. Standing for periods of time to review inpatient drug charts and patient case notes as part of the
activities of the Antimicrobial Stewardship ward-round.
Mental/emotional demands:
1. Clinical work requires a high level of concentration characterized by assessing different pieces of
sometimes conflicting information and forming a clinical opinion. This is often conducted under time
pressure and subject to frequent interruptions.
2. Requirement to concentrate for long periods of time and apply highly focussed mental attention at all
times when reviewing patients’ antimicrobial treatment as part of the Antimicrobial Ward-Round.
3. Requires confidence to contribute and lead in clinical and managerial settings. The post-holder will
need to demonstrate leadership skills and be self-motivating within a wide ranging peer group.
4. Direct contact with patients (and their carers) who can be suffering very serious or terminal disease
or may be emotionally distressed.

13. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB
Qualifications required:
MPharm degree or equivalent

Postgraduate qualification in clinical pharmacy (MSc, PhD or Diploma)
Independent prescriber accreditation or working towards achievement of accreditation with RPharmS

Diseases specialty.

NHS SCOTLAND APPLICATION PROCESS
The purpose of an application form is to help evidence that the applicant has all the requirements applicable
to carry out the job applied for.
Once in receipt of the application pack it is essential to read both the job description and person specification
to gain a full understanding of what the job entails and the minimum criteria required.

Only part C of the application form will be sent for short listing. The application form will be identified by
the candidate number only to ensure that no applicant will be unfairly discriminated against.

Please complete all sections of the application form. Those sections that are not relevant please indicate
‘not applicable’, do not leave blank.

Please feel free to use additional paper if required. Please do not add your name to any additional
information provided; secure it to the relevant section and Recruitment Admin will add a candidate
number.
