direction of a fellowship trained movement disorders neurologist. This neurology group practice includes nineteen

Rockford, IL – Palliative Care: The Palliative/Supportive Care Nurse is an Advance Practice Nurse (APN) who
provides clinical support, case management, consultation, education and program development functions for
OSF Saint Anthony Medical Center. The Palliative/Supportive APN responds to referrals and provides care and
symptom management for patients and family/ significant others across the life span. The Palliative/Supportive
Care APN collaborates with physicians in providing care and symptom management based on physical,
psychosocial, educational, safety, and related criteria appropriate to the age of patients served. The APN helps
the patient develop an end of life plan that meets their wishes. The APN assists patients and families with post
hospital discharge care planning as necessary and functions as case manager for patients of Saint Anthony
Medical Center Palliative/Supportive Care Program. The APN will provide clinical consultation in symptom
management. The APN in Palliative/Supportive Care, in collaboration with the Medical Director of the
Palliative/Supportive Care, facilitates program development by working with the interdisciplinary Supportive
Care team. The APN’s activities include writing evidence-based policies and procedures as required, internal
and external marketing, and meeting with key stakeholders. The APN will develop and present educational
programs and provide mentoring for nurses, physicians, other clinicians, patients and families in evidence-based
palliative/supportive care. The APN will participate in quality improvement and research activities as applicable.
