Admission Immunization Requirement Checklist
Use this checklist to track your admission requirements.
Actual copies of all immunization records are REQUIRED and MUST be completed and submitted no later than

IMMUNIZATIONS
√ Proof of MMR vaccination is REQUIRED. The 2-shot series OR proof of immunity by MMR Titer
MMR Shots –
This immunization is commonly done in early childhood. A Titer is only necessary if you don’t

Proof of Hepatitis B vaccination is REQUIRED. The 3-shot series OR proof of immunity by Hepatitis B Titer

Tdap
Tdap is a requirement of our rotation sites. Tetanus is not acceptable. This immunization must
be updated every 10 years.
√ Proof of Varicella vaccination is REQUIRED. The 2-shot series OR proof of immunity by Varicella Titer
Varicella Shots –
This immunization is commonly done in early childhood. Titer is only necessary if you don’t

have any record of the shots or choose not to receive the shots. Stating you had the disease is
not acceptable.

Influenza
Influenza
The flu shot is not a requirement of admission, but it is strongly encouraged that students
receive an annual flu shot. Students will be required to receive a flu shot prior to rotations.
√ Tuberculosis Testing (TB, PPD) is REQUIRED.
TB Skin Test
A 2-step TB skin test is REQUIRED.

less than 3 weeks, only a 1-step skin test will be required.
*If the time between the expiration date (1 year) of the 2-step skin test and the renewal skin
test is more than 3 weeks the 2-step skin test will need to be repeated. Make sure all annual
skin tests are done at the same time each year or close to the expiration date to avoid starting
the process over.
√ Positive TB Skin Tests
If a TB skin test is POSITIVE, you MUST provide documentation from a physician of a NEGATIVE chest x-ray along with a
physician’s verification of TB status prior to the start of the program. ALSO a TB Symptom Checklist must be completed
annually.
