Wessex Deanery is a relatively small deanery with a defined geographical area
which does lead to there being a single unit of application. In the majority of
cases successful candidates will be asked to preference their choice of location
for either one or two years. Some specialties will require successful candidates to
preference both commencing location and specialty. Future placements will be
based, as normal, on individual training and educational needs. Please note that
applications are to the Wessex Deanery as a whole. This may mean that you
may be allocated to any geographic location within the Wessex Deanery
depending on training needs.
Programme Description/kbrown/Sept08
Programme Description
The Wessex Deanery covers a geographical area from Basingstoke in North
Hampshire to Dorchester in West Dorset and the Isle of Wight to the South; in
addition some programmes rotate to Jersey and Chichester in West Sussex. This
is a spread of approximately 65 miles North to South and 76 miles East to West.
The Wessex Deanery serves a population of around 2.8 million people.
The Wessex Deanery is part of NHS Education South Central within South
Central Strategic Health Authority which covers Berkshire, Buckinghamshire,
Oxfordshire in the north (under Oxford Deanery) and Hampshire and Isle of
Wight. In addition, Wessex Deanery provides training programmes within Dorset
and South Wiltshire under a formal agreement with the South West Strategic
Health Authority. The Wessex Deanery is responsible for the training of some
2,500 trainees.
The Wessex Child & Adolescent Psychiatry SpR rotational training scheme
covers Poole (total population 140,000) and part of East Dorset (approximate
population 40,000), Southampton and the surrounding areas (total population
387,000), Portsmouth and the surrounding areas (total population 545,320) and a
post for a flexible trainee in Salisbury (total population 120,000). In other words
the scheme covers a lovely part of the South Coast with all the benefits of the
sea, beautiful countryside and historic cities. The Scheme consists of 8 full-time
SpRs, one being a lecturer’s post based in Southampton. Usually, 5 SpRs rotate
between Southampton and Portsmouth, spending 2 years in one place and one
year in the other and 2 SpRs rotate between Poole and Southampton. However,
there is flexibility within this system to meet individual trainees needs. The
Scheme supports flexible training and can provide for some additional flexible
trainees. Child & Adolescent Psychiatry SpR training to obtain a CCT in Child &
Adolescent Psychiatry is 3 years full-time or pro-rata equivalent.
Aims and Objectives of Training and Education
The Scheme aims to provide and promote training, education and professional
development in Child & Adolescent Psychiatry to a standard expected by the
Royal College of Psychiatrists (currently based on College Advisory Papers and
Annual RITA Appraisal Form), and the GMC Guidelines for Good Medical
Practice. At the end of their training specialist registrars will be expected: to have
gained a thorough knowledge of normal child development; be able to assess
children and families in order to make a diagnosis of psychiatric disorder if
present and to be able to formulate the presenting problems and to develop
treatment plans; work as a co-therapist with other members of the
multidisciplinary team; have experienced a wide range of treatment modalities
such as behaviour therapy, family therapy, cognitive therapy, solution focused
therapy, brief focused individual therapy, long-term individual psychotherapy,
group therapy and psychopharmacological treatments; worked within
multidisciplinary teams; worked with other agencies (for example, attended
Social Services case conferences, convened multi-professional planning
meetings and provided assessments for Statementing procedures); provided

