PARKER HEALTH & REHAB RECEIVES NATIONAL ACCREDITATION
FOR QUALITY ASSURANCE AND PERFORMANCE IMPROVEMENT
Facility also earns 2014 Customer Satisfaction Award
(Parker City, Ind.) – Parker Health & Rehabilitation Center, a TLC Management facility, recently earned
the 2014 Providigm Quality Assurance and Performance Improvement (QAPI) Accreditation as a skilled
nursing facility that is actively performing quality assurance and performance improvement activities.
Parker also earned a 2014 Embracing Quality Award in the area of Customer Satisfaction from
Providigm, which means the facility not only met accreditation standards, but demonstrated excellence
in areas that are critical to the care of elders and earned a high endorsement score.
QAPI Accreditation demonstrates that a facility is continually assessing residents’ quality of life with
regard to concerns such as pain, dignity, respect for resident choice, and quality of care problems such as
whether there are enough staff to meet resident needs, weight loss, infections, rehabilitation following
acute injury or illness and prevention of readmissions to hospital.

We work hard to give our residents a safe, comfortable place to live and it is gratifying to have that fact

TLC Management, located in Marion, Ind., is a family-owned company founded on Christian values and
principles. A full continuum of long-term and short-term health care services is provided through its
skilled nursing, rehabilitation centers, assisted living and retirement centers. Specialized hospice
