ROLE PURPOSE:
The Registered Nurse – Community Paediatrics is responsible for the service
delivery of the Short Term Transitional Respite Care for Medically Fragile
Children contract in the Waikato area. The role provides support to
families/whanau, enabling them to care for their child/children, aged from birth
to 14 years of age, in their own homes and local communities.
Healthcare NZ Community Health covers different modalities, and increasingly
more acute and complex clients are being managed in their own homes,
therefore enhancing the role of the specialist generalist with a broad range of
skills. Services need to be flexible, responsive and safe and enhance the
quality of life for the client and their family/whanau.
The Registered Nurse – Community Paediatrics will work very closely with
referrers, primary care providers, multi disciplinary teams and non government
organisations to provide support that focuses on the child within the context of
the family/whanau.

The Registered Nurse will work closly with Healthcare NZ Community Health
Service Coordinators and Support Workers, family and whanau to provide
education, training, competency sign off and direction and delegation to ensure
the client and family/whanau needs are met as per the assessment and
service plan.
The position will identify service gaps, client/family/whanau safety and clinical
risks and seek opportunities to manage and mitigate them thereby ensuring a
consistent high quality standard of care, support and that contractual
requirements and outcomes are met.
The incumbent will provide relief cover for the Registered Nurse- Community,
covering the Medication Oversight contract, assist with the assessments and
reviews of high needs clients; to establish a client focused, home support
service plan for service delivery and provide assistance to the Community
Service division as requested.

PRINCIPAL ROLE ACCOUNTABILITIES AND EXPECTATIONS:
Service Delivery
 To ensure the aims and objectives of HealthCare NZ Community Health are achieved
in the provision of services for clients/family/whanau.
 Uses advanced nursing knowledge and skills to assess, plan, implement and review
referred paediatric clients; establishing a client focused, home support service plan
for service delivery.
 Monitors clinical outcomes managing variances in conjunction with the Paediatric
Home Care Nurses and multidisciplinary team providing integrated care and
support.
 Assists family/whanau with bereavement processes associated with their child’s
condition and impact on family life.
 Assists with assessments and reviews of high needs clients to establish a client
focused, home support service plan for service delivery.
 Accurately and timely documents all information in client records in accordance with
HCNZ policy and procedures/timeframes and Nursing Council requirements.

Manage rosters of Support Workers in conjunction with Service Coordinators
ensuring that clients needs are met by staff who have the appropriate skills and
experience.
Clinical and Professional Leadership

Acts as a nursing resource, providing expert advice and education to family/whanau
and support staff within scope of practice.

Practices in accordance with legal, ethical, cultural and professional standards.

Ensures best practice guidelines/policies and procedures are followed and
reviewed in relation to service delivery.

Provide relevant reports on a quarterly basis to the DHB, and other documentation to
management/ service coordinators, as may be required to meet HCNZ Community
Health contractual reporting requirements and client service delivery
External Relationships

Facilitate and manage the monthly stakeholder meeting for the Medically Fragile
Children Contract.

Develop and maintain strong relationships with referring agencies, prospective and
existing clients.

Work in conjunction with the Branch Manager and Community Services Division staff
to promote HealthCare NZ services.

Responsible for the quality of service delivery to clients, risk management and
resource utlisation.

Facilitate and contribute to carrying out clinical audits to evidence outcomes and
identify gaps.

Participate in Healthcare Holdings Ltd (HHL) Group Clinical Governance working
groups as requested.

Complies with the HHL Incident reporting process.

Review and update HHL Group Paediatric policies and procedures as requested by the
Director of Nursing or delegate, ensuring they meet best practice guidelines.

PERSON SPECIFICATION:
Essential skills, knowledge and experience

Is a Registered Nurse with proven expertise in paediatric nursing

Must have a current annual practicing certificate with the Nursing Council
of New Zealand

Completed a recognised post registration qualification in paediatric
nursing or equivalent

Experience and knowledge of the primary/community health sector

Ability to coordinate care as one part of a continuum of services

Ability to take a leadership role and build excellent relationships

Competent computer skills