Programme Description
consultation to other agencies and professionals, eg Social Workers, Educational
professionals, voluntary agencies; dealt with clinical emergencies including
deliberate self-harm; and provided psychiatric assessments for the courts.
Additional Training and Education
Trainees will be supported if they wish to undertake further training, education or
qualifications by recognised bodies, eg family therapy training, cognitive
behaviour therapy training, an MSc or an MD. However, there is no automatic
additional funding available, apart from each SpRs individual study leave budget.
The Academic Programme
Trainees will be expected to attend this 3 year rolling Academic Programme
which is held at Ashurst Hospital, Southampton on the 2nd and 4th Wednesdays
of the month during term-time. The Programme includes a 2-3 day long
experiential workshop per year, a Spring and Autumn Academic day under the
auspices of the Southern Branch of ACAMH, and attendance at the academic
component of the Wessex Child & Adolescent Psychiatrists’ meetings. The
development and delivery of the programme is a combined effort between
trainees and the Academic Course Organiser. It includes seminars, journal
reviews, clinical case and video presentations, research meetings, outside
speakers and a Balint group. It is also an opportunity for trainees to meet
together as a group. Time to attend the academic programme will be protected.
Each placement has a Bench Library as well as access to University/Hospital
libraries.
Research
Trainees will have 2 protected sessions per week for research. This time should
be used to develop a research project, either one devised and set up under the
SpR’s initiative, or one that is worked as a combined project with another SpR
and Consultant, for example: an on-going project; a thesis that contributes to an
MSc; an extensive review of the literature written up for an MSc or for publication;
an extensive evaluation project evaluating services that has a cyclical element to
it or a planned series of evaluations; an extended audit which the SpR would
take at least through one, if not two cycles. SpRs will meet with the Research Coordinator at the beginning of their training and a project and Research Supervisor
will be agreed. SpRs will be expected to demonstrate appropriate use of
research sessions at their annual RITA panels. The local RDSUs offer a range of
research courses and research assistants at Ashurt Hospital are available to help
with statistics and setting up SPSS data bases etc. Flexible trainees may take
some research sessions as a 6 month block.
The research interests amongst the academic and consultant group are: ADHD,
pre-school children, autism, chronic fatigue syndrome and service evaluation.
There are good links with the psychology department in the University of
Southampton.

Programme Description
Audit
Audit plays a crucial part in Clinical Governance and SpRs will be expected to
participate in audit training and audit projects in each of their placements.
Teaching
There are opportunities to teach medical students from Southampton University,
SHOs in Psychiatry, other doctors eg GP trainees and Paediatricians, and other
members of related professions, eg Health Visitors, Social Workers, Teachers,
Nurses, Midwifes, etc, members of the multidisciplinary CAMHS teams and nonprofessionals, eg support groups, parents, school children. Different modalities of
teaching are encouraged and live supervision of teaching takes place. Patients
and their carers should be involved in teaching/training.
Management Training
SpRs will be expected to undertake both formal and informal management
training. They will also have an opportunity to attend courses on Continuing
Professional Development through the Wessex Courses Centre.
Mentor
The Mentor Co-ordinator offers new SpRs a list of volunteer Mentors from the
Consultant body from which they can choose. It is entirely voluntary on both
sides but whilst a Mentoring arrangement is in existence, the Mentor undertakes
to make at least 2 sessions per year available to the Mentoree, each of which
should be at least one hour undisturbed time.
Links with Adult Forensic Psychiatry
For SpRs who wish to become Forensic Child & Adolescent Psychiatrists (single
CCT in Child & Adolescent Psychiatry) we have good links with West Hampshire
Mental Health Trust Adult Forensic Psychiatry department at Ravenswood
House, Fareham. We have been able to arrange for individual trainees to
undertake an additional 2 years Forensic Training, 1 year in Adult Forensic
Psychiatry and 1 year Adolescent Forensic Psychiatry, in addition to 2 or 3 years
Child & Adolescent Psychiatry SpR training. This training is arranged on an
individual basis with specific approval from the Wessex Deanery and the Royal
College of Psychiatrists.
Emergencies
The Specialist Registrar accepts that s/he will also perform duties in occasional
emergencies and unforeseen circumstances at the request of his/her colleagues
both senior and junior. It has been agreed between the professions and the
department that while juniors accept that they will perform such duties, the
Secretary of State stressed that additional commitments rising under this subsection are exceptional and in particular the junior should not be required to
undertake work of this kind for prolonged periods or on a regular basis.

