Traditional Scholarship
This award may only be used to defray expenses for a graduating high school senior or college freshman
enrolled as a full time student with a minimum of 12 hours at an accredited school. Applicants must be a
NASCOE member’s child or member’s legal dependent. The NASCOE member’s dues must be paid
and membership kept current for the past 5 years, or if less than 5 years, continuous membership has
been maintained since becoming a NASCOE Member within the first 12 months of becoming a
permanent employee (8th Amendment; updated September 26, 2012). The evaluation criteria are based
on ability, incentive assistance, and other personal characteristics. Nominations for Scholarships are
competitive. Area winners are selected from State entries. The National winner is selected from Area
winners. The time schedule contained in this booklet must be followed. This scholarship can only be
won once at the Area and National level.
INSTRUCTIONS:
1. COMPLETE THIS APPLICATION IF YOU MEET THE CRITERIA ABOVE AND ENSURE ALL
SIGNATURES HAVE BEEN OBTAINED. YOUR APPLICATION MUST BE RECEIVED BY YOUR
STATE SCHOLARSHIP CHAIRPERSON NO LATER THAN January 1st
2. Include an Official transcript (A scanned copy is acceptable. Please use a permanent marker
prior to scanning to ensure any sensitive data – i.e. Student ID and/or Social Security number
are not readable by anyone) of your high school and/ or college grades. All applicants must submit a
letter of recommendation (A scanned copy is acceptable).
3. ALL grade point averages must be on a 4.0 scale. Letter or numerical grades shall be converted by
your counselor.
4. Area winners will be asked to submit a recent color photograph. NOTE: A photo is not required at
the time of application, ONLY if you are announced as the Area winner.
5. PLEASE type or print legibly in ink.
6. Keep a copy of this application for your records.

The following questions are to be answered as your application process. Please copy

Microsoft Word format. Please include this page (with your signature and date at the
bottom) with your application.

2. Please list the schools you have or plan to attend. For any schools you have
attended, please list the dates attended and grade point average.
3. Please state the name of the college, university or trade school you plan to
attend. Please indicate if you have been accepted for enrollment. What do you
intend to study? What are your plans for your future?
4. Describe the honors and awards you have received and why you were chosen by
your peers, faculty and organizations to receive the honor/award.
5. Briefly describe your involvement in scholastic and non-scholastic organizations.
Please list all positions of leadership and give a brief description of the
organization.
6. Please list the extracurricular activities you participate in. Please list all positions
of leadership and give a brief description of the activity. If you have earned any
awards or honors in this activity, please list and describe those as well.
7. Please list all community involvement and activities. List the year, name of
activity, nature of activity, offices held and any awards received as well.
8. Tell us about yourself. What are your hobbies? What do you do in your spare
time? This is your opportunity to let the judges get to know you.
