INFORMATION ON OBTAINING REQUIRED DOCUMENTS
FOR THE CNA TRAINING PROGRAM
In order for you to begin the CNA program, you will need several required documents /
certificates. This is a quick guide in helping you attain those documents.
CRIMINAL ABSTRACTS:
You have two options to access a criminal abstract:
1. Use the public terminal located in the lobby of the Wailuku Police Station or
2. The State of Hawaii's online Adult Criminal Information Site. Located at
https://ecrim.ehawaii.gov/ahewa/
Fees: If you use the public terminal at Wailuku Police Station the cost is $25 per print out. Fees
associated with using the Adult Criminal Information site are posted on that site.
Payment: Criminal abstract printouts paid for in cash or money order/cashier's check made
payable to Hawaii Criminal Justice Data Center (HCJDC)

TUBERCULOSIS SKIN TESTING:
You are required to have two (2) TB tests resulting in a negative reading. If you have taken a
recent TB test that is less than a year old, that would count as one of your 2 TB tests.
If your test comes out positive, then you will need to take a chest x-ray. A written statement from
your physician that you had a positive PPD and must be written before the chest x-ray is taken.
If you are pregnant or are unable to take a chest x-ray, show your physician the result of your
skin test and have him/her provide you a written statement why you are unable to take chest xray, but that you demonstrate no symptoms of tuberculosis.
There are several places where you can have your TB done. If you are a Kaiser member, the
test is free and can be taken at any day. If you are not a Kaiser member, check with your
physician. However, the Department of Health (DOH) also provides free TB testing in Lahaina
and Wailuku on certain scheduled days.

Tests are given every Tuesdays between 2:00 and 4:00 p.m.
Readings are held on Thursdays between 2:00 and 4:00 p.m.

Readings are held on Thursday between 1:30 to 2:30 pm.
ADULT CPR & FIRST AID CERTIFICATION:
You can obtain your CPR and First Aid certificates online at the convenience of your
home. There are many websites to choose from. One of them is CPR and FIRST AID,
Inc. According to their website, you can obtain your CPR certificate for $14.99 and the
First Aid certificate for an addition $10.00. Each training is approximately 30 minutes
and is followed by a 20 questions exam. If you pass, you can print out your certificate
and have a wallet size certificate to carry in your purse/wallet. We are not affiliated with
CPR and FIRST AID, Inc., therefore you are free to search for other websites that fits
your needs. Of course there are CPR and First Aid courses that are “hands-on” if you
should choose to utilize that route.
PHYSICAL EXAMINATION
Please have your physician complete the “Physical Exam Form” from Hawaii Healthcare
Academy. If you are pregnant, you must have your OB-GYN complete the form. In
addition, you must have a Hepatitis B Vaccination or a Waiver.