service provides therapeutic input to adolescents with complex needs who
require a level of care not usually provided by a traditional CAMHS out-patient
service. Community outreach can be provided where there is poor engagement
with traditional services.
There are teaching opportunities including presentations at the junior doctors’
journal club and case presentation meetings at St Anne’s Hospital. The
opportunity for examination coaching of SHOs planning to undertake MRC Psych
examinations is available.
The Shelley Clinic, Boscombe

substance misuse training. Mental Health Services for Child and Young People
with Learning Disabilities are provided by the Learning Disabilities Team. There
is an Outreach Team and a Learning Disabilities Psychology Service. The SpRs
have an opportunity to gain experience with children with learning disabilities.
Portsmouth and South East Hampshire
In this placement the SpR is employed by East Hampshire Primary Care Trust.
There are 3 Community Child & Family Therapy Services in the Portsmouth and
East Hampshire area which all provide a similar service and the SpR is based at
one of these clinics. Although the 3 clinics are managed by different PCTs, there
are close clinical and managerial links across all the 3 clinics. The clinics receive
referrals of moderate to severe child and adolescent mental health problems (ie
Tier 3) but each area works closely with their own team of Primary Mental Health
Workers who see children in the mild to moderate range of difficulties (ie Tier 2).
The clinics are supported by the District Unified Adolescent Team, which sees
young people between 14-19 years who have complex needs (tier 3-4). The
Trusts have contracts with Leigh House Inpatient Adolescent Unit in Winchester
(tier 4). There is a small district Paediatric Liaison Team which provides specific
interventions ranging from prevention to tier 4 work.
Each Community Child & Family Therapy Service sees patients up to 16 years of
age and/or till the end of compulsory education. Each clinic receives
approximately 500 referrals per year from a wide range of professionals. They
each have a multidisciplinary team which includes Child & Adolescent
Psychiatrists, Clinical Psychologists, Community Nurse Specialists, Family
Therapists, Psychotherapists and administrative support. The Fareham and
Gosport clinic also has an Art Therapist with some district role. In each clinic the
SpR has the opportunity to gain experience in family therapy, cognitive behaviour
therapy, individual psychodynamic psychotherapy, the creative therapies, brief
focused therapies and solution focused and narrative therapies. The SpR is also
able to gain experience in psycho-pharmacological therapies, extensive
consultation to other agencies and liaison work.
The SpR has an opportunity to work with the Psychology team which provides
mental health input to children with learning disabilities at the Child Development
Centre in Portsmouth. He/she also has the opportunity to observing assessments
by the Autism Assessment Service. All teams place a high priority on both
individual and team supervision and the Trainee is able to work as a co-therapist
with a range of professionals. The SpR has the opportunity to provide
supervision for an SHO in child psychiatry at Havant Child & Family Therapy
Service (on the Solent rotation). The SpR also has an opportunity to teach
medical students, present clinical cases at academic meetings and teach nonmedical professionals.

Specialist Placements
Unified Adolescent Mental Health Team, Cosham Health Centre, Portsmouth
The specific remit of this team is to work with high risk, disaffected young people
in many settings. Almost by definition this is a forensic population and one that all
agencies find very hard to engage and manage. The necessary processes of
assessment and management are particular and comprehensive. Networking
tasks are central to success.
The trainee is involved in the multidisciplinary working of the complex caseload
held by this service. Such work will involve assessment, therapy and
management of the many facets of the cases. Supervision of the trainee’s work
will be enhanced by the multidisciplinary team’s own supervision resources
(internal supervision expertise, and external supervision of the team). Although
this is a community team, strong links to other agencies, in particular
Portsmouth’s adolescent drug and alcohol treatment service (E’s Up) will ensure
plenty of relevant opportunities for visits to and work with our cases within
prisons, secure units, children’s homes, hospitals (adult psychiatric, forensic,
adolescent psychiatric, adolescent forensic), various special education resources
and other important organisations in the area. Because the service is closely
linked to the Portsmouth District Youth Offending Teams, Court experience and
the production of Court reports is possible.
There are opportunities for providing consultation to other agencies, clinical
supervision of work performed by other agencies, and training of other agencies
alongside other team members. The trainee is expected to be involved in
managerial and internal training aspects of the team.
Paediatric Liaison Team
In this placement the SpR is based in Portsmouth City CFTS, but he/she spends
most of the clinical time at St Mary’s District General Hospital, Portsea Island or
Queen Alexandra District General Hospital, Cosham. The SpR works with the
district Paediatric Liaison Team consisting of sessional time from a Child and
Adolescent Psychiatrist, Community Nurse Specialists, a Primary Mental Health
Worker and a Psychotherapist. The clinical tasks include: helping to facilitate the
psychosocial ward meeting with the Consultant and the IMPACT groups (for
children with long-term medical conditions); seeing patients on the Paediatric
wards; and offering consultation and direct clinical work to the Respiratory,

