CSC MEETING MINUTES
August 13, 2015 – 10:00 am
Facility Services Conference Room
Members Present Kathy Krebsbach, Chuck Emnett, Adrian Irish, Hannah Singleton, Marty Ludemann, Chuck Christensen, Chuck
Maes.
1. Introduction
Roll call: Sign in – No new introductions were made and the attendance form was signed.
2. Minutes approval
There were no changes to the June meeting minutes. Adrian Irish motioned to approve the
minutes and Marty Ludemann seconded the motion.
3. Safety Hazard (SH) Reports/Injury Summary Report
a. No Life Safety 24 work orders were issued in July. There were 21 Safety Issue work orders for
June through August 11th and only one has been completed to date. Chuck explained that with the
push to get the campus ready for fall semester, a greater number of buildings inspected during the
summer, and the response to the August 10th wind storm have slowed the progress of completing
the work orders.
b. Chuck reviewed the work comp claims with the group for July of this year with 13 in 2015 as
compared to 22 in July 2014. July had 6 notification only, 6 medical, and one indemnity claim.
The injuries in July 2015 encompassed strains, lacerations, contusions, and punctures with three
occurring during field research work.
4. Update on Safety Suggestion Form for Website
Adrian showed the Committee a couple of examples of Qualtric’s system online survey forms that
could be used as a template for the Campus Safety Committee website’s Safety
Concern/Suggestion form. Adrian pointed out that the Committee would need to decide on the
proper wording for the purpose of the form for the website link. The Committee seemed to favor
the simple on-page form and would just need to tweak the contents. Chuck Maes said that it could
be treated similar to a blog in responding to the submitting party. Chuck E. emphasized that it is
important to respond to the person sending in the concern that it was received and how it is being
acted on. Adrian will send the two examples of forms to Kathy for edits and to bring to the next
Committee meeting for a vote. Adrian mentioned that it would be nice if we could receive e-mail
notification of when a suggestion or concern was initiated. Kathy mentioned to Adrian that he
should get in touch with Tiffany Kuhn who knows Qualtrics very well and maybe able to assist in
determining if e-mail notification of an entry is possible. Kathy also mentioned that there a few
errors that need to be addressed on the Safety Committee’s website including updating current
Committee membership list.
5. Follow-up on Wind Storm 8/10/15
Kathy asked if there were any follow-up questions or concerns regarding the August 10th wind
storm that hit campus. Chuck Maes said that Dornblaser field got hit pretty hard. Kathy
responded that she and Chuck E. were out there taking pictures of the damage for insurance
purposes. Overall, Kathy mentioned that the Campus was lucky based on the number of trees that
fell but that did not hit buildings. Hanna reported that at Lommasson, the fire alarm went off at
6:20 a.m. and was on for 50 minutes during the power outage. Adrian said that all of the back-up
generators for IT worked and there was no loss of equipment. Marty informed the Committee
that the dispatch center was down for about an hour due to a problem with the generator at
Facility Services. Kathy mentioned that maybe the incident with the Facility Services generator
should be reviewed to prevent further future problems.

6. Other Items
Adrian brought up a safety issue of concern regarding the intersection of Maurice and Eddy
Avenues. It is difficult to see bikes at the intersection due to parking spaces close to the bike path
and the lack of visible and appropriate size signage at the intersection (stop signs). Marty said
that he will send Joe over from traffic enforcement to determine what is needed at that
intersection to make it safer, and will get it corrected before fall semester starts. Chuck
Christensen suggested that marking on the bike path itself with stop signage and possibly some
rougher material on the path before the intersection will help remind bikers to stop.
