RE: ASC BILLING REQUIREMENTS
Claims details submitted from Ambulatory Surgical Center providers for dates of service on or after 7/1/13 require billing of modifier SG, “ambulatory surgical
center (ASC) facility service.” Stand-alone revenue codes that represent dental surgery are NOT to include a procedure code or a modifier.

RE: ARKIDS-A 61N CO-PAY MASS ADJUSTMENT
A mass adjustment to correct $10.00 co-pays assessed in error on AID Category 61N ARKids-A since January 1, 2014 will be processed the weekend of June 27,
2014. The adjustment will be reflected on the July 3, 2014 RA. If you have collected any co-pays from the guarantor, you will need to reimburse the co-payments.