Neurology and Oncology Specialist Children’s Teams. The SpR also has the
opportunity to join the Psychotherapist for attachment work on the Neonatal Unit.

learning disability with the CDC Psychologists. The SpR has opportunities to
teach Paediatric Nurses and Paediatricians, particularly in delivering the “Child in
Mind” teaching to SHOs in paediatrics.
Salisbury Health Care NHS Trust

and has close working relationships with Paediatrics. There are opportunities for
teaching medical students, and SHOs in Paediatrics and Psychiatry, including
case presentations to the team, Paediatric and Adult Psychiatric departments.
Outpatient clinics, including ADHD clinics are held at Salisbury District Hospital
and there are peripheral clinics in Amesbury and Ringwood. There is the
possibility of working with a Primary Mental Health Worker in the Amesbury area.
The SpR has an opportunity for Paediatric Liaison work with the Paediatricians at
Salisbury District Hospital, one of whom has a special interest in head injury, and
the opportunity to work with regional specialist units such as the Burns Unit and
Spinal Unit. TheSpR is able to work with the Child Development Team and the
Learning Disability Team, with visits to specialist units, Hillcote Residential Unit
and Exeter House School. Experience in the assessment of Autistic Spectrum
Disorders is also available. The SpR will gain experience in family therapy,
cognitive and behaviour therapy, group work and use of psychotropic medication,
as well as consultation opportunities to other professionals, e.g. Social Workers.
There are good links with the Youth Offending Team and Adult Drug and Alcohol
Service.

service for children 0-16 years based in the grounds of Southampton General
Hospital and has input from Paediatricians, Psychiatrists, Psychologists,
Occupational Therapists, Psychotherapists, other therapists and a small school
which has become a Pupil Referral Unit. Patients with a range of mental health,
paediatric or mixed paediatric/mental health problems are referred, including
those with psychosomatic disorders, chronic fatigue syndrome and neurological
problems. Patients are referred from a wide range of geographical areas from
psychiatrists and from acute and community paediatricians. Admissions range
from short term assessments to longer term assessment and management,
including rehabilitation eg following severe head injury. There is very close
working between paediatricians and mental health professionals.
Southampton General Hospital has a wide range of secondary and tertiary
paediatrics, including respiratory, neurology, oncology and endocrine services
and a paediatric intensive care unit. The paediatric liaison team provides input to
all paediatric services in the hospital. On the paediatric wards SpR assesses
inpatient and out patient referrals from the Paediatricians, together with
Paediatric Liaison Team. Referrals range from cases following deliberate self
harm to those with longstanding and complex chronic illness. The SpR also
provides consultation to the various paediatric services and displines and some
staff support. The SpR has the opportunity to attend the Consultant Paediatric
Neurology clinic and other paediatric clinics. There is a teaching component in

mixture of residential Social Workers and RMNs staff the residential team. The
community team has approximately 12 full time equivalents including a
Consultant Psychiatrist, Psychologists, Learning Disability nurses, Social
Workers, a Teacher, Educational Psychology time, a Family Therapist and
Paediatrician time. The community team provides an assertive outreach
approach adapted from the model of multi-systemic therapy.

60% are looked after children. The service has no specific exclusion criteria and
accepts young people with multiple difficulties in every arena of their life. This
placement provides a unique opportunity for detailed multi-faceted assessment of
young people and their environment. There are links with Southampton City’s
Adolescent Drug and Alcohol Treatment Service (E’s Up) and Hampshire’s
Cranstoun Drug and Alcohol Services. The multi-agency work also includes
assessment and treatment of young people with a history of offending and
forensic concerns. There is an opportunity for undertaking medico-legal work
particularly regarding young people involved in the criminal justice system.
Brookvale Adolescent Service, Southampton
The Brookvale Service provides an experienced and specialised community
psychiatric service for adolescents with mental health problems. The service
provides for young people between the ages of 14 and 18 years, their families
