Terms and conditions
1. All requests should be signed by the requestor and will only be acknowledged via email submission.
2. All requisitions are subjected to the facility management’s approval.
3. Requisitions will be rejected if the protocol has expired or if the number of animals used to date has
exceeded.
4. You will be informed via e-mail/phone once the animals have been transferred.
5. This Animals Import Form should be submitted at least 1 month in advance of the estimated date that
the animals are required.
6. Animals are not allowed out of the Animal Research Facility unless prior approval has been granted by
the NTU-IACUC.
7. All animal testing costs recommended by the vet (if any) will be borne by the PI.
8. Please ensure that there is enough cage space within your allocated rack(s) to house the animals
requested.
We are agreeable to the terms and conditions stated on this Animals Import Form and will abide them.

We approve / reject this animal import. We declare that:
a) the animals will not be released following use at the laboratory facility and will be disposed from the
facility.
b) any suspected outbreak of disease will be reported to the AVA immediately and the laboratory
facility has a plan to contain an animal disease outbreak.
