2015-2016 Performing Arts Participation/Permission and Donation Slip

Please consider supporting your child’s arts education by making a donation to the LMMS Foundation. We would like to enhance
your student’s academic curriculum by exposing them to educational live performances. The LMMS Foundation would like to pursue
this local school donation so that all of the donations are used for our students. The Lost Mountain Middle School leadership team
(made up of teacher leaders, administrators, and counselors) can recommend performances that best fit the needs of our school
community. These performances may include in-house performances for our 6th, 7th, and 8th grade students, or possibly an off-site
event. Please help to support your child’s arts education by giving your tax-deductible donation. We hope that you will give the
donation of $14, but any donation will be greatly appreciated.
If you would prefer to donate to the Cobb County Performing Arts program, a flyer is included in the Cobb County student folder.
The money collected will be divided among all Cobb schools. Of course, you certainly may choose to donate to both Performing Arts
programs. 

Donations and permission forms are due between August 3 and September 4. Donations should be placed inside a white money
envelope and dropped in a black money box. The permission form is due to the homeroom teacher whether or not a donation is
made.
Checks should be made payable to Lost Mountain Middle School Foundation. Change cannot be returned.
