1. What extra-curricular activities have you been involved in? Indicate leadership positions
you have held in those organizations. Please indicate dates of involvement in all
activities, as well as a brief description of the organization(s) and your involvement in the
organization(s).
2. What volunteer work (excluding academic internships) and service to the campus or
community have you engaged in? Please include dates of involvement and personal
contributions made (as opposed to service with a larger group), as well as a description of
the organization.
3. Describe your involvement in campus programming of events such as Activities
Commission, Inter Residence Council, Inter Greek Council, and other campus wide
events. Explain why and how you became involved? Explain the extent to which you
